Case study of physiotherapy treatment of a patient after ischemic stroke in basal ganglia with right sided spastic hemiparesis

Název: Případová studie fyzioterapeutické léčby pacienta po ischemické cévní mozkové příhodě v bazálních gangliích s postižením pravé strany těla v podobě spastické hemiparézy Cíl: Tato práce si klade za cíl zkonsolidovat teoretické poznatky o ischemické cévní mozkové příhodě a představit podrobnou kazuistiku pacienta trpícího postižením pravé strany těla v důsledku ischemické cévní mozkové příhody v bazálních gangliích. Metody: Tato bakalářská práce je rozdělena do dvou hlavních částí: obecné a specifické. Obecná část komplexně pokrývá teoretické aspekty související s cévní mozkovou příhodou, zahrnující typická fyzioterapeutická vyšetření pro diagnostiku, spolu s výběrem běžně používaných metod a postupů ve fyzioterapeutickém řízení těchto pacientů. Specifická část se zabývá kazuistikou pacienta po ischemické cévní mozkové příhodě, zaměřuje se na následné postižení pravé strany těla v podobě hemiparézy. Zahrnuje podrobné popisy počátečního a finálního kineziologického vyšetření, průběh jednotlivých terapií a komplexní shrnutí výsledků terapie. Altered morphology of white and grey matter in patients with Alzheimer disease and Schizophrenia on MRI

Cortical folding of the anterior cingulate cortex (ACC), particularly the cingulate (CS) and the paracingulate (PCS) sulci, represents a neurodevelopmental marker. Deviations in in utero development in schizophrenia can be traced using CS and PCS morphometry. In the present study, we measured the length of CS, PCS, and their segments on T1 MRI scans in 93 patients with first episode schizophrenia and 42 healthy controls. Besides the length, the frequency and the left-right asymmetry of CS/PCS were compared in patients and controls. Distribution of the CS and PCS morphotypes in patients was different from controls. Parcellated sulcal pattern CS3a in the left hemisphere was longer in patients (53.8 ± 25.7 mm vs. 32.7 ± 19.4 mm in controls, p < 0.05), while in CS3c it was reversed-longer in controls (52.5 ± 22.5 mm as opposed to 36.2 ± 12.9 mm, n.s. in patients). Non parcellated PCS in the right hemisphere were longer in patients compared to controls (19.4 ± 10.2 mm vs. 12.1 ± 12.4 mm, p < 0.001). Therefore, concurrent presence of PCS1 and CS1 in the left hemisphere and to some extent in the right hemisphere may be suggestive of a higher probability of schizophrenia. Measurement of an hippocampal area or volume is useful in clinical practice as a supportive aid for diagnosis of Alzheimer's disease....
Cognitive sequelae from methanol poisoning

This thesis focuses on cognitive functions in patients after methanol poisoning. The theoretical part consists of a description of methanol, characteristics of methanol poisoning and its neurological sequelae. Methanol poisoning affects the basal ganglia, therefore we describe them and their role in cognitive processes. The theoretical part also describes a case of mass methanol poisoning in the Czech Republic in 2012-2013. The practical part compares results of a control group and a group of patients poisoned with methanol in cognitive assessment. Results show that the patients after methanol poisoning have significantly lower scores in tasks testing memory and executive functions than the control group.

The work consists of three parts - theoretical, focused and practical. The theoretical part summarizes problems related to Parkinson's disease and its pathogenesis, clinical presentation, diagnosis and treatment possibilities. The focused part refers to physiotherapy of patients with PD. Emphasis is layed on modern strategies published in European Physiotherapy Guideline for Parkinson's Disease published in 2014. The practical part consists of two case studies of patients with Parkinson's disease, which include close kinesiologic examinations (entry and final), individual therapy design, realization and comparison of measured parameters.
