Executive functions-diagnostic and rehabilitation

Executive function testing still appears to be very complicated for many reasons. In practice, we can meet the situations in which commonly used standardized neuropsychological tests do not reveal injury in executive functions, which is absolutely obvious in everyday life. The goal of the submitted research was to confirm a hypothesis whether getting the executive function testing closer to real life may contribute to their diagnosis specification. In the conducted research, the individual performance of the group comprising 29 persons with dysexecutive syndrome was compared with the individual performance of the control group of 29 healthy persons. The testees' performance was monitored in two test situations - at standstill (standard testing) and during noise load (real conditions simulation). In both test situations the identical tests of NEUROP-2 (KIQ, PAARE, GO/NO-GO, LISEQ, HANOI and NATE) were used. In compliance with the hypothesis, negative impact of noise on the performance of persons with dysexecutive syndrome was proved in the KIQ a HANOI tests. The main theme of the diploma thesis is the impact of these effects on the patient's family. The work consists of a theoretical and an empirical part. The theoretical part is divided into four spheres - neuroanatomy and pathology, brain injury, psychosocial consequences and rehabilitation. In the empirical part the author use a qualitative research approach to deal with the impact of psychosocial consequences after the traumatic brain injury to the patient's family. Feasibility of Non-Pharmacological Intervention in Therapy of Cognition Deficit in Czech Schizophrenia Patients - Computer-assisted Cognitive Remediation

There is a body of scientific literature describing cognitive deficit in schizophrenia and the possibility of its remediation. However, in the Czech Republic, there have been so far just few studies that examined the topic. The objective of this work was to explore the feasibility of non-pharmacological intervention in the form of computer-assisted cognitive remediation of Czech schizophrenia patients with diagnosed cognitive deficit. Our results confirmed the positive short-term effect of targeted remediation of selective domains of cognitive deficit in patients with schizophrenia. Our findings suggested that 5 working memory function is the domain most sensitive to the structured intervention. These results are consistent not only with findings from other published studies that used similar computer method for remediation, but also with other studies that used different approaches. The Motivation of Spouses to stay together after a Brain Injury

This diploma work deals with the situation of married couples, people who suffered a brain damage. After a brain damage many various mental changes of grave importance may appear apart from the physical consequences. These consequences affect not only the person that suffered the brain damage but also his closest mainly his partner and affect also the quality of their marriage. The theoretical part of the diploma work deals primarily with the importance of the marriage from the sociological and the psychological point of view as well. Further the work focuses on the brain damage itself and its consequences. The attention is further drawn to the impact of such situation on the husband or wife of a person after brain damage. Following part deals with the motivations for marriage. The empirical part of the work focuses on qualitative research based on eight individual in-depth interviews with husbands/wives of a person after a brain damage. The main goal of the work is to map the motivation of a husband/wife of the individual after a brain damage for remaining in marriage and identify other factors connected with this motivation.
Adaptation of the EBIQ neuropsychological questionnaire for the diagnostic of the emotional and psychosocial problems after brain injury.

Neurorehabilitation for people suffering from brain injury has traditionally focused on rehabilitation of the somatic and cognitive functions. Research into the emotional aspects of brain injury has, however, been limited. The theoretical part of this thesis was therefore to map diagnostic tools suitable for the investigation of emotional problems of patients after brain injury and to classify these tools for the needs of neuropsychological practice. In the empirical part the author focused on the adaptation and pilot verification of the EBIQ (European Brain Injury Questionnaire) neuropsychological questionnaire, for a sample of patients having experienced brain injury and their immediate family and friends. The practical outcome of work in addition to the psychometric characteristics is a Czech working version EBIQ-P (version for patients) and EBIQ-R (version for family members), including well-prepared instructions for evaluating the results and examples for use in clinical practice. EBIQ provides an alternative to the previously used questionnaires SCL-90 and MMPI-100, since these, according to our findings and the findings from other studies on the group of patients after brain injury provide invalid results.
The use of the DMS 48 test for the diagnosis of the mild cognitive impairment and the initiatory phase of dementia

This thesis presents a contribution towards the diagnosis of the Mild Cognitive Impairment and to the initiatory phase of dementia. We have used the DMS 48 test to analyze the differences in the area of memory capabilities damage. The research has been carried out with the sample of 50 patients with MCI, who have been diagnosed according to the actual Petersen concept. There has been a control group of 34 persons created with no exposure of the cognitive function damage. The research has proved that the achievements of the patients with MCI have been in the DMS 48 test significantly lower than the achievements of the control group. The achievements of the patients with MCI in the DMS 48 test have been also compared with the results of patients that have been tested in other chosen neuropsychological tests with the aim to confirm the ability of this test to identify hippocampal memory damage. We have chosen 2 neuropsychological memory subtests, which according to the theoretical constructs can measure the hippocampal memory damage and another two subtests, which are not able to distinguish the hipocampalnian memory damage from other types of memory damage. Wechsler memory scale-III., abbreviated test description and preliminary standarts for 20-39 yars old

This thesis is about Wechsler Memory Scale-III Abbreviated (WMS-IIIa). WMS-IIIa is a screening test for measuring memory functioning. It contains measures of visual and verbal memory, immediate and delayed memory. Theoretical part of the thesis is an attempt of a Czech version of the test manual. It summarizes information about its original development, structure, purpose, administration, scoring and possibilities of interpretation. Moreover there is ona charter which summarize current knowledge about memory and one charter devoted to possibilities how to measure memory via psychological methods. The empirical part is a research targeted on acquisition of Czech norms on representative sample of nonclinical population at the age of 20 to 39 years. Statistical analysis was computed from data of 154 individuals. Demografic characteristics of research group was compared with characteristics of Czech population at the age of 20 to 39 years as described by Czech Statistical Institute.
Rehabilitation of patients with schizophrenia: training of cognitive functions

Rehabilitation of patients with schizophrenia is currently an important part of the therapeutic care of individuals with psychotic disease. Its effectiveness is often limited by the cognitive deficits which people with schizophrenia have to face with also in their everyday lives. In the presented paper we outline the basic theoretical outcomes and specifics of the psychosocial rehabilitation and training of cognitive functions applied to the patients with schizophrenia. In the empirical part we concentrate on possible effects of the training program of cognitive functions in non-govt. organization o. s. Green Doors. During the study, which took 35 months, we managed to get complete research data from 20 non-resident patients who attended a three-month intensive cognitive training and from their 14 persons closely related. The research data were collected by means of administrated neuropsychological test battery, three self evaluating questionnaires focused on cognitive deficits and its influence on ordinary life, designed for the patients, and two questionnaires for evaluation by close people.
