Free time activities of adults with mental disability in Prague - the possibility of support in the planning and implementation

Diploma thesis "Free Time Activities of Adults with Mental Disability in Prague - the Possibility of Support in the Planning and Implementation" was written with the general objective to point out this issue in the Czech Republic. The theoretical part is aimed at introduction into areas, which are closely related with this issue. It deals with free time in general, characteristics of mental retardation and the adulthood of a person with intellectual disability. The final chapter, which deals specifically with leisure of people with intellectual disabilities, is based primarily on foreign studies and experience. The practical part consists of two investigations. The first was focused on finding organizations providing support to people with intellectual disabilities in leisure time and on precise determination of their activities (through interviews). A directory has been created with a list of offered individual support and group activities. The impact of a three-month training on motor functions in preschool hockey players

Title: The impact of a three-month training on motor functions in preschool hockey players Objectives: The goal of this diploma thesis is to summarize the knowledge about motor functions in preschool children. Also to find suitable test methods which should help to evaluate the motor level of children this age who are attending the preparation course in the hockey club Sparta Praha and also evaluate the influence of a three month hockey training on postural maturity and development of motor skills on the regular surface and on the ice. Methods: The pre-experiment was conducted in hockey preparation course HC Sparta Praha in preschool children during their training. It was carried out using 5 tests evaluating postural maturity, which were evaluated by two physiotherapists and 4 tests evaluating the level of motor skills on the regular surface and on the ice, which was measured on time. Tests were performed twice, with a three-month interval. Results: After three months of training, there was no substantial improvement in postural maturity assessment tests, although the consensus of the two physiotherapists in the assessment of the postural tests maturity was relatively high, for both measurements. Prescription of the new antidepressants in psychiatrist's surgery

The main aim of presented labour was to compare the prescription of the new antidepressants and the others as SSRI, TCA, IMAO.The comparison has been done in generally and also with regard to patients's age and gender. The research has been done in group of 387 patients, actually 370 ones, in psychiatrist's surgery whom prescription had not been restricted from new preparations. The results have been worked up statistically. The new antidepressants are heterogeneous group of medication containing active ingredients with specific act of action and with lower side effects. Bupropion, escitalopram, milnacipran, mirtazapin, reboxetin, trazodon and duloxetin belong to this medicine group. In practical part of the labour the prescriptions evaluation of patients group (insured people in 2005 and 2006) in Prague psychiatrics surgery has been done. The results have confirmed my own assumption and that's total antidepressants usage is on increase and the new antidepressants's participation as well. SSRI have been the most often prescribed. The prescriptions of new ones was showed 16 % of all groups of antidepressants in 2005. A year later the prescription was risen up to 19 %. Church of the Virgin Mary The final thesis deals with the theme Church of the Virgin Mary of Karlštejn castle. This space is situated on the second floor of the Virgin Mary Tower. The thesis we notice on the gradual transformation of the originally inhabited room into the church. The individual architectural changes till 20th century are solved. The essentials part of the project deals with the iconographic analysis of the church. There is given a description of the individual illustrations of the Apocalypse, the so-called Relic Scenes and the drawings under the ceiling that were destroyed at that time. The Problems of Craniocerebrals Injury in Intensive Care

The topic of this Thesis is the intensive care of patiens suffering from a craniocerebral Injury, such injuries contributing to high mortality and morbidity in patiens with trumas. The theoretical part of this Thesis gives a brief outline of the anatomy and physiology of the head and goes into a greater detail into the kinds of injuries, their symptoms, diagnostics, and treatment. The empiricical part hereof is produced in the form of case study and focuses on the intensive care of patients so injured. The chosen patient in the case was a jung man after a traffic accident who had suffered from a very serious craniocerabral injury. Monitored is his hospitalization from his reception to his release from the AR ward. The tenth day hospitalization is dealt with in detail.

Cleft sentences in English and Norwegian

The subject of this thesis is the use of the cleft construction in English and Norwegian. These languages employ a formally similar construction to focus a sentence element, but the use of the construction is not always identical. The analysis is carried out on English and Norwegian translations of Czech texts. The main aim of this thesis is to identify and analyse possible types of motivation for the use of the cleft construction. The studied types of motivation are the FSP, textual and syntactic motivation. The analysis of the FSP motivation is based on the theory of the FSP as described by Jan Firbas (Firbas 1992). The list of textual functions is based on the work of Jan Firbas (Firbas 1995) and Hilde Hasselgrd (Hasselgrd 2004). Some types of the syntactic motivation are proposed by Libuše Dušková (Dušková 1999: 319), but the types described in this thesis result from the present analysis. In addition to the motivation for the use of the construction, the thesis deals with Norwegian and English counterparts of the analysed cleft sentences. Quality of life of seniors hospitalized in long-term care.

The basic theoretical basis: According to demographic development the average and maximum lifespan prolongs. Therefore global population spends a bigger part of adulthood in their old age. This phenomenon is irreversible and brings significant changes into global geriatric nursing and we need to take note of the care that is provided to old patients. The seniors form an irreplaceable and a more and more considerable part of population, they create a varied group with individual needs, priorities and demands in sickness as in health. It is the very polymorbidity of senior patients which is often the reason for a long-term hospitalization. Nowadays, for these purposes more and more beds are built in long-term care facilities (LTCF) which are officially defined as specialized medical institutions which provide care to patients whose health states cannot be substantially improved by any medical treatment as they worsen without a continuous care. The life quality of seniors and the quality of the provided care in these facilities are the main long-term care level indicators. The goals of the thesis: 1) Ascertain the life quality of seniors hospitalized in long-term care institutions. The research questions: 1) How does the hospitalization of seniors in LTCF influence their subjectively perceived quality of life? 2) In which areas do seniors hospitalized in LTCF feel the difference of life quality in LTCF and at home? 3) How are the patients in LTCF satisfied with the quality of health care? 4) Within the rise of seniors' life quality in LTCF, in which areas should the nurse improve the provided care? Methods employed: For the empirical part of the thesis the qualitative analysis of gathered data method was used. The data accumulation was implemented by semi-structured interviews with clients-patients hospitalized in LTCF. The instructions for the interviews were adopted from a QOL Research Unit life quality model of the university in Toronto. This model emphasizes the individual's relations towards the surroundings and the possibilities of maintaining and improving skills. It is divided into 3 categories - existence, togetherness and adaptability. Interview transcriptions create a part of the thesis on an enclosed CD. For illustrative purposes, the results of the research were transformed into mind maps using the XMind software. There are also annotations with respondents' quotations concerning individual map as a part of the thesis. The results: The analysis of in-depth interview gathered information brought several factors which are highly subjective and have a logical level of individuality. The results concerning the research of the seniors' life quality dimensions are more or less in agreement with the basic division found in professional literature. However, they bring a further view on these dimensions and introduce subjective opinions, wishes and evaluation of seniors who found themselves in unknown environment of LTCF. The positive influence of nurses, which often impact the satisfaction of seniors in a medical institution, proved its importance. However, it was the area of communication skills of medical staff which was mentioned as lacking or insufficient. Also the fields of free time activities, seniors' adaptation in LTCF and social isolation were found to be neglected. Conclusion: The long-term care becomes, thanks to demographic development, the most rapidly evolving kind of health care. The emphasis on life quality of hospitalized seniors, as their biggest consumers, should be a matter of course for every medical and social facility. The contribution of the thesis is in detailed mapping of the issue of life quality in LTCF, warning against lack in health care and the neglected fields of life quality.

Psychosocial load at work of nurses and social workers

The Thesis deals with the questions of psychosocial load at work for nurses and social workers. The Thesis is divided into two sections. The theoretical part deals with definitions of terms like caring professions, load and stress, then load at work and psychosocial load. There is also a brief characteristic of the professions of a nurse and a social worker. The aim of the empiric part was to find out on a selected sample of respondents (i.e. nurses and social workers) how the respondents perceive the character of their psychosocial load at work and whether they are at risk of burnout syndrome, to find out whether nurses or social workers perceive the psychosocial load more intensely and whether there is a difference in the risk of burnout syndrome between them. A quantitative questioning method and questionnaire technique were applied on data collection and processing in the research part of the Thesis. An anonymous questionnaire was used for the investigation. The questionnaire preparation was based on a questionnaire provided by my thesis guide. The original questionnaire was more extensive and was only designed for nurses, which was why I had to select suitable items and add some special items applicable on nurses as well as social workers. The questionnaire consisted mainly of closed questions where the respondents chose from five answers the one best corresponding with their own opinion. I set 3 hypotheses before starting the research itself. The questionnaire answers were processed both, manually by means of the stroke method and by a statistic programme SPSS. Segment and bar graphs in Microsoft Office Excel were used for graphical interpretation of the results. After evaluation of the questionnaire research results I found out that the previously set hypothesis H1 suggesting that nurses and social workers perceive various elements of their psychosocial load differently, was confirmed, hypothesis H2 suggesting that nurses are more loaded by the work with patients/clients than social workers was not confirmed, and hypothesis H3 suggesting that nurses are at higher risk of burnout syndrome than social workers was confirmed. The results of my thesis may serve to inform general public as well as professionals on the problems in question. They may also serve as a possible incentive for managerial staff to pay higher attention to the problems of psychosocial load at work among caring professions at individual medical care facilities. The results may also serve as a base for further studies and research in the field.
