The Approach of Czech Health System to Psychosomatic Medicine in Comparison with International Discourses
Wolfová, Magdaléna ; Háva, Petr (advisor) ; Kotrusová, Miriam (referee)
The research problem in this thesis is the quality of treatment of Czech patients suffering from somatoform disorders. The goal is to evaluate the state of scientific discourse of Czech specialists in Psychosomatics, compared with the international situation as a potential starting point of further institutional change, including its legitimacy (interviews with citizens). Methods were used: (1) critical discourse analysis and (2) semi-structured interview. Findings: The existence of barriers between representatives of the various professional disciplines prevents the development of Czech scientific discourse of Psychosomatics, combined with the applied reform changes focused on individualism and competition. Czech Republic unsufficiently utilizes the knowledge support from international organizations (WHO, OECD, EU) in the field of mental health policy. The formation of Czech health policy is not compliant with human rights related to health and is more focused on the question of financing than on patients' health. Citizens perceive deficits in psychosomatic approach and try to substitute with complementary and alternative forms of medicine.

The theoretical and empirical part come out of bio-psycho-social conception of health in relation to spiritual dimension understood as person's orientation towards the values of meaning and spirituality. In the theoretical part of the work we dealt with the definition and development of constructs of quality of life, personal well being, religiosity/spirituality, and other variables like social support, gratitude, sense of coherence, the locus of control, meaningfulness, lifestyle, which are connected to the main constructs. The work deals with the questions of measuring, epidemiological and overview studies, questions of relationship causality and hypothesis of possible psychological mediators functioning in complex relations on multiple levels of generality. The constructs of subjective quality of life, and religiosity/spirituality were operationalized as multidimensional variables. Abroad, majority of studies showed positive associations between measuring of R/S and the overall health; however, contradicting results of measuring were not rare. The relation between variables appears to be a lot more complex...

Janíková, Martina ; Hnilica, Karel (advisor) ; Zoufalá, Marcela (referee)
The aim of this thesis is to describe sociocultural aspects of mental illness. The thesis concerns with sociocultural influences on formation and conceptualization of illness explanatory models that shape unstructured fyziological phenomenon into structured experience. Specific cultural content of biological categories is the foundation of health care systems, lay conceptions and prejudices and determines social status of the mentally ill. Also attitudes of society towards its deviant members reflect her essential characteristics. Therefore, this thesis outlines historical and cross-cultural transformations of mental illness within the broad social context. Describes health care system as a specific layer of social and symbolic reality and points out the influence of dominant psychiatric discourse on the conception of illness. Attention was also paied to media selective portrayals of mental illness that were proved to strenghten negative stereotypes. Mental illnesses, how was shown, differs cross-culturally. Though basic psychiatric syndromes occur in every society, culture promotes and shapes particular symptoms, so that progress and prognosis can be very different. We can, therefore, conclude that every illness is in a way culture-related.
