Trigger points problems in physiotherapy

My bachelor thesis deals with trigger points (MTrPs) problems in physiotherapy and their therapy. The aims of the thesis was to map the trigger points problems and clarify possibilities oh therapy these points. In the theoretical part of the bachelor thesis I describe division, causes, and mechanism of origin, manifestations and consequences, therapy, presence and ways of chaining, referred pain of trigger points and head-forward posture. For the practical part of the work, I chose the method of qualitive research. There were 10 probands in the research, for which I looked for trigger points and then based on theoretical knowledge, I compared the presence of trigger point patterns in various symptoms of the probands. Then I treated 4 of the selected probands with these points. During therapies, I combined manual techniques, thermotherapy and physical therapy. In my opinion, the topic of trigger point is still open and some questins are not answered yet. The subject of research is still the mechanism of MTrPs. The most frequetly mentioned theory is the Integrated hypothesis. There are other hypothesis that are not sufficiently scientifically based. Ways to chain of MTrPs and ways or mechanism of referred pain are others not entirely explanatory chapters. According to theories of several authors, reffered pain is manifested by activity of nervous system at the level of the spinal cord. Based on researches by various authors related to trigger point therapies, the following conclusions can be drawn. When comparing ischemic compression and the trigger point pressure release technique, it is better to use the pressure release technique. When comparing manual techniques with dry needle application, it can be said that both techniques have a comparable effect. However, the use of tape should be combined with another technique, as the tape acts rather locally. After treatment of MTrPs in probands, MTrPs reformed due to poor movement stereotypes. Therefore, in my opinion, it is necessary to pay attention to the reeducation of movement patterns such as poor posture.

Name: The Effect of Facilitating Kinesio Taping of lower scapula fixators on pain threshold of trigger points in the m. trapezius pars descendens Objectives: This study is a pilot study. The goal of this study is to determine effects of Facilitating Kinesio Taping of lower scapula fixators on painfulness of trigger points in the m. trapezius pars descendens. Methods: The study took place in a private medical office with thirteen patients. All of these patients were physically examined using standard tests of physiotherapy. Patients were measured using a pressure algometer to assess trigger points as painful. They reported the intensity of pain on VAS. The places of measurement were trigger points one and two in the muscle according Travell and Simons. Data were analyzed using the statistical program SPSS 15.0 and Microsoft Office Excel. Results: This study had three hypotheses. In the case of the first hypothesis, I concluded that KT has an effect on the pain threshold and its application will reduce the pain threshold in m.trapezius pars descendens. Statistical significance was the value of p = 0.03 and p = 0.011, the value of material significance (101.31 kPa and 106.7 kPa) confirmed my theory that after KT will increase the pressure of 100 kPa in the TrP1 and TrP2. Clinical effects of treating extraocular muscles

Extraocular muscles have the structure of striated muscle with a very fine innervation. In addition to the obvious function of moving eyeballs to ensure binocular vision, they have an irreplaceable role in the field of proprioception, in maintaining balance and posture. In physiotherapy, we use the eye movement to facilitate/for inhibition of various muscle groups or even whole muscle chains. Extraocular muscles themselves and their condition generally eludes our interest, although even there we find the first signs: the study of the relationship of trigger points and cephalea. The theoretical part of this thesis provides comprehensive information on the anatomy and function of the eye muscles. It also deals with selected pathologies that primarily relate to other structures of the human body, but whose manifestations can also be observed on the function and activity of extraocular muscles. A separate chapter is the relationship of extraocular muscles, field of vision and stability. The practical part of the theses is designed as a pivot study. The application of combination therapy in the treatment of cervical and lumbar spine pain

This bachelor thesis deals with the effect of combination therapy (simultaneous application of electrotherapy and therapeutic ultrasound) in the treatment of vertebral algic syndrome in the cervical spine and lower back with a significant functional component (trigger points) without radicular symptoms. Furthermore, the effect of the treatment is compared with the effectiveness of transcutaneous electroneurostimulation. The theoretical part of this bachelor thesis is devoted to the description of the vertebral algic syndrome and the used treatment methods. The research part is focused on verifying the effects of combination therapy and transcutaneous electroneurostimulation, using each method in combination with individual therapeutic exercise. A total of twenty adult participants were included in the study, ten of whom were treated with combination therapy and ten were treated with transcutaneous electroneurostimulation. The effects of the therapies have been evaluated using selected pain questionnaire methods (visual analog scale, short form of the McGill Pain Questionnaire, Neck Disability Index, Oswestry Disability Index) and combination therapy intended for diagnostic purposes. The objective of this thesis is to provide a comprehensive summary about the topic of myofascial trigger points, which are considered to be the most common cause of myofascial pain and dry needling, which presents one of the therapy options. A part of the topic summary is dedicated to the Czech legislation concerning the application of dry needling in the physiotherapy practice. The main objective of this thesis is to create a systematic research of clinical studies, which compare the efficacy of dry needling to standard methods of physiotherapy. "Case Study of Physiotherapy Treatment of a Patient with Pain in the Medial Part of the Left Calf" The following Bachelor thesis took place at the outpatient rehabilitation department of the Central Military Hospital, in Prague, for a period of two weeks. The aim of this work is, firstly, to present a theoretical overview about the area of the body subject to the patient's complain, exploring the leg and the triceps surae from an anatomical and kinesiological perspective. Then the muscular pathological finding that is the most probable cause of the patient's pain is investigated. The second part of the thesis concerns the practical work performed in hospital and wants to show if and how the initial physical condition displayed by the patient can change through the application of appropriate physiotherapeutic procedures, which are based on the results of examinations and on the investigation of the possible causes of the triceps surae pain. The 30-year old patient showed pain to the touch in a restricted area of the medial aspect of the left triceps surae that was also tense and swollen.
