Processing and Visualization of Diagnostic Data from a Bionic Upper Limb Prosthesis

Bionické protézy se běžně používají k tomu, aby pacientům pomohly efektivněji vykonávat každodenní činnosti. Zahrnují různé metody získávání vstupu od uživatele a používají elektromotory k ovládání prstů a/nebo jiných částí končetiny. Bohužel míra odmítnutí protézy dosahuje v průměru 44 % [64], což znamená, že téměř polovina pacientů přestane protézu používat. Protéza Z-Arm od společnosti Z-Bionics se pokouší řešit nízkou míru akceptace mimo jiné tím, že zaznamenává diagnostické údaje a pomáhá pacientovi, když detekuje nesprávné používání nebo možnou vadu protézy. Protože však návštěvy pacientů nejsou dostatečně časté, aby specialisté získali aktuální diagnostické údaje, je třeba navrhnout vhodný způsob jejich získávání na dálku. Jako řešení byla implementována mobilní aplikace využívající Bluetooth ke komunikaci s protézou doprovázená rozšířením interní webové aplikace o nové stránky, koncové body API a databázi MongoDB. Ačkoli testování s pacienty stále probíhá, první dojmy zaměstnanců a pacientů jsou vesměs pozitivní a naznačují možné zlepšení komfortu při používání protézy.

I Abstract Author: Ondřej Peleška Title: Case report of physiotherapeutic care of a patient after transfemoral amputation Objectives: To provide a comprehensive overview and analysis of amputation in the lower limb region with a focus on transfemoral amputation. To present the current surgical procedures, rehabilitation methods, psychological aspects and prosthetic solutions, and then to develop a case report of physiotherapy care of a patient after transfemoral amputation. Methods: The theoretical part of this bachelor's thesis was elaborated with the help of scientific literature and scientific articles available mainly online. At the same time, services based on the use of artificial intelligence were used, namely Evidence Hunt, Microsoft Copilot for Institutions, Connected Papers and DeepL. The special part of this thesis was prepared using practical skills within the scope of the bachelor's degree in physiotherapy during my continuous professional placement in the medical facility of Nemocnice Na Homolce. Results: The physiotherapeutic intervention led to an improvement in the patient's physical and psychological condition and subsequently to her discharge to her home environment. Education of diabetic patient with diabetic food after amputation

This bachelor thesis is focused on education of patients with diabetic foot syndrome with limb shortening amputation and prevention of higher amputation. The theoretical part deals with the disease diabetes mellitus, its complications, diabetic foot syndrome and education on prevention of amputation. This disease is currently prevalent and its incidence is increasing. The aim of the study was to find out who and by what method most often educates patients with diabetic foot syndrome about prevention of amputation, in what way patients perceive shortcomings on the part of education and to map their knowledge about prevention of diabetic foot. A quantitative method of investigation was used for this study and data was collected using a self-administered questionnaire which was filled by a total of 30 respondents. The research sample consisted of 30 respondents (100 %). The results show that 24 people (80 %) are educated about amputation prevention by both a nurse and a doctor and 6 people (20 %) by a doctor only. The form of education was oral and written for 21 respondents (70 %) and oral only, in the form of an interview for 9 people (30 %). The biggest barriers to education were lack of time for 13 respondents (43 %), inappropriate environment for 9 respondents (30 %), lack of self-will to listen to the educator for 7 people (23 %), and lack of empathy from the educator for 3 people (10 %). 10 respondents (33 %) described their knowledge of prevention as good, 19 respondents (63 %) described their knowledge as normal and 1 respondent (3 %) described their knowledge as poor. The contribution of this bachelor's thesis can be pointing out the shortcomings in education and trying to eliminate them. This thesis contains an overview of recommendations that should be followed by patients with diabetic foot syndrome, so it can also serve as an educational material for nurses and diabetic patients.
Design of mechanism for pronation and supination of the upper limb prostheses

The goal of of this work is to propose a mechanism for pronation and supination of the upper limb prosthesis and its subsequent implementation of functional prototype. The teoretical part summarizes the basic types of forearm amputation and the effect of restricting rotation of the forearm. It also describes the basic distribution of upper limb prostheses and drive types. Construction design includ complete technical documentation is made in Autodesk Inventor Professional 2013 and Rhinoceros 5.0 SR8. The prototype is made of aluminium and ABS plastic.
Design of lower-limb prostheses

The goal of this bachelor's thesis is to give an overview of the current state of lower limb prostheses after amputation between the hip and the knee. It deals with the human gait, its analysis and with deviations in gait with the prosthesis. It also describes basic division of lower limb prostheses. The main part is the description of these prostheses designs and especially the design of main components and general overview of available types.
Kinematic analysis of movement of cyclist with transtibial prosthesis.

This work is aiming to the area of biomechanics and deals with the experimental finding of appropriate kinematic ratios of crank mechanism on racing bicycle and transtibial prosthesis of racing cyclists with an assessment of kinematic ratios depending on the overall athletic performance. The primary aim of the work is investigation of the impact of inappropriate existing kinematic ratios of leg prosthesis and crank mechanism on the kinematics and dynamics of top athlete. The work presents experiments that proposes find out appropriate kinematic ratio of the length of kinematic members and possibility to improve overall athletic performance. The measurement is supplemented about cycling analysis of racer realized via cameras tracking system and measuring muscle activity by EMG. The work also includes a description of impact of various factors on cycling.
Mechanical upper limb prosthesis

This thesis elaborates on design and fabrication of an artificial mechanical upper limb prototype for transradial amputations, i. e. for patients with stump between elbow and wrist joints. The artificial limb is designed for use in a wide spectrum of patients. Therefore, the main components are designed to be applicable to smaller as well as larger hand dimensions. The artificial mechanical limb is designed to be controlled by the other hand of a patient and enables grasping of objects via cylindrical and hook grip. Fingers are driven by tension springs and can be locked in extension. Clenching can be initiated by pressing a button, which unlocks the fingers, thus an object can be grasped. Wrist joint is adjustable when pressed a release button. Forearm joint is locked by preloaded spring. The lock of the forearm joint is released by sufficient torque. New position of the forearm joint is locked automatically when the torque is not being applied. Prototypes of the artificial mechanical upper limb were manufactured by using of rapid prototyping technologies (FDM, SLM) as well as CNC milling machine. All parts of the artificial limb were designed with regard to manufacturability by conventional methods such as casting, injection and the like. The artificial limb prototype had been designed on 3D data basis of a specific patient, who already has tested the prototype. Following materials were used on the prototype: ABS plastic, SikaBlock M940 polyurethane, aluminium and 316L stainless steel.
Prosthetic Solutions to Transmetatarsal Traumatic Amputation

Title: Prosthetic solution of traumatic transmetatarsal foot amputation Objectives: The main goal of my work is to design a prosthetic solution for a patient after a traumatic transmetatarsal amputation of the leg with the intention of restoring the function of the leg with respect to its natural appearance, maximally reducing the risk of injury to the patient so that he can use normal sports shoes. A partial goal is to increase the mechanical resistance of the prosthesis during increased physical activity and to create a production procedure that can be used by other active patients. Methods: The bachelor thesis consists of a theoretical part and a practical part. In the theoretical part I primarily draw on professional publications and articles related to the given issue. The practical part consists of the actual case study of the patient with a focus on the production of a prosthesis according to his disability, which is based on the data obtained from the analysis of posture and gait. Results: The case report was processed within two months in the non-state medical facility Proteor CZ s.r.o. Ostrava, based on the specific needs of the patient. Furthermore, I will present a case study of physiotherapy treatment of a patient with a diagnosis of unilateral transfemoral amputation and evaluate the effect of therapy with subsequent discussion. Methods: The bachelor thesis is divided into theoretical and special parts. The theoretical part is a research elaboration of kinesiological background and theoretical knowledge regarding lower limb amputations with a focus on the subsequent rehabilitation, especially its physiotherapeutic part. In the special part, a case report of a patient after transfemoral amputation, which was prepared in the Regional Hospital Kladno, is presented. The case report contains the patient's history and initial kinesiological analysis on the basis of which short-term and long-term goals were developed. vi Abstract Title: Case study of ortho-prosthetic care of a patient with the diagnosis of transtibial amputation Objectives: The main goal of this bachelor's thesis is to process a case report of a patient with a detailed description of the production and application of a prosthetic device. Another goal is to process basic theoretical knowledge about anatomy, amputations and prosthetics of the lower limbs with a focus on transtibial prostheses. Methods: The bachelor's thesis is divided into a theoretical and a practical part. The theoretical part summarizes basic knowledge regarding anatomy, amputations and transtibial prosthetics of the lower limbs. The practical part contains a case study of a diabetic patient with the diagnosis of lower leg amputation, which consists of a description of their medical history, measurement taking and the process of manufacturing a prosthetic device. Results: The case report was prepared as part of my professional training at Protetika s.r.o.
