Use of the Modified Frenchay scale in stroke patients with spastic paresis

The objective of this thesis is to describe the Modified Frenchay scale and its use in stroke patients with spastic paresis. The thesis is divided into two parts - theoretical and practical. In the theoretical part, the issue of spastic paresis in stroke patients is specified; and in addition to this the methods of examination and assessment of spasticity are presented and described in detail. A large part of the thesis deals with the history of Frenchay arm test and its modification - Modified Frenchay scale. The theoretical part also contains a description of the phases and types of grasp, some of which can be observed during the assessment with Modified Frenchay scale. The practical part consists of a questionnaire mapping the situation in the institutions in the Czech republic, that use the Modified Frenchay scale in their practice, and of two case reports of patients after stroke. Examinations and evaluations of spasticity, including Modified Frenchay scale, which are described in the theoretical part, are applied in these case reports. The practical part also contains an evaluation of inter-rater reliability of the Modified Frenchay scale. Evaluation of usage effectivity of myoprosthesis in individuals with inborn or aquired amputation of upper extremity

Thesis title: Evaluation of effectiveness of using myoelectric prosthesis by people with congenital or acquired amputation of the upper limb Aims: The aim of my thesis is to evaluate the effectiveness of myoelectric prosthesis using by people with upper limb amputation. Find out how effectively users are able to manage individual tasks with myoprosthesis and basic ADL activities. Methodology: The research group consisted of 6 people with upper limb amputation. In this thesis, there were used three methods of data collection. The first method that was applied was a survey, which was used to obtain anamnestic data by the research group. The second method of data collection was the observation of probands during the performance of individual tasks. The third chosen method was measurement by using a special test aimed at evaluation of the effectiveness while using and the involvement of myorpothesis in the locomotor chain against a healthy upper limb. Results: Based on observations and measurements, it was performed that all probands are able to functionally and effectively involve the myoprosthesis in the locomotor chain in the performance of individual tasks. Fine motor skills and laterality of preschool children with cerebral palsy

The diploma thesis is focused on fine motor skills and laterality of preschool children with cerebral palsy. Based on previous researches was found out, that cerebral palsy is influencing fine motor skills. With reference to these researches, the aim of this thesis is to find out, if there is difference between intact prechool children and preschool children with cerebral pals in fine motor skills and what particular differences there is. The thesis include laterality and question of pronounction in preschool age. The theoretical part describes evolution of children, defines fine motor skills and laterality, deals with cerebral palsy and describes fine motor skills of children with cerebral palsy including previous researches. The empirical part defines the aims of research, explorational questions, methods of research and testing activities. Next part is focused on selection of respondents and their characterics. Grasp evaluation in patients after stroke

Title of diploma thesis: Grasps evaluation in patients after a stroke Objective: The main goal of the pilot study was to determine minimal clinically important difference in selected tests focusing on improvement in the grasps of patients in the chronic phase post stroke. The main hypothesis was set to prove that the differences between the initial and final assessments based on the Modified Frenchay Scale characterizes patients' improvement more accurately than the Score for visual evaluation of functional task of the hand. Methods: The research group consisted of 40 participants (26 men and 14 women) all in the post stroke chronic phase who were selected based on the criteria for inclusion in the research. Data was collected at the Department of Rehabilitation Medicine and the First Faculty of Medicine. Using video documentation of the Modified Frenchay Scale used during both the initial and final assessment, the test itself was then analysed. A 3rd subtest was also used to evaluate the Score for visual evaluation of functional task of the hand. Using the Spearman correlation coefficient, the statistically significant level was established (p <0,05) followed by calculating the critical value of the Spearman correlation coefficient (rsp (0,05,40) = 0,264), and thus verifying the hypothesis. The Bachelor thesis is focused on description of individual points of the 0-10 scale of the modified version of the Frenchay arm test in patients after cerebrovascular stroke (CVA). The thesis is divided into two parts - theoretical and practical. The theoretical part describes the issue of CVA in detail. It includes statistics, types of strokes and clinical symptoms. Further, in the theoretical part, tests for upper limb examination are mentioned. The Modified Frenchay Scale and the Frenchay Arm Test are described in more detail, two other tests are mentioned only marginally. The theoretical part also mentions the types and phases of grip that we can observe in the Modified Frenchay Scale and the list of common daily activities. The practical part of the bachelor thesis contains three case reports of patients after CVA. Patients in the bachelor thesis represent the acute, subacute and chronic phase of CVA. The case reports include descriptions of each task of the Modified Frenchay Scale and their scoring on the scale 0-10. Attachments include a manual with suggested descriptions for the individual points of the 0-10 scale of the modified version of the Frenchay arm test.

Name: Case study of physiotherapeutic care of a patient with a gunshot injury of the left hand. Aims: The main aim of the thesis is description of the theoretical knowledge about anatomy, kinesiology and function of the right hand. The thesis also deals with characteristics of gunshot injuries, methods of operational solutions by Kirchner wires and case study of a gunshot injury of the left hand. Methods: The bachelors' thesis is composed of the theoretical and special part. The theoretical part deals with anatomy, kinesiology and function of a hand and also describes fine motor skills. The characteristics of the gunshot injuries and Kirchner wires is also covered in the theoretical part. The practical part deals with a case study of the patient, initial kinesiological analysis, therapies, output kinesiological analysis and evaluation of the effect of the therapy. Title: Self-sufficiency and self-care when applying myoprothesis Objectives: The main objective of this work is to present the theoretical summary of the amputation problematic, followed by the further prosthetic usage of myoprothesis and the principles of controlling the myoprothesis. Key is the methodical process of learning how to use myoprothesis, in terms of own self- sufficiency and self-care in the daily activities. Methods: I used the method of literary research in this work. I drew upon the knowledge gained throughout the study of available Czech and foreign technical literature, from internet sources and materials obtained in prosthetic companies, which I visited during my practical internships, mainly through the personal interviews and consultations with prosthetists and ergotherapists. Occupational therapist's methodics of work for patients with myoelectric prosthesis

The aim of this study was to develop methodics of occupational therapy for patients with moelectric prosthesis, which is used by patients with amputation of upper limbs as well as patients with congenital problems. In the Czech republic there are no complex methodics about occupational therapy and myoelectric prosthesis, despite the huge mount of foreign studies of this topic. Myoelectric prosthesis uses the principle of sensing myoelectrical signals which are then transformed to the motor output in the terminal portion of the prosthesis. Occupational therapy intervention begins at the preprosthetic phase, the occupational therapist assesses the general potential for the use of myoelectric prosthesis and picks up myoelectric signals from the stump. In interprosthetic phase occupational therapist is dedicated to the training of activities of daily living with myoelectric prosthesis and training residual stump on the basis of myotest. The phase after prosthetic content of occupational therapy evaluation and assessment, which focus on patients ability of myoelectric control and prosthesis involvement in activities of daily living and training of grip. Methods of a grip evaluation after reconstructive surgery of the spastic hand

The aim of the thesis was to find out which areas of life of people with hand spasticity may affect the performance of reconstructive surgery. Furthermore, to determine which specific factors involved in the function and dysfunction of the upper limb in people with upper motor neuron syndrome can affect the results of the intervention, and what are the relationships between these factors. The group consisted of 13 people with cerebral type of spasticity. Subjects were examined preoperatively, three months after the intervention, and five months after the intervention. Operations were performed in the wrist-hand and elbow region. Subjects were examined repeatedly by some sensory modality, goniometry, Frenchay arm test, Score for visual evaluation of functional task of the hand, The QuickDASH, modified Ashworth Scale, modified Barthel Index and each person was classified according to The Manual Ability Classification System. There was an elbow contracture release and an improvement in an active motion in the wrist as result of the intervention. The intervention of occupational therapy in patients with impaired hand function after surgery

This bachelor's thesis deals with the surgical treatment of patients with hand impaired function and subsequent intervention of occupational therapy, and the patients who underwent this treatment. Thesis is divided into two parts-theoretical and practical. The theoretical part describes the function of the hand and the possibilities for its disorders. Subsequently, thesis describes the principles of surgical treatment of various diseases and hand injuries that impair its function. Another part relates to intervention of occupational therapist in patients who underwent surgical treatment of hand. The practical part contains two casuistries, methodology and processed results and discussion. The first casuistry deals with patient with carpal tunnel syndrome solved by surgery. The second casuistry deals with a patient with finger's flexor injury on the left upper limp. During processing has been found that occupational therapy is integral to rehabilitation after hand surgery.
