Time of primar rehabilitation and care by patients with fracture of proximal femure
Lehovec, Roman ; Douša, Pavel (advisor)
The topic of the thesis I chose on the basis of their interest in the subject orthopedics . Fractures of the proximal femur fractures are among the most common and most serious. Incidence with us around 50 to 60 to l00 000 inhabitants a rising trend. Surely it plays an important role increasing middle life expectancy. (14) Fractures of the femoral neck and intertrochanteric fractures occur virtually same frequency . The average age of patients with fractures of the proximal femur is 77 years and the proportion of women and men is 3:1 ( 14). Intertrochanteric fractures that represent about 10 % of fractures of the proximal femur have two peaks occurrence , between 20-40 years and in patients over 60 years old . In younger arise as result of high energy trauma in the elderly etiology is the same as the cause of other fractures of the proximal femur. The mechanism of injury is the most direct impact on the trochanter during a fall to the side or limb external rotation during the fall forward. (14) For the treatment of hip fractures osteosynthesis or prosthesis use . The method of choice in the treatment of trochanteric fracture osteosynthesis . (15) The aim of this work was to evaluate patients treated at the Orthopedic - Traumatology Clinic of the Faculty Hospital in Prague for fracture the proximal femur in...
Development of the surgical instrumentarium for the anterior cruciate ligament reconstruction by the hamstrings tendons
Hanus, Martin ; Trč, Tomáš (advisor) ; Podškubka, Aleš (referee) ; Douša, Pavel (referee)
The paper presents the development of instrumentaria designed to solve ACL lesions using hamstrings (tendons of semitendinosus and m. gracilis - ST / G). The work itself took place in two phases. Part of the first phase was the development of its own instrumentation, including the implant designed to fix hamstrings tendons. The inherent part of the development was the performance of both mechanical and functional tests. The most tested element of the instrumentation was the screw with eye. In the course of its development, mechanical tests of tensile strength were carried out on the individual variants of the solution, and then the rapture of the screw with the eye from animal bone - simulating the function of the product in a real environment. Part of this was the development of methodology and work with new instrumentation. Clinical testing was performed in the second stage and the results of the operations are part of the work. The self-developed ACL lesion reconstruction toolkit is designed to use a graft made up of four strands of the above tendons. In the experimental part of the instrumentation development, the suitability of the implant for clinical use has been demonstrated. The newly developed instrumentation for ACL reconstruction, using the test implant for fixation of the hamstrings...
Treatment of Pelvic Ring Fracture Using Spinopelvic Fixation (Types of Fixation, Indications, Functional Results, Mechanical Characteristics)
Weisová, Drahomíra ; Pavelka, Tomáš (advisor) ; Douša, Pavel (referee) ; Chaloupka, Richard (referee)
In the case of a high-energy injury (traffic accident, fall from above), a pelvic ring injury with vertical instability and/or a sacrum injury with spinopelvic instability may occur. Such instabilities threaten the basic functions of the pelvic ring and lumbosacral junction. Adequate transmission of body and limb load is not possible through such injured segments. The solution is some form of reduction and stabilization to achieve the restoration of anatomical conditions and a gentle and firm connection. Spinopelvic dissociation in some cases accompanying pelvic ring fractures and / or damages is a complex injury with a difficult solution. The frequency of occurrence is reported in the literature 2.9-3.1 %. The number of such accidents is on the rise with higher density and aggressiveness of traffic, the great popularity of dangerous human activities and the accelerating pace of life. Most of these fractures are treated surgically, non-surgical treatment is reserved only for patients who are unable to undergo surgery for internal or local reasons. In our study group, we evaluated a group of patients with a pelvic ring fracture and / or sacral fracture with spinopelvic dissociation who were treated at our workplace in the period 2009- 2019. During this period, 222 patients with a pelvic fracture...
