Physiotherapy for gastroesophageal reflux
ŠIRMAROVÁ, Jitka
This thesis deals with the issue of etiology and gastroesophageal reflux disease (GERD). It has been proved that the locomotor system affects occurrence of this disease in many aspects therefore, apart from the primary care of gastroenterologists and general practitioners, treatment using physiotherapeutic techniques becomes significant. Sphincter mechanism at the lower end of the oesophagus being the most important anti-reflux barrier in terms of function is described in the theoretical part in detail. Possible ways and factors involved in occurrence of gastroesophageal reflux and then current treatment of gastroesophageal reflux disease are mentioned. The other part of the theoretical part is aimed at: relations between the internal organs and locomotor system, the diaphragm function and its relation to occurrence of GERD, and possibilities of rehabilitation treatment of patients with this disease. The goal of the theoretical part was to provide a research on the current situation. The practical part was prepared by means of a qualitative research. The tested set consisted of three patients diagnosed with GERD being treated at the Gastroenterological Ward in České Budějovice Hospital. Patients? anamneses were taken and kinesiological testing was done. Similarly, all three patients were identified with a disorder of trunk stabilization. Upon this, individual therapies were proposed and carried out, which was the goal of the practical part, too. Physiotherapy was aimed at: affecting the position of the rib cage and improving its dynamics while breathing, training of breathing stereotype, activation of the abdominal wall, affecting the stabilizing function of legs, exercises in developmental series and modified positions, Jacobson's progressive muscle relaxation, and affecting reflexive changes using soft techniques. The research issue was whether the symptoms of GERD could be affected by treatment of functional disorders of the locomotor system in a positive way. There were certain changes in the kinesiological testing (e.g. change in the activity of lower and upper fixators of scapulae with relaxation of the trapezius, releasing the rigidity of the chest, affecting the dynamic stability of the body) during a three-month therapy. However, functional disorders were not affected that much so any described symptoms of GERD could be changed. The results of the therapy were limited by motivation, attitude and patients? metal state. Not only the internal organ but also the locomotor system probably plays a role in occurrence of the reflux; but also psyche, and all these parts of a human being affect each other. Therefore the treatment of a multifactorial disease such as GERD should be comprehensive. Apart from physiotherapy, metal area, too, and all factors should be treated at the same time, which requires interdisciplinary cooperation unfortunately lagging behind.

Employment forms of comprehensive rehabilition in senior´s house.
ŠTRAMBERSKÁ, Iva
This bachelor?s thesis deals with application of forms of comprehensive rehabilitation in care for elderly. The main task of this study is to determine the level of awareness of application of forms of comprehensive rehabilitation in care for elderly, namely of employees of the Home for the elderly Kaplice, which is located in the South Bohemian Region. The first section of the theoretical part is dedicated to characteristics of comprehensive rehabilitation, primarily to characteristics of forms of comprehensive rehabilitation. The therapeutic part of comprehensive rehabilitation includes diagnostic and therapeutic measures, which improve the functionality of the organism. Another essential section of the comprehensive rehabilitation is the social rehabilitation. The aim of this form is the process of integration of the individual. Subsequently, this form provides to people with disabilities legal protection, as well as financial support. Another section is the educational rehabilitation. The effectiveness of this form of comprehensive rehabilitation is seen primarily in childhood and adolescence. The last section is occupational rehabilitation. The goal of the occupational rehabilitation is to encourage people with disabilities, and to improve their employability in the labor market The second section of the theoretical part describes in detail the issue of aging and old age in relation to the changes occurring in these stages of life. With aging, there is a destructive mental processes. Old age is also accompanied by a series of changes that take place in the social situation of a person. Above all it is retirement which brings a number of changes that considerably influence the economic situation of a person. The last section of the theoretical part discusses the institutional care for the elderly, namely homes for the elderly. The practical part of this study describes the views of respondents on the application of forms of comprehensive rehabilitation care for elderly. To acquire the information, the qualitative research method was used, more precisely the method of questioning. In addition, technique of semistructured interview was used. The interview was done with five respondents who are employees of the Home for the elderly Kaplice. The obtained data were subsequently analyzed using the cluster method. Data were divided into groups according to similarity of the statements related to the particular sections of the interview. There was established the main research question: What is the awareness of the application of forms of comprehensive rehabilitation in care for elderly? In addition, there were defined four sub-research questions of the research. The research showed that the care of seniors is chiefly realized through medical and social rehabilitation, which meets their needs. It is primarily the health care and motivational activities what significantly affects the quality of life of seniors. Educational and social form is not represented much in the care, mainly because of lack of interest from the clients, caused by their poor health. Respondents perceived the comprehensive rehabilitation as comprehensive care that helps individuals to improve their health. However, they recommended the medical environment. They did not mention the possibilities of occupational or educational rehabilitation. This bachelor?s thesis may serve as an information resource for the management of the Home for elderly Kaplice. The gained knowledge may also serve as a source for an expert workshop for employees of Home for elderly Kaplice.

Psychosocial adaptation to paralysis after spinal cord impairment
Šámalová, Kateřina ; Kodymová, Pavla (advisor) ; Černá, Petra (referee)
Coping with life crisis situations and adapting to them is a topic of great human interest and importance. Sustaining a spinal cord injury with the consequent lifelong paralysis may be considered to be one of the most serious life events due to its devastating and wide-ranging impact on people's lives. Spinal cord injury is a traumatic injury, which typically occurs suddenly and without any warning. Its consequences present an enormous physical, social and psychological trauma which the individual has to confront and come to terms with. The changes caused are life-long and affect every aspect of a person's life. Paralysis leads to wheelchair dependence for mobility. In addition, the individual with a spinal cord impairment may become dependent on others for help and assistance in order to be able to carry out most of the tasks of daily living. All the losses and changes caused by the injury have profound effects on both the spinal cord injured person's social roles and those of their loved ones. The individual's and his/her family's psychosocial adaptation to spinal cord injury, which is the main theme of this work, involves an intricate adjustment process. Stage coping theory, which dominates the current scientific literature concerning coping with life crisis situations, is not sufficient for a...

Postpoliomyelitis syndrome - etiology and RHB treatment options
Mejsnarová, Alena ; Demeková, Júlia (advisor) ; Kobesová, Alena (referee)
The bachelor thesis called "Postpoliomyelitis syndrome - etiology and RHB treatment options" deals with issues of this illness, officially recognized as a separate diagnosis until less than thirty years ago. The postpoliomyelitis syndrome can develop in individuals who have experienced poliomyelitis, an infectious viral disease affecting cells of the spinal anterior horns, in their childhood or youth. The main symptoms of this syndrome are: muscle weakness, increased fatigue, muscle and joint pain. They usually appear after a period lasting at least 15 years after primary infection, during which the health state of these individuals is relatively stable. The thesis presents a wide range of pathophysiological processes responsible for the etiology of these so-called late effects of polio. It also deals with the clinical picture, diagnosis and reflects the mental condition of the affected individuals. A large part of this thesis is addressed to therapy of postpolio syndrome, especially rehabilitation, which is an important part of comprehensive care of these patients. It is a literature retrieval of available, mostly foreign sources, dealing with this topic. Also one case report is described at the end of the theoretical part. It includes medical history, examination and treatment of a patient with...

Title: Effect of a comprehensive rehabilitation program of children with asthma bronchiale Objectives: The main objective of this thesis is to show the usefulness and necessity of physical therapy in children with bronchial asthma and how does it affect them. Another objective was to summarize the possible available methods with demonstrations of examples of individual exercises that can be used for this disease. The aim was also monitoring and evaluation of the patient's subjective feelings. Methods: In the thesis we carried out a single arm experiment, where we used comprehensive kinesiology analysis of individual subjects and the methods of lung function tests. From the lung function test, we chose the spirometry test by method of curve flow / capacity and also we used the peak expiratory flow monitoring using peak-flow-meter. We compared the subjective feelings of clients at the beginning and the end of the treatment program. In the period we watched the changes of value in individual measuring techniques. Results: We found that a comprehensive treatment of children asthma has a significant impact on their posture and also on the respiratory functions and subjective perception of individual symptoms investigated clients. All clients received the same treatment and each of them occurred in...

The issue of nursing care about patients with geriatric fragility.
ŠINDELÁŘOVÁ, Renata
Bachelor thesis titled The issue of nursing care about patients with geriatric fragility is a theoretical work, that tries to catch and describe the most information about geriatric frailty and specifics of nursing care for geriatric patients, who are withering away. Geriatric frailty has significant influence on quality of seniors life who currently live longer. Because the population is getting older and number of geriatric patients is increasing and it will be stil in the future, the geriatrics and the gerontology began to focus on geritraic frailty in their research. The geriatric frailty is called "The Holy Grail" of this medicine that tries first to maintain the longest possible psychical and physical senior activity and to turn away institutionalization of geriatric patient mainly by appropriate interventions in the provision of comprehensive nursing care. The aim of great effort in this work was to map and find out all details not only about geriatric frailty, but also about areas with influence on the emergence and development of the geriatric frailty and also on quality of senior life. Geriatric frailty does not have set definition yet and it is not now known if it is a syndrome, a symptome or a medical diagnosis. Involution, that goes along and has significant influence on men's life, causes natural biological aging process of organism, which is manifested by decrease or loss of functional human reserves. If these reserves are also influenced by inner factors for example multimorbidity or undesirable effects of medicaments or external factors, where we put malnutrition, sarcopenia, cognitive deficit, inactivity, instability but also loneliness, excessive load, inappropriate housing and insufficient financial income, than the "spiral of deteriorating" is developing and heading towards disability, immobility and danger of senior life. It is necessary to add that senior with geriatric frailty may not be always ill but he wither away. So effort of my thesis is to bring meaning and impact of "geriatric frailty" and "spiral of deteriorating" in the seniors life, that are not in the awareness of medical staff who provide nursing care to seniors (Kalvach, 2008). Geriatric frailty is getting worse by its progressivity and causes emergence of other syndromes, of which the most serious are immobilization syndrome and terminal geriatric deterioration followed by death. We need the presence of three basic characteristic of five to diagnose geriatric frailty in the eldery. It is malnutrition, sarcopenia, self-reported exhaustion, reduced level of mobility and weakness. As already said geriatric frailty is now the main theme of geriatric medicine, gerontologists and geriatrists, and because it can meaningfully contribute to providing of nursing care with propriate interventions, I tried in my thesis to determine specifics of nursing care focusing on areas, which have significant influence on emergence and development of geriatric frailty. It is primarily nutrition, rehabilitation nursing and motivation of frailty seniors, but also decubitus, falls and injuries of patients, inappropriate communication with elements of ageism or sleep disturbances can affect the emergence of frailty. The result from acquired information is that the main goals of nursing care for patients with geriatric frailty are to make eat, get to go and motivate patient. Effort this thesis is not to disparage present nursing care for geriatric patients, but to explain and give information about the necessity of greater focus of nursing care on risks mentioned above. Nursing care will have to make certain changes, as well as the geriatric medicine is adapting to the population aging and geriatric frailty. If this bachelor thesis will enrich awareness of nurses and other members of multidisciplinary team with problem of geriatric frailty, specifics of nursing care of patients with geriatric frailty, attitude and motivation of these risk patients, then the.
