Psychosocial aspects in the lives of children suffering from errors of metabolism

The present bachelor´s thesis titled {\clq}qPsychosocial aspects in the lives of children suffering from errors of metabolism`` deals with the issues related to hereditary metabolic derangements. It puts together knowledge and examples from day-to-day lives of people suffering from hereditary metabolic disorders. The main purpose of the treatment and nursing care for patiens diagnose with hereditary metabolit errors is to help these accomplish ideal development in their childhoods and a maximum of independence, social integrity, and self-esteem in adolescence and adulthood. These goals can only be accomplished thanks to a multi-system approach. The theoretical part describes heredity, clinical symptoms, diagnostic methods, and treatment of hereditary metabolic disorders. It deals with phenylketonuria (PKU) in more details, it clarifies the mechanism of PKU origins, screening, symptoms, treatment, forecast, and also maternal phenylketonuria. The following part compiles known facts about the personality of a chronically ill child, its psychic needs, and social relations. It also deals with parents´ educational attitude, their strategies for coping with stressful situations, approach of the closest people and society as such. It gives an outline of psychic and social consequences of being chronically ill. The research part of the thesis includes qualitative survey employing case reports, personal documents analysis, and the method of guided interview. The target survey group comprised of 7 children aged 6 to 16 living in the south of Bohemia who are diagnosed with hereditary metabolic disorder, i.e. phenylketonuria. The target of the thesis was to find out about the way children diagnosed with hereditary metabolic disorder and their parents live, what most affects their lives, whether and how much they are informed about the disorder, and whether then disorder brought only negative things or also some positive ones. The survey questions concern the following: the metabolic disorder influences the life of the child, the disorder influences the whole family, and if the child follows properly given medical instructions, it can have a normal life like its healthy peers. The outcome of the thesis may be used as a basis for further improvements in complex care provided to children with metabolic disorders and their families.

Radial approach to coronary catheterizations and interventions

Radial approach in coronary catheterizations and interventions has been only an alternative of the femoral approach. But due to some important advantages radial approach has a chance to become the first choice in majority of catheterized patients. The most important advantage is the reduction of vascular access site bleeding complications. Additonal benefits are patient satisfaction, cost reduction, shorter hospital stay and possibility of the same-day discharge catheterizations and interventions. The aim of our work was to try to answer three open questions: 1. is it possible to catheterize majority of our patients from the left radial artery when 90% of them are right-handed ? 2. what is the optimal care for the radial artery after the procedure in prevention and treatment of radial artery occlusion? 3. is it effective to implement radial approach in primary PCI as a first approach for STEMI patients? The first part relates the left radial approach. After construction of our special variable support for the left arm and forearm we succesfully used this approach in our studies in almost 90% of patients. Protective sexological treatment and related issues

Summarry The presented thesis deals with Protective sexological treatment and with topic related issues, discusses in detail the anchoring of the Institute of protective treatment in the legislation of the Czech Republic (the substantive and procedural page), which is a better outline of the whole problem with the use and knowledge of forensic disciplines such as forensic psychiatry, forensic sexology and psychology. Protective treatment is an institute of criminal law, one of the types of protective measures, forms irreplaceable part of the czech system of criminal sanctions. Metoda: Tato práce Je rešeršního charakteru. Zvolenou metodou bylo tedy vyhledání dokumentů souvisejících se zadaným tématem, zpracování informací a začlenění získaných dat do kontextu obecně uznávaných poznatků o tématu formou komparativní kritiky. Závěr: Ze získaných informací o běžně uznávaných vědomostech v problematice bolestivého hemiplegického ramene a výstupech plynoucích z výsledků provedených studií vyplývají v mnoha ohledech rozpory a další otázky vhodné k řešení. Polemika plynoucí z rozdílných závěrů naznačuje, že mnohá zavedená dogmata o bolestivém rameni po prodělané CMP by bylo vhodné přehodnotit nebo podrobit dalšímu výzkumu. Nurse {--} patient relation in emergency room sector

"Nurse {--} patient relation in emergency room sector{\crq}q was the topic of my thesis. The bachelor thesis was divided in two parts. The theoretical part dealt with questions of those factors which influence the nurse - patient interrelation. The thesis tried to describe the most important factors which effect the establishing of the patient {--} nurse interrelation; it tried to describe nurses{\crq} points of view as well as patients{\crq} points of view because their views can be fundamentally different and in a way they can zoom in some out-patients{\crq} aspects. The length of waiting time for treatments, overall impressions related to their visits in emergency rooms, patients{\crq} characters and the level of emergency room nurses{\crq} stress play significant roles. Two targets were set in the practical part of my thesis. The first target was to map factors which take share in forming a positive client {--} nurse relation in emergency room care. The second target of my thesis was to map factors which take share in forming a positive nurse {--} client relation in emergency room care. Furthermore, five hypotheses were set. The first hypothesis was: ``Waiting time before treatment influences nurse - client relation.{\crq}q The second hypothesis was: ``Women are more willing to cooperate than men.{\crq}q The third hypothesis was: ``The shorter the client{\crq}s waiting time for treatment, the more willing s/he is to cooperate.{\crq}q The fourth hypothesis was: ``Emergency room nurse{\crq}s high stress influences her relation to clients.{\crq}q The fifth hypothesis was: ``A large number of clients waiting for treatment are a strong stress factor for nurses. The qualitative method with the anonymous questionnaire technique was used for the real status discovery. Two kinds of questionnaires were used. The patients{\crq} questionnaire contained 24 questions. The questions were closed ones and half-open ones - for amending of brief answers. The introductory questions were identifying ones. The other questions were focused on positives and negatives which are closely connected with visits in emergency rooms. Nurses{\crq} questionnaire contained 29 questions, which were closed ones and half-open ones - for amending of brief answers and one question was open one - for expressing of their own opinion. The other questions were focused on positives and negatives which are connected with providing care to patients who come into emergency rooms for treatments. Treated patients, emergency room nurses and ordination nurses in Písek, Strakonice and Brno were my research population. 200 questionnaires were distributed; 100 questionnaires were distributed to emergency room nurses and 100 questionnaires were distributed to patients who were treated in emergency rooms. Findings were illustrated in diagrams, which reflect the answers of nurses and patients. The discussion was confronted with technical bibliography. In the conclusion there are stated the summarised results of the thesis, corroborated or refuted hypotheses as well as possible suggestions how to influence positively the forming of interrelations between nurses and patients in emergency rooms.

Quality of Life in patients with the short bowel syndrome

The goal of bachelor's thesis is to analyze quality of life in patients with the short bowel syndrome. It tries to show significant impairment of life in these patients. In the theoretical section, basic terms with the inclusion of definition of short bowel syndrome are defined. The observation section uses qualitative method (interview) with the patients with the short bowel syndrome. The own perception of problems need not necessarily correlate with laboratory markers of the basic disorder activity. The questions were about affecting life, perception of disorder, treatment and health troubles, change of food and global quality of life evaluation. Subjects were drawn from inpatients and outpatients which are treated in Na Homolce Hospital. The work is intended mainly for nursing staff.

Possibilities and limits of the Pesso Boyden psychomotor therapy in working with traumatized clients

Pesso Boyden psychomotor therapy (PBSP) belongs to a body-oriented psychoterapies that in their thought basis point to the importance of linking physical, emotional and cognitive components of each of us. The subject of the dissertation is: Possibilities and limits of the Pesso Boyden Psychomotor Therapy in working with traumatized clients and is focused on the psychological characteristics of the psychotherapeutic movement, which is processed by the available literature only to a limited extent. PBSP is presented in the context of working with traumatized clients. The theoretical part consists of two basic topics-introduction of practical and theoretical framework of PBSP and lead-in into issues of trauma. The theory of Pesso Boyden Psychomotor Therapy is expressed primarily through general psychological topics and developmental perspective on individual personality, PBSP practice is reflected in the detailed description of the psychotherapeutic process and approached by fictitious case report of PBSP. In the theory of trauma is particularly pointed out its genesis, heterogeneous definition of the term, both positive and negative symptomatology and treatment option for clients with traumatic history. An Analysis of the Counselling-Therapeutic Process in the Substitute Programme Subutexem o.s. Prev-Centrum

This diploma paper focuses on the analysis counselling process in The Subutex® substitution programme Prev-Centrum, NGO, in Prague. On the strength of contemporary knowledge questions are asked about both this process and the whole programme. The theoretical part of this paper overviews contemporary knowledge on drug abusing; focussing on opiods, drug dependence and drug addiction and possibilities of drug users' treatment. It maps the situation in this field in the Czech Republic at the present time. This paper concentrates on both pharmaceutical and psychosocial stages of drug treatment, with special attention on the use of buprenorphine (Subutex®) as substitution. In the practical part of this paper the author analyzes her own factual examples of counselling praxis in The Subutex® substitution programme, with regard for previous theoretical basis. Pointing to key moments of this counselling process she is looking for their reasons and consequences, and considering the practical use of final statements in The Subutex® substitution programme Prev-Centrum, NGO.
