Nurse and pain

The Bachelor thesis deals with the chronic pain issues, the main point of interest therein is the pain connected with oncological diseases. The cancer pain, means of its treatment and the role of a nurse as far as the correct monitoring as the major attribute in cancer pain therapy is concerned are amongst the most important pillars of this work in theory. In terms of successful coping with pain, psycho-oncology should also be referred to as one of the pillars of the treatment with significant impact. Furthermore, the theory part informs about using the adequate coping strategies. The theory part is completed with the cancer patient description and it characterizes the specifics of nursing care for such clients. The empirical part focuses on pain intensity findings for oncological patients as well as formulating the frequency of using the coping strategies and examining the differences between the genders with regards to using these coping methods. The data, which was taken as a statistical base for the calculations of this thesis, were collected using the questionnaire method. A numeric scale was used to identify the pain intensity and the CCQ (Cancer Coping Questionnaire) was utilized to monitor coping strategies and define the frequency of their use.
Pain therapy with focus on pacients with oncological disease

This therapy doesn't solve the cause of cancer, but significantly increase patient's quality of life during oncological treatment, even in the terminal stage. The main target of this work is to give basic information about pain treatment, focusing on the oncological pain, and elaborate a review of pharmacological groups and individual substances most used in oncological pain treatment. Basic pillar of the cancer pain treatment is pharmacotherapy, which goes out of the WHO three-grade analgesic ladder. Basic therapy is made of nonopioid analgesics. In case that they aren't enough, we can add weak opioids, which can be replaced by strong opioids afterwards. Analgesics are usually used repeatedly and often in combinations. Dosage interval depends on pharmacological properties, dosage form and patient condition. Occupational therapy approaches in patients with chronic pain focusing on fibromyalgia

Name of thesis: Occupational therapy approaches in patietns with chronic pain focusing on fibromyalgia Aim: The aim of my thesis is to find out whether occupational therapy has something to offer patients with fibormyalgia, as well as the determination of the individual problem areas of these patients, in particular from the perspective of occupational therapy and designing specific therapy approaches. Method: In the theoretical part I focus on the summary of the information from the available literature on pain in general and also on the latest findings in the field of diagnosis and treatment of fibromyalgia. In the practical part I focus on three patients suffering from fibromyalgia. The necessary information to determine the individual problem areas I gained by using the methods of qualitative research, using structured interview and therapeutic examination. Results: Based on studying of literature and the implementation of my exploratory investigation I came to the conclusion that Occupational therapy in these patients is very essential part of the healing process. Coping with chronic pain

This thesis describes coping in pacients with chronic musculoskeletal pain. It focuses especially on the cognitive evaluation of pain and coping strategies. Furthermore it deals with the emotional changes in chronic pain. The theoretical part consists of the development of access to pain and the overview of some current knowledge of biological and psychosocial determinant of pain. Emphasis is placed on a holistic, biopsychosocial approach to the issue. Empirical part consists of research aimed at assesing the important factors that contribute to cope life with pain, as in terms of personal dispositions of an individual, so in terms of environmental influence. Medication of prescribed opioid analgesics in combination with alcohol or other illegal drug in age group of 50 - 64 years

Background Based on the gathered data from the previous pilot study and the results of this study we proceeded to the following study which is being focused on the surveillance of the age group 50 - 64 years in the area of prescribed opioid analgesics. Alongside with this, we explored in what possible combinations the medications can be used by the chronic pain patients in the current moment. Methods The base of the research was the qualitative study which was performed by the half-structured deep interview with the participants from the chronic pain patients. The interviews were carried out during the year 2011. Within the main study were pursued two quantitative substudii during the January and March 2012. The first study was focused on the questionnaire survey about the number of the chronic pain patients in the Czech Republic. The second substudy followed the chronic pain patients and their influence by the environmental conditions with the help of the questionnaire in case of the combinations among the prescribed opioid analgesics with alcohol or non-alcohol addictive drugs. Pain perception in elders

The aging of the population is one of the problems of the current period. With increasing age, we have to deal with the accompanying phenomenon more and more - the pain which significantly inconveniences the life and makes normal integration of elderly people into society impossible. This bachelor thesis summarises the present knowledge of pain in elderly people. It addresses the mechanism of pain perception, physiology of aging and types of pain which occur in elderly people. Some types of pain are accompanying diseases which occur during old age - neuropathic, chronic and oncologic pains. The higher occurrence of these types of pain could be explained by the progressive degeneration of nerve system during aging, either due to demyelization and reduction of the number of nerve fibres or due to accumulation of free radicals in the organism. On the other hand, there are some types of pain which elderly people feel with lower intensity - visceral and postoperative pain. At the same time, the pain threshold is increasing in the elderly people. This paradox, i.e. on one hand higher occurrence of chronic pain at higher intensity and on the other hand reduction of visceral pain and postoperative pain could be explained by diffuse noxious inhibitory controls. Somatognostic functions and spatial memory in patients with failled back surgery syndrome

Aim: The aim of this thesis is both evaluate a somatognosis quality and consider rate of personality dimensions in patients with FBSS. Methods: 30 patients with FBSS and 25 probands of healthy control group were examinated in laboratory of spatial cognition called Blue Velvet Arena. They underwent spatial navigation task and body scheme tests. We made comparative analysis of these measurements between groups. We evaluated personality dimensions in patients group, by using NEO personality inventory and Petrie test. Finally we consider relationships of these variables and another related factors. Results: Signicant difference between patient's group and control group was found both in spatial navigation task and body scheme tests. In spatial navigation task control group had lower results compared to the patient's group. In body scheme tests was found reverse results. We found high correlation of body scheme with age, pain duration and activity level. There was high percentage of underestimating probands and probands with high score of conscientiousness. Error rate (judgement out of tolerance zone) was higher in patient's group compared to the control group. Body schema and spatial orientation patients with Failed Back Surgery Syndrome

This diploma thesis summarizes recent knowledge about body scheme, spatial orientation and their change in context of chronic pain. The purpose of this work is to find out if chronic pain negatively affects the body scheme and spatial orientation. The thesis includes a comparative analysis of experimental measurements between the patients with Failed Back Surgery Syndrome and healthy control group. The analysis compares tasks based on spatial orientation and body scheme tests. Results show the patients' group having lower results in body scheme related tests, showing a noticeable disparity compared to the control group's test scores. These results were further supported by portion of spatial orientation based tasks, where patients' group again performed at significantly lower level than the control group. In conclusion, our thesis, along with other academic works, states that there is a correlation between chronic pain, disrupted body scheme and spatial orientation. The role of the paramedic in the treatment of pain in the prehospital emergency care

The Bachelor thesis deals with a very current and spread topic in the sphere of the pre-hospital urgent care, it concerns the issue of pain and the role of the paramedic at its treatment in the pre-hospital urgent care. Pain is one of the most frequent indications initiating the medical ambulance service intervention. It is an affiliated and really essential stressor. The task for the paramedic is to help the patient suffering from pain and to use all ways of treatment which are possible to be applied in the sphere of the pre-hospital care. Therefore it is very important for the paramedics to be well informed on the issue of analgesia and to have good theoretical knowledge and to be able to apply it in practice. The theoretical part deals with the complex issue which is related to pain and with the role of the paramedic. At the beginning of the work there are defined basic concepts concerning the wide issue of pain, i.e. its types and categories form the localization point of view. Further, there is described the basic examination and getting information from the patient which form the integral part of information and data for consequent diagnosing and its possible therapies in the pre-hospital care. Therefore there is also a detailed elaboration of evaluating scales including the children´s ones. In order to get a sight into the issue of pain treatment there is integrated the subchapter concerning the invasive and non-invasive methods in the work. Next, the attention of the work is mostly focused on defining concepts of the pre-hospital urgent care and on the competences of the paramedic which form an integral part of the knowledge of this profession. It is related to possibilities of non-pharmacology and pharmacology treatment in the pre-hospital urgent care, which the specialist can use. The practical part was focused on recording of knowledge and procedures of paramedics in the process of pain treatment in the pre-hospital urgent care. There was carried out the whole republic inquiry using the quantitative method of anonymous questionnaire form. It was designed for the medical care workers working as paramedics. All regions in the Czech Republic were contacted; however, only eleven joined the research. Upon agreement with the managers of non-medical workers the questionnaires were delivered to paramedics. There were sent out 210 questionnaires while their return was 71.43% (150). All questionnaires were included in the consequent resultant inquiry. Responding paramedics were informed on the character of the research and assured that the anonymity of their person and the provided information will be preserved. The questionnaire involved 17 questions in total and the first five questions were statistical ones. The questions were focused on knowledge of analgesia and on procedure used by paramedics in the fieldwork. Questions 2, 4, 4a, 4b, 5, 8, 8a, 10, 10a, 11 were selected in order to get information whether the responding paramedics have knowledge on the issue of analgesia an in order to get base for answering the research question. The obtained information was consequently evaluated. Each question was answered individually and then transferred into diagrams (sometimes only into charts), which show their absolute and (mostly) relative frequency of obtained answers. The stated research question was (on the basis of the above mentioned 10 questions) evaluated by means of chi quadrate test. The obtained level of significance refused the stated ideal statement and confirmed results of the research which says that the knowledge of the paramedics is insufficient. The goal of the work was to record possibilities of pain treatment in the pre-hospital urgent care by the paramedic and it was fulfilled. Statistically evaluated questionnaire inquiry and answer to the research question show that further education of paramedics as well as their accepting the complex view of the issue is absolutely necessary.
The life of patients with chronic pain

The pain, especially chronic pain, can influence many aspects of life. It is not only physical factors, but also psychic and social factors that can affect its perception. The long-term pain can handicap the patients not only in the somatic area, but can also have a negative effect on social and psychological position in society, family or at work. Nowadays, chronic pain seems to become more common, its severity is increasing and that is why there is also a progress in the field of treatment methods. The treatment of chronic pain is often very complicated and full recovery can hardly be achieved. Above all, the main goal is to decrease the intensity o pain, improve patient{\crq}s quality of life and to assist in increasing patient{\crq}s independency. The aim of the proposed thesis was to describe the quality of life of the patients with chronic pain, treated in the ambulatory departments for the treatment of pain and to verify the hypothesis that the patients with chronic pain present with lower scores of quality of life in comparison with the domain scores of the common population. The quantitative research on quality of life of patients with chronic pain was evaluated using the WHOQOL-BREF questionnaire by the World Health Organization that is intended for evaluation of quality of life. The questionnaire inquiry was performed during the period from November 2008 to February 2009 an two workplaces {--} at the Ambulatory department for the treatment of pain in České Budějovice (private facility) and at the Ambulatory department for the treatment of pain of Nemocnice České Budějovice, a.s. The cohort contained data form 73 respondents. The cohort was further subdivided into age and gender categories. The average raw scores of single domains and two independent items were calculated, evaluating the overall quality of life and satisfaction with health. The basic descriptive statistics was calculated and correlation analysis was performed. Furthermore, the differences between the gender and age categories were calculated. The thesis revealed the lower scores of quality of life in respondents with chronic diseases in comparison to the domain scores of the common population. Furthermore, significant relations were observed between the Physical Health Domain and independent item Quality of Life, respectively between the domain Enjoyment and item Quality of Life. The comparison of the domain scores in men and women did not reveal any statistically significant differences, there were only minute differences between the age categories. The perception of quality of life in the patients with chronic pain is tightly associated with their enjoyments, as confirmed by the results of the correlation analysis. Therefore, I would recommend the more thorough psychosomatic approach for the treatment of pain at the ambulatory departments for chronic pain, but also in the offices of general practitioners, who are the first to meet the patients suffering from pain and frequently it is them, whose education about adequate treatment is crucial.
