Paint treatment after gynecological performance

The theme of this thesis is the treatment of postoperative pain after gynecological surgery. The work consists of theoretical and empirical part. The theoretical part describes the physiology of pain, its classification, types and factors affecting its intensity. In subsequent chapters, the theoretical part describes the diagnosis, treatment, intervention of a nurse in a patient with pain and patients' rights to adequate pain treatment. In the empirical part are processed data from the survey. Quantitative survey method is determined, patient satisfaction with pain in the care of nurses. Quantitative survey was distributed to the respondents Gynecology - Obstetrics, 1st Medical Faculty, Charles University in Prague. The aim of the research was to determine patient satisfaction with pain in the care of nurses, chart type preference analgesia after gynecological surgery, and positive and negative effects on pain perception, determine the impact of postoperative pain on sleep and self-care. The inquiry also diagnosis of pain intensity. From the data obtained, showing that 100% of the patients in the sample were satisfied with the care of nurses. Title: Case study of physiotherapy treatment of the patient with the diagnosis of ankylosing spondylitis. Objective: The main target of this bachelor's thesis was to take up with the issue of the ankylosing spondylitis, as the rheumatic disease and to outline the possibilities of its treatment. The next target was the elaboration of the case study of the patient with this diagnose, followed by the appraisal of the therapy's effect. Methods: In the general part is concerned with the Ankylosing spondylitis issue, namely with its symptoms, manifestations, methods of examination and a subsequent treatment of this disease. The second, special section of this bachelor thesis is focused on the individual patient with ankylosing spondylitis, physiotheapy assesment, subsequent therapy and the final effect of it. Results: The results of the physiotherapeutic care of the patient were observed to be positive in objective (clinical) and subjective way. The painful situations became milder and the patient assumed a positive approach to the kinesitherapy, which she didn't have till now. Title: Case study of physiotherapy treatment of a patient with the diagnosis of ankylosing spondylitis The target: The main target of this bachelor's thesis was to highlight the problems of rheumatic disease ankylosing spondylitis. The emphasis is placed on the role of the physiotherapy practise as a part of treatment. Additionally there are also written possibilities of pharmacological treatment. Methods: The bachelor's thesis is divided into two parts. In the general part I deal with the symptoms of the disease on the basis is subsequently diagnosed the ankylosing spondylitis. Additionally there are also analyzed the typical symptoms of the disease, methods of examination and treatment. The special section is focused on the specific and individual patient with ankylosing spodylitis, subsequent therapy and the effect of therapy. This case study was created during the passed practice in the Institute of Rheumatology in Prague the period 1st-6th January 2014. In the empirical part two case reports of patients with vertebrogenic difficulties are presented. The Klapp therapy was used for climbing in a short and long physiotherapeutic plan. The work objectives were the use of the Klapp method for vertebral diseases in its contemporary concept.
The role of TRPV1 receptors in chemokine CCL2 induced modulation of nociceptive synaptic transmission at spinal cord level

Modulation of nociceptive synaptic transmission in the spinal cord dorsal horn is a significant mechanism in the development and maintenance of different pathological pain states. Accumulating evidence indicates that the TRPV1 (transient receptor potential vanilloid 1) receptor and chemokine CCL2 (C-C motif ligand 2) may play a critical role in this process. The aim of this diploma thesis was to investigate the CCL2 induced modulation of nociceptive synaptic transmission in the dorsal horn of spinal cord and the role of the TRPV1 receptors. To investigate this aim patch-clamp recordings of spontaneous and miniature excitatory postsynaptic currents (sEPSC, mEPSC) from superficial dorsal horn neurons in acute rat lumbar spinal cord slices were used. After acute application of CCL2 on the slice preparation from naïve animals, a frequency increase of both sEPSC and mEPSC was present. This CCL2 induced increase in both sEPSC and mEPSC frequency was prevented by the TRPV1 receptor antagonist SB366791 application. No changes were observed in the amplitudes of sEPSC or mEPSC after application of the CCL2, SB366791, or co-application of CCL2 and SB366791. Fear of pain

This bachelor thesis deals with the important relationship between pain, fear of pain and an avoidance behavior of patiens. The thesis is devided into two parts - theoretical part and empirical part. The first part defines the basic concepts such as pain, pain behavior, anxiety, fear and avoidance behavior. This thesis explains what is normal fear and which fear is already phobia. There are described differences in detail between anxiety and fear, which are often confused with each other. It brifly desribes two theories of anxiety and fear - Rachman's and Eysenck's. It adds four models of relations between fear and chronic pain that attempt to answer the question, such as bio-psycho-social mechanisms fear of pain may affect patient behavior. In conclusion, the theoretical section describes the methods of evaluation the fear of pain and strategy how to overcome it and not to give up it. The empirical section is focused on the intensity of fear in patient with chronic pain depending on gender, age and their current pain. There are also researched differences in the use of strategy of control over fear among men and women. Finally we found which gender use more avoidance behaviour of painful activities and movements.
