Patient's status and rights during emergency medical services

It is necessary to consider each patient to be a unique entity and respect his or her rights. This position requires an individual approach of paramedics. They should give the client a sense of security, certainty of assistance and take into account the fact that the patient is exposed to stress, which can change his behaviour. The paramedic must cope with these facts, he cannot be influenced, he must know how to prevent conflicts and behave according to the rules of medical ethics. The aims of this thesis was to map out the level of knowledge of patients in the area of their rights, and to find out their views on the attitude and behaviour of the paramedics during an intervention of a medical rescue service. The third part of this bachelor thesis deals with the knowledge of paramedics in the area of the patients' rights and their observance of these rights. Three research questions were set up. Whether the legal awareness among patients regarding the intervention of medical rescue services is sufficient and how patients evaluate the attitude and behaviour of paramedics during the intervention. A research question was also set up for the third aim of this thesis: what is the point of view of paramedics on respecting the patients' rights during pre-hospital emergency care. In the theoretical part I focused on explaining of the basic concepts of the medical ethics issues, its history and development. The most important part of the thesis is the ethic of emergency medicine and the issues of the rights of patients during prehospital emergency care. The thesis also discusses the ethical dilemmas that paramedics deals with during their responses and an equally important area is focused on different patients' rights, including legal standards that modify them. The research part consists of interviews with two groups of respondents: eight paramedics of the Ambulance Service of Pardubice Region and eight patients who used their services. The research showed that the legal awareness of patients is inadequate. At first glance, all patients were satisfied with the intervention of the medical rescue service and had no objections. After targeted questions targeted on the compliance of individual patient's rights, I concluded that in some cases the right to privacy and dignity of the patient was not respected. The patients were not sufficiently informed about their health and procedures that paramedics performed. None of the interviewed paramedics complied the patient's right to know the name of the medical staff member who treated him. The paramedics themselves commented on the issue of the rights of patients during the pre-hospital emergency care in the sense that they try to comply these rights, but they had problems with is not always possible. They are aware of the ethical code of the rights of patients, but they had problems with telling its content. The most frequent reason for non-compliance of the rights of patients was reported in connection with responses to people under the influence of alcohol, drugs or to patients with disorders of consciousness. The surprising result of the research was the finding that half of the responding paramedics have used violence against a patient during their practice. Among the groups of patients, to whom the responding paramedics do not like to drive, are mostly Romani people, homeless people and repeatedly calling patients. This bachelor thesis will contribute to a better awareness of paramedics and the general public about the respecting the rights of patients. Paramedics can use the observations of patients and by that remove the deficiencies they have been committing during their responses.

Observance of elderly patients rights in long-term facilities.

The topic of this diploma paper is ``The Observance of the Rights of Senior Citizens at Long-Term Care Wards{\crqq}. The growing number of senior citizens brings the increasing number of hospitalized people at wards of long-term (subsequent) care. The paper focuses on the observance of seniors{\crq} rights by the attending staff at these wards. It is divided into two sections {--} a theoretical one and a practical one. The theoretical section describes patients{\crq} rights in relation to ethics. It develops the Ethics Codex of Patients{\crq} Rights, the European Charter of Senior Patients. It also discusses patients{\crq} rights in relation to the legislature. It deals with individual laws and their observance by the attending staff that is involved in the care of senior citizens at long-term care wards. It describes the duties of the staff based on individual laws. The practical section sets the objectives focusing on finding out about the observance of senior patients{\crq} rights at long-term care wards as seen by general nurses, senior citizens, and family members. It also attempts to find out about the most frequent forms of non-observance of seniors{\crq} rights. To achieve these objectives, a quantitative research survey was used. The techniques of data collection from the senior citizen respondent group included a structured interview, and a questionnaire for the groups consisting of nurses and family members. The survey resulted in the finding that, according to the respondents, (senior) patients{\crq} rights are always, or almost always, observed in most domains. In spite of this, the respondents perceive some problems in the domain of respecting patients{\crq} shyness and attending staff{\crq}s anonymity. Only some members of staff introduce themselves to patients upon their first meeting, and seniors know the names of only a few attending team members. In the course of hospitalization, a senior patient is not only a recipient of care but also a partner to the attending staff. To provide quality nursing care, which is highly emphasized these days, the knowledge of (senior) patients{\crq} rights by attending staff is essential. Only a senior citizen who knows their rights as well as an attending person is able to protect fully the hospitalized seniors{\crq} rights. The results of this diploma paper can be used in seminars, conferences for medical staff to deepen the knowledge of (senior) patients{\crq} rights. Similarly, senior citizens themselves will be made more aware of the rights they have in relation to receiving medical care.

The inclusion of the rhythmical forms in physical education at primary school in České Budějovice

The aim of the graduation thesis was finding out the inclusion of dances and rhythmical forms in physical training lessons at primary school level. The research field was pupil´s relation to doing exercise accompanied by music. The resource file was 158 pupils of the second class of primary schools in České Budějovice. The research method was a questionnaire. The results file confirmed the presumption that the inclusion of rhythmical forms in physical education is not frequent, although the rhythmical forms are included in curriculum. The results were different in each school and even in each class. The research proved that only a half of pupils is a member of some physical, sport or dance interst group. This graduation thesis shortly reffered to the use of music accompaniment in education.

Emil Filla and China

It is well known about Emil Filla (1882-1953) that he was passionate about collecting non-european art. During his life he tried to find some kind of universal language which would allow him to connect the artistic styles of different cultures from all around the world. Filla sacrified a huge part of his life to studying chinese art, which became popular in Europe in the beginning of 20th century. Filla did not only frequent Asian art exhibitions, he himself owned many artistic objects and Chinese ink paintings. Thanks to studying books and paintings he became familiar with philosphy and media typical for Chinese landscape painting. The most obvious evidence of Chinese influence is the thin and long form of painting, use of ink and the illusion of timeless atmosphere, eternity and infinity. The result of this are wider landscapes which differ greatly to the rest of traditional Western landscape painting.The landscapes from České středohoří were Filla's last works. He had them painted at the very end of his life around the 1950s, when art was under the control of government authorities. These landscapes from České středohoří are unique and they are definitely not the perfect example of social-realistic painting which was popular by this time. Filla's aim was more about spiritual value for the artist...

The Possibilities to Influence the Response of Innate Immune Cells to Gliadin

Gluten sensitive entheropathy - celiac disease is a lifelong, genetically predisposed, immunologically mediated susceptibility to dietary wheat gluten, most frequently demonstrated by small-bowel damage and malabsorption syndrome. Strict adherence to gluten-free diet is the sole rational therapy of the disease. One of the possible therapeutic strategy for the treating of celiac disease is to utilize the synthetic polymer P(HEMA-co-SS). This polymer is capable specifically bound gliadin in gastrointestinal tract and by this way to neutralize the damaging effect of this alimentary protein on mucosa of small intestine in celiac patients. The in vitro study on human PBMC and specimens of small intestinal biopsies of celiac patients in our laboratory demonstrated that putative therapeutic ability of P(HEMA-co-SS) is substantially influenced by degree of proteolytic processing of gliadin and P(HEMA- co-SS) and also by different timing of per os administration of both components in organism. Another putative adjuvant therapy of celiac disease is employing of the beneficial probiotic bacterial strains. Proposal Possibilities of Touristic Deconcentration - A Case Study of District Český Krumlov.

4 ABSTRACT The aim of this thesis is to draw attention to the process of concentration in tourism.Various locations within the monitored area have different potential for tourism. This aspect leads to a concentration of tourists and services to certain centers. In the area of interest there are two attractive location for a tourist city of Český Krumlov and Lipno dam. These two sites are the frequently visited places, both in southern Bohemia and in the whole Czech Republic. However, the area has a hide potential for the development of alternative forms od tourism. The intent of this thesis is based on the available data and information to evaluate the process of concentration of tourism in the case of Český Krumlov district and propose appropriate tools to deconcentration of tourism and the use of peripheral sites. Experience the czech population with age discrimination

Abstract. At present ageism is a commencing problem which is necessary to deal with. Originally an English term has no Czech equivalent and it denotes discrimination or differentiation on the basis of age implying prejudices towards elderly people. It concerns statements and attitudes without true and reasonable basis. Some manifestations of discrimination are evident, some remain not pronounced in vertones. Ageism appears most often in the sphere of employment, where people are rejected for the reason of alleged decreasing performance and frequent sickness, according to prejudices connected with their more advanced age. The practical part of my thesis aims at a comparison of people over 50 and those who are still under this imaginary limit. By collecting primary data the following hypothesis was tested among others: it is more difficult for people over 50 to find work than for younger age categories. Respondents were chosen among job applicants of the Job Centre in Tábor. On the basis of a questionnaire and a secondary analysis of the data it was proved that 50+ applicants at the Job Centre in Tábor had to face age discrimination at an interview more often and also have more frequent experience with being dismissed due to their advanced age, which confirms the determined hypothesis. It is interesting that as people grow old they tend to think that after being 50 their work performance will decrease. Moreover, the research implies in general that people over 50 are a worse employable group than those of lower age categories. A feasible solution of this situation might be adjusting of stricter recourses for this kind of discrimination. In the Czech Republic this problem has been treated directly only under the rule of employment law which bans this kind of discrimination. A possible contribution might be an anti-discrimination law now being prepared. Promoting the effort to change social awareness concerning the qualities and abilities of older generation is also essential. Media should present a more positive image of elderly people; after all decreasing strength is compensated by great life experience, therefore this combination may not influence their general ability and performance.

Potenciometrické stanovení gentamycinu ve farmaceutických přípravcích

Gentamicin is an aminoglycoside antibiotic, and can treat many types of bacterial infections, particularly Gram-negative infection. It is one of the heat-stable antibiotics that remain active after autoclaving, which make it particularly useful in the preparation in the certain microbiological growth media. Traditionally, pharmaceutical analysis of these compounds relies heavily on chromatography, yet also frequently requiring the use of reagent-based detection principles like spectrophotometric, chemiluminescent detection in flow-injection analysis , capillary electrophoresis, a novel LC/ELSD, thin-layer chromatography, HPLC.A simple and sensitive potentiometric procedure for the determination of gentamicin is presented, after development of selective electrodes based on PVC membranes using different type of cyclodextrins and its derivatives, or Evans Blue complex with gentamicin as ionophers. Several mediator solvents and lipophilic ionic species were also used in order to optimize the characteristics of the membranes. The evaluation of the general characteristics for all the units prepared revealed an analytical linear range of about 2x10-4; 5x10-3 mol L-1, slopes of about 50 mV/dec, good reproducibility and low response times. Comparison of the psychosocial intervention within the basic sectors of the Integrated Rescue System

Theoretical part First chapter of my thesis on Comparison of Psychosocial Crisis Intervention among Basic Parts of the Integrated Rescue System deals with general topics like stress and its symptoms. The next chapter follows individual parts of the integrated rescue system and stress situations resulting from their jobs. Work in IRS varies a lot but the impact on people employed in IRS is pretty similar. Third chapter deals with crisis management. Following is chapter number four presenting signs, forms and steps of crisis intervention. The most important chapter of the theoretical section of the thesis is the fifth chapter called Options of crisis intervention for IRS parts. The options of psychological support for firemen, policemen and paramedics are described here in detail. Research part I intended to find out to what extent is a job with IRS burdensome for its members, whether they know about the options of psychological support and how they make use of it employing a survey consisting of 26 questions. 300 people were addressed with the return rate amounting to 87,67%. Basic descriptive statistics were produced. Hypothesis no. 1 and 2 were tested by Chi-square test on the level of significance of 0,05. Hypothesis no. 3 was tested by Fischer exact test on the level of significance of 0,05. Objectives of the thesis were following: to explain terms related to the topic of psychosocial crisis intervention to assess the means of support each part of IRS offers its members to find out whether the members of IRS know about these means of support and use them to compare these support services provided by respective parts of IRS and come up with suggestions for eventual improvement Results The survey contains all three parts of IRS proportionately. The most respondents work at ZZS (paramedics, 36,5%), the least at the Police force (30,8%). Regarding the hypotheses and objectives of my thesis the outcomes are following. 68,8% of the respondents agreed that their job is mentally very demanding. Level of psychological stress is the same for all parts of IRS. This corroborates the first hypothesis. 87,8% of respondents claim to know about the option of psychologist or peer consultations. Yet only 27,8% of respondents admit to make use of this option, with firemen doing so most frequently. The second hypothesis was corroborated only to a certain extent for only 16 members of Police claimed to make use of psychological support. Of all the respondents in the survey 158 people admit to have never used this kind of support. Hypothesis no. 3 was thus corroborated. Conclusion All the members of IRS regard their work as psychologically very stressful. The research part proved that level of information about means of crisis intervention is high among the members of IRS. Mainly firemen consider the psychological support to be necessary, a fact which could be expected relating to the nature of their job. Policemen are those who use the least of crisis intervention support (and provided the smallest number of respondents for the survey as well). Crisis intervention is most popular with the firemen. Nevertheless it is necessary to continue raising consciousness about crisis intervention and its practical delivery.

Internet communication is becoming increasingly frequent communication tool, compared to the normal communication (like phone calls, sending letters, etc.). Work described the development of Internet communication, focusing also on its forms (formal and informal). Based on these forms of work it also demonstrated how to create a username or avatar, that applies to the traffic. The thesis focused on the social networking site Facebook, which is becoming increasingly popular. Pointed to the history, usage and also describes the basic functions. Facebook is very good commercial tool, so the work is focused on displaying the possibility of commercialization of this social network. The thesis pointed to other social networks, which may not be as popular, but worth a demonstration. Generally it's not just a social network that could be used for Internet communication. On the contrary, this form is younger than communicators, which are also described and introduced. It focused on the best-known communicators such as ICQ, Skype, etc. The work is a survey and analysis, which focused on the global Internet communications. The thesis is based on research. Firstly, there is a focus on the security account on Facebook, as well as the use of online communications companies and finally to use Internet communications of the faculty.
