Quality of life in patients in substitution treatment
Kalužíková, Miroslava ; Matoušek, Oldřich (advisor) ; Petrošová, Blanka (referee)
The opiate type of drug addiction represent a social problem in many world countries for a long time. Dependence on drugs is hardly curable but today exist time-proved ways how to reduce its negative social incidences (impact) and incidences on each person, namely first in health and social field. There is a complex system of treatment for opiate addicted people in the Czech republic. One of the links in this system is substitution treatment, which register positive results of general stabilization of heroin users. The substitution treatment promises better standard of living to a patients in substitution than usualy addicted people on opiates. The poin of this thesis is in one way a collection of facts about problems around substitution and also a discription of some basic characteristics of patients using methadon or buprenorphine and summary of their own subjective perception of quality of live (QOL). After an introduction of basic conception connected to the drug-addiction, system of the treatment and particular description of substitution treatment, its points and its faults, comes quantitativ research, which where realized with an standartised version of question-form by WHO. This research describes QOL in a sample of patients from four Prague's substitution programmes, it research impresion from...

Lukáš, Richard ; Karpaš, Karel (advisor) ; Štulík, Jan (referee) ; Chaloupka, Richard (referee) ; Rudinský, Bruno (referee)
1. SUMMARY Introduction: During the period of the last 25 years remarkable changes occurred in the treatment of traumas of thoracolumbar spine. The surgical treatment was extended in cases where instability and impairment of its physiological shape are expected. This development is based on the progress in imaging technologies, more detailed understanding of spine biomechanics and a pathophysiology of its impairment. The situation was also influenced by the progression in surgical instruments and implants and also in the technology of bone substitutes. Aim of the study: The aim of the study is to analyze the long term morphological and clinical results after the surgical treatment of thoracolumbar fractures, in which the bioactive glass-ceramics in the form of granules was used as a bone substitute. Material and method: A series of 140 patients treated surgically for unstable thoracolumbar fracture (LSC points 4 - 6) during the period from 1997 to 2003 was studied. The average age of patients was 44 years (14 - 78). The patient collection consists of 83 males in the average age of 43,7 years (14-73) and 57 females in the average age of 44.9 years (15-78). The minimum follow-up was 24 months, the average follow-up period was 29 months. Patient series includes a group of 91 cases, in which the implant was...

Default of time in business transactions and its consequences
Hrušková, Jana ; Liška, Petr (referee) ; Patěk, Daniel (advisor)
in English Default of time in business transactions and its consequences The topic of a default of time in business transacions becomes very current in our society, which is one of the reasons I have chosen this topic for my diploma thesis. The objective of my work is to define the conditions due to which the debtor or creditor get in the default of ime and explain the consequences of the default of time of one of the contractual parties. In the thesis I shall focus primarily on the definition of the period for performance of the debtor's obligation, because definition of this matter in the Commercial Code is currently quite difficult to interpret and I shall bring attention to certain problems of the legislation related to the treatment of the consequences of the default of time by any of the parties. I would like to pay more attention to the default of time payment interests, as these are the most typical result of the debtor's default of time. My diploma thesis is divided into nine chapters. The first chapter is the introduction defining the objective of my thesis. The second chapter is called the "Default of time in Business Relationships" and specifies the term "default of time". The default of time become when one of the parties, a debtor or a creditor, is in a default of time with performance...

Diagnosing vascular prosthesis infection by means of the FDG-PET/CT
Špaček, Miroslav ; Bělohlávek, Otakar (advisor) ; Vojáček, Jan (referee) ; Gregor, Zdeněk (referee)
Introduction: Vascular prosthesis infection (VPI) is a life-threatening complication in 0.5- 5% of cases. Early and reliable diagnosis is a necessity for adequate treatment. Computed tomography (CT) is the gold standard diagnostic method used world-wide with excellent results, but in cases of advanced graft infection. Low grade infections in non-acute patients are a diagnostic challenge requiring a new method with good diagnostic accuracy. The aim of this work is to define the accuracy of 18 F-FDG PET/CT in these settings and to identify essential parameters of its evaluation. Material and Methods: Seventy-six consecutive patients with total of 96 vascular prosthetic grafts in which infection was suspected underwent PET/CT examination prospectively. PET/CT scans were analysed in terms of presence and intensity of focal and diffuse FDG uptake, presence of anastomotic pseudoaneurysm, irregular boundary of infiltration, combination of these, and uptake ratio between graft and blood background. Also, other parameters like white blood cell count, C-reactive protein, and microbiological cultures were tested. The final diagnosis (gold standard) was based on operative/histopathological finding or >6 months clinical follow up. Results: In our series the prevalence of VPI was 57.3 % (55/96 prostheses). Among various...

Laboratory diagnostics of micrometastases in breast cancer patients
Mikulová, Veronika ; Zima, Tomáš (advisor) ; Svoboda, Marek (referee) ; Průša, Richard (referee)
Introduction: The presence of circulating tumor cells (CTC) in the peripheral blood has been associated with worse prognosis and early relapse in breast cancer patients. CTC determination in the peripheral blood has been considered as a liquid biopsy. The aim of this project was to analyze the presence of CTC followed by their molecular characterization with the potential use not only as a new biomarker for real-time monitoring of therapy efficacy but also as a suitable tool for patient's stratification and individualization of treatment for breast cancer. Methods: A total of 54 patients with diagnosed early breast cancer were enrolled into a prospective study. Ten millilitres of peripheral blood were sequentially collected to test for the presence and characterization of CTC during the follow-up of patients. CTC isolation and detection was performed by AdnaTest BreastCancer™ (AdnaGen AG, Germany), which is based on the detection of EpCAM, HER2 and MUC1 specific transcripts in enriched CTC- lysates. cDNA from isolated CTC has been further used for newly optimized qPCR assays for breast tumor and therapy resistance associated genes: TOP1, TOP2A, CSTD, ST6GAL, KRT19 and reference gene actin. qPCR results have been analyzed by Genex software (MultiD Analysis). Results: 195 blood samples have been...
