Cognitive Impairment in Parkinson's Disease

Cognitive impairment is considered as essential feature of non-motor symptoms in Parkinson's disease (PD). It is a result of underlying pathological processes in the brain of PD patients and it leads to decreased quality of life. In this thesis an analysis of the structure and profile of cognitive impairment is presented with special emphasis on executive functions and memory. We take diagnostic entities developed for the description of PD cognitive spectrum such as mild cognitive impairment (PD-MCI) and dementia (PD-D) as examples of heterogeneity and different severity of cognitive impairment in PD. However, neuropsychological methods in Czech version that would measure these diagnotic units were not adequatly validated. In the experimental part we test a hypothesis, if gait disorder with falls in PD is interconnected with cognitive impairment, and if PD-fallers have more severe cognitive deficit than PD-non-fallers. On the basis of nine validity or normative data studies we show psychometric properties and clinical utility of several basic neuropsychological methods in the Czech population for memory (Rey Auditory Verbal Learning Test, California Verbal Learning Test, Second Edition, Memory For Intentions Screening Test and Enhanced Cued Recall Test), sustained attention and executive functions...
Chosen Analogues Derived from Substance 7-MEOTA Action on Some Aspects of Cholinergic System

This thesis deals with effects of some chosen 7-methoxitacrine (7-MEOTA) analogues on enzymatic activity of acetylcholinesterase (AChE). 7-MEOTA is a derivative of tacrine, which had been used for symptomatic treatment of Alzheimer's disease (AD), until drugs with better therapeutic index were developed. 7-MEOTA the same way as tacrine therapeutically acts by inhibition of acetylcholinesterase and a neurotransmitter acetylcholine rise in the organism. It shows similar strength and type of inhibition, but it's less toxic contrary to tacrine. Some of the previously examined analogues of 7-MEOTA were as strong or even stronger AChE inhibitors than 7-MEOTA and so promising future medicaments. However, all the compounds analyzed in this thesis showed weaker enzymatic reaction inhibition and AChE affinity. For each of the examined compounds IC50, Ki and Ki' were calculated and AChE inhibition type was determined. All the 7-MEOTA analogues showed a mixed type of the inhibition. The theoretical part of this thesis deals with manifestations and origins of AD, its genetic factors etc. and tries to show some of the anthropological findings a theories connected with the theme.
Activating the elderly with dementia in residential homes

In the theoretical section we have focused on the process of ageing and made a geographical and statistical assessment. The base was our country's legislative framework for providing care to the elderly with dementia, the issue of the illness of dementia, its causes, as well as the approach and care provided to people with dementia. We drew from the terminological definitions and theoretical resources for employing elderly people with dementia. We have made a note of the philosophical and psychological source. We lay emphasis on information concerning the possibilities of employing elderly people with dementia and their activation. We have followed theoretical resources which, in employing these people, are especially constructive for the users, providers and above all for the family. In all the items observed we respected the limit given by the diagnose of dementia. In the practical section, we have paid attention to already existing activities employed in residential homes for the elderly with dementia. We give a list of some types of homes where people with dementia are placed and where a survey was made. Psychosocial context of care for person with dementia in the family

My diploma thesis is focused on problematic of care giving for persons with dementia in their home environment. This thesis could also provide useful advice for caregivers, show communication options with patients affected by dementia, and suggest how to avoid uncomfortable situations. Additionally the work is aiming to list the options of maintaining maximal comfort and well-being of persons with dementia. The comfort of caring person is also important, and is addressed by this work. Gathered knowledge leads to better understanding of way of life of the caregiver as well as person with dementia. The conclusion of theoretical part is mapping the topics such as burn-out syndrome or activization and offers the options where to find useful advice. Empirical part of this thesis summarizes with whom the person with dementia shares the household and whether caregivers notice the changes in behavior of person with dementia.

Thesis deals with ill person`s needs and needs of nurturing person. Nursing care for patients with Alzheimer's disease

The thesis deals on nursing care of patients with Alzheimer's disease. The work consists of two parts. The theoretical part is focused on Alzheimer's disease, diagnostic methods, symptoms, therapy, care for the patient and patient's family members and organizations that help patients and relatives, such as the Czech Alzheimer Society. Alzheimer's disabilities

This diploma thesis, "Alzheimer's disabilities ", deals with questions around people with Alzheimer's disabilities, which is the most common type of dementia. Alzheimer's disabilities is also very widespread in the population; and it is the 4th or 5th most common cause of death. The thesis is divided into two parts, the first part is theoretical and the second is empirical. In the theoretical part the thesis deals with the general characteristics of this disorder such as: causes, prevention, clinical manifestations, consequences, diagnostics and pharmacological approaches. The main focus of the theoretical part is in the second chapter, which deals with non-pharmacological approaches to people with Alzheimer's disabilities. In the empirical part, case studies on sufferers of Alzheimer's disabilities will be included. The empirical part focuses on substantive changes which occur in the lives of people with Alzheimer's disabilities, and whether these changes can be confirmed as being consequential of the affliction.
