Mechanisms of comorbidity of metabolic and neurodegenerative diseases
Tauchman, Martin ; Telenský, Petr (advisor) ; Brožka, Hana (referee)
In Czechia, number of people with neurodegenerative diseases is in the hundreds of thousands, and the lifetime health care costs and social impact of each patient's disease reach hundreds of thousands of euros, but these costs could be reduced by early and effective intervention. Its correct implementation could be helped by knowledge of causal links between neurodegenerative and metabolic diseases, whose prevalence is correlated in the population. One of the important factors is an increased pro-inflammatory immune response. In people with obesity and type 2 diabetes mellitus, systemic inflammation evolves into neuroinflammation, which subsequently leads to neurodegeneration. Another mechanism is hyperglycaemia, which is a consequence of insulin resistance. Hyperinsulinaemia and hyperglycaemia lead to impaired expression of glucose transporters and insulin-degrading enzyme, resulting in reduced clearance of amyloid beta. Genetic background is also recognized as a highly influential factor, affecting various mechanisms in both beneficial and harmful ways. Lifestyle is also an important factor. In general, smoking and alcohol consumption are harmful to health. Both increased consumption of alcoholic beverages and smoking tobacco products can lead to metabolic disorders as well as neurodegeneration. On the...

Osúchová, Veronika ; Hasmanová Marhánková, Jaroslava (advisor) ; Numerato, Dino (referee)
This diploma thesis is focused on professional carers, who are trained experts employed in specialised institutions that take care about more than one person with dementia in one group. The professional carers are under pressure during this time and there is a certain tension between reaching the maximal possible high quality care and growing number of clients in institutions specialised in dementia and dementia discourse, in which framework especially negative connotations are highlighted. It is just an approach of the carers that plays an important role in this issue. The work with dementia discourse and dealing with the above mentioned issue from the side of carers has a considerable impact on functioning, health condition and quality of life of individuals with dementia. Their dealing with the issue can influence strongly and steer the running an institution and the quality of life of such an institution clients. A difficult situation of professional carers also influences the carers themselves, their concepts of this illness and concepts of their own ageing. This diploma thesis is aimed at an analysis how the pictures of dementia are constructed by professional carers who take care about clients with various forms of dementia in the long term. In the framework of this thesis, their subjective...

The issue of evaluating and alleviating pain in the elderly in nursing homes
PANOVA, Vladislava
The bachelor's thesis "The issue of evaluating and alleviating pain in the elderly in nursing homes" deals with research into the nursing care of elderly people with pain who are permanently housed in social facilities, so-called nursing homes. The research was conducted in selected homes for the nursing homes in the South Bohemian Region. The thesis consists of two parts: theoretical and empirical. The theoretical part is divided into 3 individual chapters. The first is about gerontology and geriatrics, it describes different perspectives on aging, aspects of old age, society's view on the elderly, etc. The second clarifies the concept of pain and related areas: anatomy and physiology, collection of patient history, therapy, etc. The third tells about social facilities and legislation. The empirical part was carried out by a qualitative research investigation, using a semistructured interview method. Data analysis took place in the form of open coding, using the "pencil and paper" method. The codes created further were classified into individual categories and subcategories. According to the obtained data, we can say that nurses are well informed about the methods of pain assessment of their clients based on standardized methods and personal experience while working with them. They know well the pharmacological and nonpharmacological methods of pain relief and they are excellent at cooperating with the members of the multidisciplinary team who oversee implementing some of the given methods. In addition to the good aspects, the lack of a certain standard for the assessment of pain in seniors suffering from dementia syndrome is discovered, which needs to be created in the future not only for social, but also for medical facilities. The results of the bachelor's thesis can be used for further research in the field of nurses' work in social facilities and nursing care for elderly people with pain and inability to express themselves verbally.
