The clinical relevance of implementation of new technologies in extracorporeal circulation
Svitek, Vladimír ; Harrer, Jan (advisor) ; Tošovský, Jan (referee) ; Černý, Jan (referee)
The clinical relevance of implementation of new technologies in extracorporeal circulation Summary Background: An organism's inflammatory response related to cardiopulmonary bypass (CPB) use is caused by activation of immune system cells with the production of a whole spectrum of mediators, whose activity can result in organ dysfunction. Current research, in trying to eliminate the negative effects of CPB, is engaging in innovating technology of systems for CPB. One of the last complex innovations is the so-called miniinvasive cardiopulmonary bypass. Till now though, the clinical benefits of using this system has not been clearly proven. Methods: A group of 54 patients, who were indicated for elective coronary surgery were randomised into two groups - group cCPB (patients operated on using classic CPB in the open modification) and group mCPB (patients operated on using minicircuit). We monitored and compared concentration of interleukin-6, polymorphonuclear elastase, monocyte chemoattractant protein-1, interleukin-10 and soluble receptors for tumour necrosis factor-α in both groups during operation and postoperatively. At the same time we compared the postoperative clinical course. Results: Both groups did not differ in the basic pre and peroperative characteristics (age, EuroScore, ejection fraction,...

Wireless Capsule Endoscopy in Experimental and Clinical Diagnostics of Enteropathy Induced by Nonsteroidal Anti-inflammatory Drugs
Tachecí, Ilja ; Kopáčová, Marcela (advisor) ; Lata, Jan (referee) ; Zavoral, Miroslav (referee)
Introduction Non-steroidal anti-inflammatory drug (NSAID)-induced enteropathy represents an important complication related to one of the most commonly used drugs worldwide. The prevalence has been underestimated for a long time due to previous diagnostic difficulties. Objective The main goal of our project was to evaluate the basic characteristics of NSAID- induced enteropathy as well as the benefit of capsule endoscopy in its diagnostics. Methods and results In the first, experimental, part of the dissertation, we elaborated methodology of capsule endoscopy in an experimental animal and checked it on 7 healthy pigs. Although normal enteroscopy pictures were found in all the animals, some limitations were identified. The markedly worse visibility of the small bowel mucosa, and incomplete evaluation were the main ones. A model of NSAID-induced enteropathy (using a 10-day per oral indomethacin administration - 400 mg/day) was projected and verified in 8 healthy pigs consecutively. The endoscopy findings were confirmed by means of autopsy. We revealed the presence of small intestinal mucosal lesions on capsule endoscopy in the majority of the experimental pigs (87.5%). The most frequent findings were mild mucosal injury (red spots and erosions) in 6/8 animals. These findings were confirmed in 50.0% at...

Salmela, Alexandra ; Velkoborsky, Jan Petr (referee) ; Lindroosová, Hilkka (advisor)
My master's thesis' goal is to introduce the contemporary Finnish drama to that Czech and Slovak speaking reader, who might be interested in Finland and its culture. The emphasis of my master's thesis is the theme of the contemporary Finnish drama and its placement into a broader social context. Whilst the contemporary Finnish drama is a broad term, I first had to determine the corpus of my investigation. I have decided to limit the time period by the year 1995, when Finland seemed to definitelly get over the grievous economical crisis of the early 1990s and became a member of the European Union, the upper boarder was the present years of writing the thesis, that is 2007/2008. Then, I only deal with the dramas originally written in Finnish language, thus there are some remarkable Finnish Swedish playwrights missing from this master's thesis. On the grounds of genre and originality of the dramatic texts I have decided not to be concerned with any other texts than the original dramatic texts such as adaptations of prosaic works, benefitions, poetic montages, musicals, etc. On the basis of target group I have excluded the plays for young and fairy tales. I thus decided to concentrate on a corpus consisting of original dramatic plays for adults that are of certain artistic quality and statement value and were...
