Stress-strain analysis of lower limb prosthetic

This master thesis deals with lower-limb prosthesis. The aim of the first part of this work is stress-strain analysis of trans-tibial prosthesis, which is understand without prosthetic socket and the foam prosthetic feet. Analysis is made using finite element method in ANSYS Workbench 12.0 software. For the purpose of setting up the computational model it is necessary to make few partial models. Model of geometry of the prosthesis is made in SolidWorks 2009. Based on the outputs of stress-strain analysis and the results evaluation, the critical component is chosen and this one is evaluated according to limite state for fatigue. The algorithm of evaluation of named component in the state of high cycle fatigue is discussed in the second part of this thesis.

The role of radiographer during MR angiography Objectives: The thesis and the tutorial CD provide information on the magnetic resonance angiography {--} MRA. Due to upgraded hardware and software and the non-invasive character of the examination the method is currently the most common form of the imaging of flowing blood in vessels. It is essential for a radiographer to acquire comprehensive knowledge of the basic principles and techniques of MRA. The objective of my thesis was to find out if the method can be used not only for a high-quality imaging of the arteries but also for subsequent therapeutical procedures. Methodology: The research was conducted with 106 patients (66 men and 40 women) whose lower limb arteries had been tested by MRA between 1 January and 31 December 2007 at the Multiscan RC Pardubice. I determined the number of patients who had undergone a therapeutical procedure after the MRA test (operations on arteries, percutaneous transluminal angioplastics) and assessed to what degree the MRA examination had been sufficient for the surgery. Results: In 91 % out of 46 patients indicated for therapeutical procedures the results obtained by MRA enabled other treatment or a surgical procedure. Only in 9 % patients the results obtained by MRA were not sufficient for other medical treatment and suplementary examinations such as the diagnostic digital subtraction angiography were necessary. Conclusions: MRA is a non-invasive medical diagnostic technique to visualize the blood in vessels which provide satisfactory accuracy without exposing the patient to any ionizing radiation. The results are used for other treatments. Radiographers who master the MRA techniques avoid mistakes that might devalue the test. The role of ATM in breast cancer

Incidence of breast cancer is continuously increasing in the Czech Republic. Tumor development is a result of gene alterations'accumulation, particulary associated with genes involved in regulation of cell growth and division. Hereditary carcinomas account for approximately 5-10% of all breast tumors and in 60-80% cases are caused by a germline mutation in the major predisposition genes BRCA1 and BRCA2. Nevertheless, other genes, mostly of lower penetrance, may play a role in breast pathogenesis such as the ATM tumor-suppressor gene. ATM is the apex of the repair pathway of DSB. This protein kinase activates through phosphorylation of its substrates cell cycle checkpoints, which leads either to the delay of the cell cycle progression until DSB are repaired or to the promotion of apoptosis. To sum up, the ATM gene seems to have a role in breast cancer development in a minority of the high-risk families in our population which is significantly lower compared to BRCA1/2 and it also seems to be involved in pathogenesis of sporadic breast cancer. Despite the ATM gene's length, we do not perform the preventive screening of this gene in breast cancer high-risk families. Nevertheless, we offer the molecular diagnostics of ATM to ataxia telangiectasia patients.

Molecular factors of artherogenesis in chronic kidney disease with experimental model of chronic inhibition of Nitric Oxide Synthesis and its Effect of HMG-CoA reductase inhibitoin

The experimental part of the thesis covered the markers involved in nitric oxide deficiency model in rats. The analogue of arginine N-nitro- L -arginine-methyl ester ( L -NAME) was used. L -NAME administration resulted in sustained elevation of BP, decreased GFR, and in higher albuminuria compared to control animals. The administration of hypolipidemic drug - atorvastatin to Wistar rats was studied. The statin used caused lower albuminuria and prevented decreases in GFR in L -NAME rats without having an impact on hypertension. CAV1 protein expression was similar in all groups of rats. In contrast, VEGF expression and RhoA activity was increased in L -NAME-treated animals, and normalized with coadministration of atorvastatin. Clinical part of the thesis focused on the analysis of markers of oxidative stress in obese and non-obese patients with CKD and after kidney transplantation. The patients with CKD had elevated serum concentrations of AGEs, sRAGE and ADMA with the maximal concentrations of AGEs in the patients on hemodialysis. The successful kidney transplantation decreases the AGEs, sRAGE and ADMA concentrations but not to normal value according to general population. Clinical and elektrophysiology longitudinal study of children with hereditary neurophathy Charlot-Marie-Tooth type 1A

Hereditary peripheral neuropathy, known asCharcot Marie Tooth disease (CMT) and with an incidence of 1:2500 -1:10 000, is the most common hereditary neuromuscular disorder. Type CMT 1A is the most common form of CMT refering to the group of primary demyelinitateing motor and sensory peripheral neuropathies. CMT phenotype is clinically characterized by chronic slowly progressive distal muscle weakness and atrophy with hypo or areflexia and mild to moderate acral sensory loss. The lower limbs are predominantly affected. The aims of this study were to describe the first and most common signs of CMT1A during the first decade of life, to characterize their progression, and evaluate the sensitivity of CMTNS (Charcot-Marie- Tooth neuropath scale) for CMT1A young children. Sixteen children aged 3 to 10 years with genetically proven CMT 1A were examined. All patients were clinically examined, underwent electrophysiological examination, and were scored by CMTNS. Eight were followed for up to two years. Our data shows that CMT 1A in children under the age of 10 years causes only a mild disability. Initial signs of CMT 1A were difficulty in heel walking (15/16, 93%) and lower limb hypo or areflexia ( 13/16, 81%). Stress - strain analysis of jaw with tooth implant

This diploma thesis is oriented on a stress – strain analysis of the jaw bone with a screw dental implant. There is a literature search on this theme in the beginning of this work. Solutions deformation and stress the system lower jaw and implant was performed computational modeling, by using the finite element method. Modeling is part of the lower jaw with dental implants applied screw type Ankylos, Bränemark and implant with metric thread. The thesis is described in detail development of calculation model system and solutions. To create a geometry model of geometry Solidworks 2007 was used. To create calculation model and the solution was implamented in the computer systém ANSYS 11.0 and ANSYS Workbench.

Lower limb prosthetics FEM analysis

This masters´s thesis deals with computational modeling of transtibial lower-limb prosthesis. For assesment of loading character and geometrical configuration, the gait analysis of an amputee, including ground reaction force measurement, strain gauge analysis and motion analysis, was accomplished. Information on geometry was obtained using 3D optical scanning procedure. Material model was gathered using non-destructive mechanical testing and mimicked in a FEA software. For loading conditions the static structure analysys using FEM was accomplished. The critical poins in construction was found. Recognition of agreement about experimental and computational model was accomplished.

The effect of gait training with a prosthesis on stability and parameters in patients with thigh amputation

Diploma thesis "Effect of gait school with prosthesis on stability and walking parameters of patients after transfemoral amputation deals with problems of patients after transfemoral amputation due to vascular etiology. It mentions elementary knowledge of rehabilitation after amputation of lower extremity. Furthermore, there are described main complications that amputation brings into the patient ́s life: pain associated with amputation, skin complications and psychological hardships. Emphasis is laid on gait and gait practice for patients with amputation. I further deal with deviations commonly observed on gait of these patients and with factors influencing the gait, including the influence of particular parts of the prosthesis on patients ́ gait. The practical part is dedicated to comparison of two different approaches to gait practice. The effect of these approaches is evaluated on the basis of results of two groups of patients. The results were gathered by examination on force plate, measurement of gait velocity, examination of gait cadence and evaluation of patients ́s gait videos. Furthermore, I use functional tests: LCI - 5, ABIS - R, ABC scale and PEQ. Title: Comparing muscle timing during cycling on land and in water Objective: The main goal of this thesis was to compare timing of muscles of lower extremities and of back muscles during cycling on land and in water. Methods: In this experiment the relationship between muscles of lower extremities/back muscles and air/water was processed and analyzed. As an objectification was used surface electromyography. The same aqua bike Sapilo was used for both environments. Four women took part in the experiment. None of them was professional cyclist in the past. Findings: Timing of muscles is similar for both environments. The influence of water was reflected in the speed of pedaling and in the level of muscle activity.
