Integrated treatment of people with a dual diagnosis in the aftercare program: a series of case studies

Background: In the addiction treatment continuum, there is a relatively large group of clients with a dual diagnosis who repeatedly fail in treatment and follow-up treatment, with frequent relapses and recurrences of the disease and subsequent returns to the treatment continuum. The case study is focused on the perception of the integrated treatment of dual diagnosis in the follow-up care program by clients with dual diagnosis. Objectives: The research aims to map the perception of the integrated treatment of dual diagnosis in the aftercare service by the clients themselves. The sub-goals were to map the perception of the connection between addiction treatment and their mental disorder, what factors clients perceive as important in their journey to recovery, and how they perceive the presence of clients without mental illness. Motivation of graduates of the bachelor's study programme Addictology to work in addiction treatment services

Background: A study programme Addictology was first opened in the academic year of 2005/2006. The profession of addictologist was enshrined in legislation by the Act No. 96/2004, of paramedical professions, in 2007. Thus, graduates in bachelor's degree in addictology became healthcare professionals gaining qualification to exercise this profession (Miovský et al., 2021). Job motivation represents a psychological process bringing energy and giving direction to work of a person. Motivation to choose and remain in helping professions is the topic of many specialised publications in Czech and foreign literature but minimum number of those focusing on motivation for work in addiction treatment services. Addictology as a field of study is specific not only by its historical context and development, but also by its scope and specialization. This might be also influential in specificity of motivation of workers to enter and stay working in the services. Limits in working with patients with an addiction problem in the practice of general practitioners in Prague

Backround: Practitioners who do not have in-depth knowledge in the field of addictionology come into contact with patients in their practice who may have or already have an addiction problem. This patient is often accompanied by stigma and taboo, and even at a professional level it is often difficult to address the addiction problem. Doctors are obliged to map the problem and provide help in the form of a brief intervention and can thus motivate their patient to seek professional help. There are both barriers and supportive factors in the situation of initiating and conducting a conversation about addiction with a patient. Aims: The aim is to map the limits in working with patients with addiction problems from the perspective of a general practitioner. To describe and understand the limits of working with patients with addiction problems in GP practices in Prague. Possibilities and limits of the work of an addictologist in the Prison Servise of the Czech Republic

Background: Substance use among people commiting crime has been an important issue for a long time. Since 2016, the Czech Prison Service (VSČR) has established the position of an addictionologist to provide expert addiction care in prisons using the same interventions and procedures that are used in civilian facilities. The position is classified under health services, but is expected to work closely with correctional and detention services. The conditions of work depend on each prison specifics. Aims: The research describes actual situation of addictologists employed in the Prison Service of the Czech Republic. Using sub-objectives, the author tried to find out the characteristics of people working as addictologists in the Prison Service, to describe their activities in the Prison Service system and to find out the addictologist's view on the perceived possibilities and limits of this position. Resources: Due to the demographic aging of the population in developed countries, the proportion of seniors in the population is increasing. This trend is also reflected in the adictology which we are increasingly meeting the target group addicted to addictive substances (gerontoadictology). The Czech republic has a relatively well-established network of services for addiction clients in which the residential care service has a justified place. It is focused on maintaining positive behavioral changes, gradual integration of s, which programmes and adaptations it has for them, or what are the gaps in the care of these clients. Background: The study of general medicine aims to equip a future medical practitioner with the knowledge and experience they will need in their future practice. The physician should master the methods of Orientational diagnostics and Brief intervention in the field of substance abuse and addiction. A marginal part of the time is donated to this topic as part of undergraduate studies. Aim: The purpose of this study is to find out the level of certainty of the students of the 1st Faculty of Medicine in their knowledge regarding Orientational diagnostics, Brief intervention and to find out the student's attitude toward addictology. The partial goal is to find out what the students need to gain confidence in providing care to addiction patients. Methods: The research was conducted using an anonymous survey. The survey was shared on the social network Facebook in six individual private groups of medical students. The resulting data were statistically processed using Microsoft Office Excel and Microsoft Word. Alcohol use among women in their middle age: series of case studies

Background: The case study is focusing on women who have completed alcohol dependence treatment in their middle age. The research studies the reasons for getting addicted, which are based on true facts. The research as well as practice shows that middle age is highly risky to develop addiction. The topic is insufficiently discussed yet actual. Targets: The object of this work is to describe and understand the issue of alcohol dependence in women in their middle age whit higher socioeconomic status. . This issue is described in the case studies of three respondents who meet the selected criteria. Methods: The practical part of the final work consists of case studies based on moderated interviews. These were created from the anamnestic sheet, from questions focused on the researched issues and from supplementary questions. The data were processed by a narrative method. Results: The stories of our respondents are different but factors that lead them find a solution in alcohol are similar. They describe emptiness, loneliness, sadness, change of appearance, health issues related to higher age and also pressure at work. Background: The case study is focusing on women which have completed alcohol dependence treatment in their middle age. Look into the causes of addiction which are based on facts. This topic is insufficiently discussed but still actual. Targets: The object of this work is to describe and understand the issue of alcohol dependence in women in their middle age. This issue is described in the case studies of three respondents who meet the selection criteria. Methods: The practical part of the final work consists of case studies created on the basis of moderated interviews. These were created from the anamnestic sheet, from questions focused on the researched issues and from supplementary questions. The data were processed by a narrative method. Results: The results show that middle age and related events are risky for the development of addiction. However, this doesn't mean that alcohol is a means of life change. Rather, we can talk about self-medication. Conclusions: The conclusion of this work can be considered that the awareness of the risk of addiction, rick factors, stressful situations and the possibility of professional help for middle- aged women is very low. An Analysis of the National Tabacco Quitline Czech Republic Clients Experience Using Nicotine Replacement Therapy

Background: The use of nicotine-containing tobacco is among the substances with the highest dependence potential. The negative impact and consequences of cigarette use are discussed not only among experts. In contrast, nicotine replacement therapy and user experience are unspecified. Aims: The primary goal of the research was to map the experience of clients of the National Tobacco Quitline Czech Republic with the use of nicotine replacement therapy in the treatment of tobacco addiction. The categories examined include: dosage of replacement nicotine, preferred forms of replacement nicotine, reported adverse effects, and route of administration of replacement nicotine. The study was carried out in order to refine the dosage of nicotine replacement therapy, increase the compliance of nicotine replacement users and increase the quality of services provided by the National Tobacco Quitline Czech Republic. Possibilities and limits of tobacco dependence counseling and smoking cessation among patients in the medium-term alcohol addiction residential treatment

Background: Despite most studies have long term and repeatedly confirmed that the prevalence of smoking among individuals treated with other primary addiction is two to four times higher than in the general population, most services do not provide some form of smoking cessation. Smoking cessation is not clearly grounded in these services. An important aspect for the integration and implementation of smoking cessation programs in addiction treatment services are opinions and attitudes of the workers themselves. These opinions and attitudes are very often ambivalent and perceived as controversial. Attitudes and opinions can determine the possibilities and limits of working with the smoking cessation during the treatment of other primary addictions. Aims: The study has four research objectives that reflect the results of theoretical-critical analysis.
