Czechoslovak-Spanish relations 1945-1975
Vurm, Filip ; Opatrný, Josef (referee) ; Nálevka, Vladimír (advisor)
This study deals with the history of spanish - czechoslovak relations in 1945 - 1975. In this period both countries were on the others sides of Iron curtain which separes two ideological blocks created after WWII. Czechoslovakia, where the soviet influence was strong since 1945, became a communist state after coup in February 1948. Franco's regime in Spain was legitimased with the victory in the civil war over the coalition of republicans, which was dominated by the Communist party of Spain (PCE). Anticommunism was an important part of ideology of Franco' s regime and was used at the beginning of Co Id war for improving the image, which was damaged by colaboration with Axis powers during the WWII. For this reason Spain started to be asylium for exiled anticommunists from Eastem block and enabled them to lead political activities. It was also the case of Czech Zdeněk Formánek and Slovak Jozef Cieker. These two men represented different exile organization and had different points of view about Czechoslovakia. Formánek was defending democratic system of Czechoslovak 1. republic. Cieker was slovak nationalist, who wanted to destroy Czechoslovakia, to create independent Slovakia. This is why they were adversaries. They both died in the 60. when anticommunist exile m Spain had much less importace then before. In...

Induction therapy in lung transplantation: initial single-center experience comparing daclizumab and anti-thymocyte globulin BACKGROUND: Acute and chronic rejection remain unresolved problems after lung transplantation, despite heavy multidrug immunosuppression. In turn, the strong immunosuppression has been responsible for mortality and pervasive morbidity. Because acute rejection is associated with inferior outcomes in lung transplantation, we have routinely employed anti-thymocyte globulin (ATG), or daclizumab as adjuncts to reduce rejection. Daclizumab is a human monoclonal antibody that binds to the interleukin-2 receptor. METHOD: We performed a controlled clinical trial of these 2 therapies to determine differences in post-operative rejection, infection, bronchiolitis obliterans syndrome (BOS) and survival. 25 consecutive lung transplant patients received ATG (n = 12)(Group 1) and daclizumab (n = 13)(Group2) as induction agents. The groups had similar demographics and immunosuppression protocols differing only in induction agents used. RESULTS: No differences were observed in immediate post-operative outcomes such as length of hospitalization, ICU stay, or time on ventilators. No patient receiving daclizumab developed drug specific side-effects. There were no significant differences in the...

Patients'Recruitment Issues in Clinical Trials
Hrubá, Dagmar ; Doskočil, Ondřej (advisor) ; Štegmannová, Ingrid (referee)
This thesis examines patient recruitment in clinical trials. It is a current and pressing issue, which is not sufficiently embedded in the thinking of doctors and patients, their patterns of communication, and more broadly in the traditional doctor-patient relationship, which continues to be burdened by post-socialist paternalism. The formal framework and ethics have been developed relatively recently, and their application causes confusion to all participants regarding how to proceed in specific situations. In addition, the choice of topic, and especially the form of dealing with the topic, was motivated by the relative lack of academic resources and publications in the field. The theoretical section of the thesis defines clinical research and its phases, and introduces basic terminology and documents closely linked to clinical research. In addition, it focuses on research ethics, thoroughly examining the issue of informed consent - a fundamental document necessary for recruiting a patient in a clinical trial. Finally, one chapter of the thesis focuses on the issue of medical literacy, the level of which is integral to the participants' understanding of the importance of clinical research, and directly affects participants' decision-making. The empirical section is divided into two parts. The...

Co-evolutionary aspects of genetic diversity: milk production and lactase persistence
Priehodová, Edita ; Černý, Viktor (advisor) ; Brůžek, Jaroslav (referee) ; Macholán, Miloš (referee)
Lactase persistence (LP) is genetically determined ability to digest lactose (milk sugar) in adulthood. Lactose is digested by the intestinal enzyme - lactase. In mammals, production of lactase declines after the weaning period. In case of the LP, the lactase is produced during whole life and thus it is possible to drink larger amounts of fresh milk without digestive problems. However, LP is documented only in some human populations. Its worldwide distribution is unequal, the highest rates are found in northern Europe and in pastoralists living in arid regions of Africa and Arabia. The origin of this trait is related to the emergence of agriculture 10,000 years ago, and the beginnings of milk production. In some populations, there is a strong signal of positive selection for the LP caused by various factors depending on various living conditions. The LP is associated with single nucleotide polymorphisms (SNPs) located approximately 14 kb upstream from lactase gene. Nowadays we know five different SNPs which cause the LP and they have probably different origin and have expanded independently. One of the possible centres of the LP origin is the Arabian Peninsula, where the LP variant -13,915*G probably first appeared and spread. In this thesis, occurrences of different LP mutations were investigated...
