Adherence to treatment regime of patients with chronic kidney disease

Introduction: Chronic kidney disease in the progressing stage can significantly affect the quality of the patients' lives. It is estimated that more than 10 % of the worldwide population suffer from this disease. Proper nursing care, lifestyle modification and following proper medical treatment could significantly slow down the progression of disease. The main aim of the bachelor thesis was to find out the level of adherence to the treatment regime of patients with chronic kidney disease. Methodology: Quantitative research was chosen to process the practical part. Results were collected using a standardised questionnaire containing 50 questions to determine the attitude of haemodialysis centre patients towards adherence to dialysis plan, medication, fluid restriction and diet. Collected data was analysed and statistically processed using Microsoft Excel programme. Results: The research sample was made up by 94 respondents. The results show that 58,5 % patients undergoing haemodialysis treatment have an adherent attitude to the treatment. Keeping therapy treatment is influenced by the attitude of nursing staff. For most of the respondents, it is important to follow a treatment regime, so that their health does not deteriorate or because they realise that the condition of their kidneys requires them to follow experts' advice. Conclusion: The results of our thesis highlight the importance of nurses paying attention to patients with chronic kidney disease. Patients with whom nursing staff regularly talk about their treatment follow recommendations more conscientiously. The most frequent problem among patients is misunderstanding treatment and recommendations.
The patient healing hemodialysis at home by the paramedic´s perspective

This bachelor's thesis focuses on the issue of domestic haemodialysis with special attention to the role of paramedics in dealing with acute problems associated with this procedure. The work is intended as theoretical, without the author´s own research. It aims at reviewing treatment options for kidney failure, describing the principle of home haemodialysis, its benefits and disadvantages for patients and their families. Special attention is paid both to potential risks brought about by this method of dialysing blood and to ways of addressing the complications from the perspective of pre-hospital care. To achieve its goals, the work outlines and compares current attitudes towards these issues both in this country and abroad. Its introductory part summarizes the structure and functions of kidneys, the most frequent causes of renal diseases as well as their possible therapies. Further on, it introduces in greater details both general and specific procedures applied by emergency medical squads in case of complications during domestic haemodialysis. The main scientific contribution of the work consists in the very synthesis of the present knowledge and methods of treatment and - at the same time - in defining the so far unresolved issues. The part of the thesis devoted to home haemodialysis, which still represents a relatively rarely used method, underlines particularly the fact that emergency medical procedures responding to specific complications connected with this treatment have not been sufficiently defined yet. The thesis therefore comes to the conclusion that paramedics facing acute problems associated with domestic haemodialysis have to rely mainly on general procedures determined for providing pre-hospital care. That is why this field still remains open for further theoretical research. Detailed potential practical steps aimed specifically at complications during home haemodialysis are in demand as well.
Specifics of diet in patients with chronic kidney disease

Approximately 10 % of local population is affected by chronic kidney disease. CKD is not only a major health issue but it does have socio-economic impact on society. Early diagnosis and adequate therapy helps to delay the necessity of dialysis treatment, leads to a better quality of life of patients and can lower the economic costs of the therapy. Adequate therapy and regime intervention may positively influence wide range of risk factors of CKD progression. Patients with CKD have to comply with strict dietary restrictions including reduction of proteins. When starting dialysis, proteins intake increase is necessary, because of amino acids dialysate losses. Registered dietitian (RD) aims to maintain an optimal nutritional status of the patients and to minimize complications of CKD. Patients should be educated about nutrition principles during predialysis and diet changes after initiation of dialysis treatment. This bachelor thesis aims to establish an awareness of the dialyzed patients about nutrition measures. Questionary survey was used in set of 86 patients treated on dialysis in Klatovská nemocnice. Dialysis pump

Dialysis is still very important treatment process today which needs to be constantly improved. This thesis focuses on the development of a pump for dialysis units, specifically on the pressure pulsations reduction in the rotary peristaltic pump. Using experiments, the problem of peristaltic pumps is explained. With the computational modelling there are proposed design modifications leading to the pressure pulsations reduction.
Muscle metabolism and its effect on physical condition in patients with chronic renal failure receiving hemodialysis

Title: Muscle metabolism and its effect on physical condition in patients with chronic renal failure receiving hemodialysis Objective: Assess the status of muscle metabolism in patients starting hemodialysis and patients receiving hemodialysis long time, to determine some changes in muscle metabolism in both groups in the time period and ultimately assess its impact on physical condition and quality of life of hemodialysis patients. Method: Fill anamnestic questionnaire and examination of body composition using bioimpedence device (BCM) in the group of patients with chronic renal failure who are beginning to be treated with hemodialysis and a group of patients treated with hemodialysis for several years. Compare the results of both groups. To obtain additional information from medical records. Results: It was found that hemodialysis therapy affects the status of muscle metabolism in terms of reducing muscle mass (LTM). The dependence of LTM, however, the duration of hemodialysis treatment assays. The study showed that if patients are nutritionally stable and regularly engaged in some physical activity, loss of muscle mass is lower than in individuals who have a passive way of life. The aim of this bachelor's thesis entitled "The effect of dialysis on activities of daily life" is to provide a brief overview of relevant basic knowledge of hemodialysis and peritoneal dialysis using existing and already published litarature and try to determine the influence of the dialysis on activities of daily life. The thesis is devided into theoretical and empirical part. The theoretical part is focused on kidney disease, hemodialysis and peritoneal dialysis, principles of dialysis therapy, their differences, complications, dietary measures and approaches to dialysis. Further attention is paid to the comparison of the advantages and disadvantages of both methods of dialysis among themselves. Part of the work deals with human needs and activities of daily life. The empirical part consists of an exploratory survey, which was given to patients in dialysis centers Fresenius Medical Care Prague 5 - Motol, Prague 10 - Vinohrady, Prague 4 - Krc, Prague 9 - Vysočany, Slaný, Louny, Chomutov, Most and dialysis center department of Internal Medicine Strahov Všeobecná fakultní nemocnice in Prague. Comparison between the quality of life of the patients with peritoneal dialysis in the year of 2004 and 2012

The Bachelor Thesis looks into the standard of quality of life of patients undergoing peritoneal dialysis. The aim of my work was to determine if the quality of life of the patients treated within the period of eight years moved towards any progress. Whether the patients have any problems at work, in their personal life, and if their illness allows them to enjoy leisure activities and last but not least, if they perceive any change in applying this treatment and how. In the theoretical part of my thesis, I am focused on summarizing the information related to anatomy and physiology of kidney, renal failure and compensation of renal function. The comprehensive description of the quality of life of these patients undergoing peritoneal dialysis forms an integral part of my work. In the empirical part, I have used quantitative research and data I obtained via questionnaire survey. In my further investigation I applied comparative research and its results were compared with the research undertaken in 2004. The nurse's role in patient care elimination methods

The aim of thesis was to map the care of nurses and paramedics for patients on continuous renal replecemant therapy. Find out what nurses/paramedics have difficulty taking care of the patients and what would have welcomed into practice to improve care. The theoretical part focuses on the description of the method of elimination, nursing care and competence of health professionals in the care of patients on elimination method. The research was compiled by the method of quantitative research, for whose realization was used a questionnaire. The questionnaire was intended for nurses and paramedics caring for patients on continuous elimination method. For statistical data processing was used responses from 105 respondents working in the Institute of Clinical and Experimental Medicine. The results of the survey indicate that nurses and paramedic extensively involved in the specialized care of patient with continuous elimination method. Medical staff perceived as the most difficult managing a large number of skills associated with the preparation and handling of elimination device in this area. On the basis of these findings was prepared brochure focusing on continuous elimination method, which is the output of this work. The most frequent locomotor disorders in patients with chronic renal failure on dialysis

The diploma thesis deals with muskuloskeletal system disorders in hemodialysis pacients with chronic renal failure. Theoretic informations about the most frequent disorders of musculoskeletal system in uremic patients, patophysiologic consequences, clinical symptoms, posibility of investigations and common therapeutic management. Secondly this thesis interests about of annamnestic data, physiotherapeutic investigation and evaluation of the data. Aim of this study was supported importance of the physiotherapeutic intervention.
