Effect of piracetam on advanced dementia with predominated cardiovascular symptoms

From 2001 to 2004 a retrospective non-invasive clinical trial on the effects off Piracetam was carried out in the nursing home in Česká Kamenice. The effect of Piracetam was observed on 150 patients aged 84,29 ( +- 3,5 years ) who suffered from an advanced form of vascular dementia. The patients were diagnosed in accordance with all currently valid criteria. At the time of the clinical trial the patients had been suffering from the disease for more than 6 months. A placebo was administered to the reference group composed of patients suffering from the same disease. The clinical trial lasted for a period of 12 months . The treatment with Piracetam did not demonstrate any positive effects which would be statistically substantial. This is probably due to an increased number of advanced forms of dementia syndrome with an irreversible loss of the brain's ability to compensate. For the above mentioned reasons an early diagnosis of demetia is of high importance because of a possibility to positively influence the disease. Effect of pentoxifyllin on advanced dementia with predominated cardiovascular symptoms

The high consumption of Pentoxifyllin in patients suffering from vascular dementia led to a concern and a clinical trial conducted in the nursing home in Česká Kamenice from 2001 to 2004. The retrospective survey was carried out on 176 patients and lasted 12 months. If we consider the currently valid criteria for the diagnosis of vascular dementia, the patients showed an advanced form of the disease. The experiment did not demonstrate any positive influence of treatment on either cardiovascular diseases or the mortality of afflicted patients. Despite the fact that we could expect the medicament to have a favourable influence on the symptoms of the advanced form of vascular dementia it was not proven so. It is absolutely essential for doctors to have good background knowledge of diseases and all kinds of particularities accompanying old age, especially when it comes to the early detection of dementia. High consumption of Pentoxifyllin led to a number of experiments and studies trying to prove its efficiency and hence justify prescripcions for the drug. Rehabilitation treatment at the spinal unit. Theoretical basis and practical experience

After spinal cord injury occur a lot of complication, with resulting from neurological picture. These can be central, peripheral or mix paresis in some cases even plegie. Often an autonomic nervous system can be demaged, causing complication practice in nearly all body systems. Rehabilitation attend belongs to basic care of paretic patients and is closely connected with basic with basic nursing methods from which the physiotherapy of these patients arise. This survey gives a summary of procedures and methods of physiotherapy , which help the patient to prevent and better respond to the complication treatment e.g. on respiratory and movement apparatus. Complex physiotherapeutic treatment concentrates mainly on prevention of complications. The physiotherapist must anticipate formation of complications and adapt the comlexity of the treatment. The aim of the thesis is to prove that active techniques of respiratory physiotherapy together with aerosol inhalations contribute effectively to treatment of acute and chronic respiratory problems and also to prove that this therapy increases patient's motivation to realize the importance and effectiveness of respiratory exercises . Benefits of above-mentioned therapy influence patient's professional life. Quality of life has also impact on family and social relationships of respective patient. Correlation of functional muscular test and fine motoricity of upper extremities in patients with congenital neuropathies

Objective of the work is to consider the connection between functional muscle test and fine motor skills of upper extremities of patients with hereditary neuropathies, especially with Charcot-Marie-Tooth (CMT) neuropathy. In the first part of the work there is mentioned some brief information about the characteristics, classifications, clinical symptoms, deformities of upper extremities, rehabilitation and treatment of CMT neuropathy. The practical part of the work is concerned with testing and evaluating the upper extremities of patients with CMT in the functional muscle test, the Jebsen-Taylor test, examination and handgrip rating. Novel approaches to diagnosis and treatment of selected endocrine disorders

Je známou zkušeností, že hyperkortizolismus urychluje aterosklerotické děje. Ty je možné sledovat ultrazvukovým vyšetřováním karotid a nověji též vyšetřováním cévní mikrocirkulace laser-Dopplerovskou metodou. Pro urychlení aterosklerotických změn u nemocných s Cushingovým syndromem jsou nesporně důležité známé faktory: hypertenze, obezita a hyperlipidemie. К nim se počítá zvýšená inzulínová rezistence, ale nemocní s floridním hyperkortizolismem mají normální hladiny glykemie a imunoreaktivního inzulínu. К aterogenezi u floridního hyperkortizolismu může přispívat zvýšená tvorba volných kyslíkových radikálů vznikajících při oxidaci lipidů a zvýšené hladiny pozitivního proteinu akutní fáze-orosomukoidu. Imunosupresivní charakter se projevuje snížením hladin autoprotilátek a snížením hladiny intercelulární adhezivní molekuly-1 (ICAM-1). К urychlení aterogeních dějů dochází i u nemocných, kteří jsou již dlouhou dobu po úspěšné normalizaci hyperkortizolemie. I když čerstvě operovaní a normalizovaní nemocní ztrácí do jednoho roku svou Cushingovskou obezitu, dochází později u operovaných nemocných opět к nadváze. Ta je spojena s poruchami lipidového spektra a u čtvrtiny nemocných i s hypertenzí. Nemocné po operaci pro Cushingův syndrom je nutné pravidelně sledovat, i když jsou dlouhodobě normalizováni. Problems of Care of Persons Suffering From Alzheimer's Disease in the Czech Republic

This bachelor thesis deals with the issue of care for people suffering from the Alzheimer disease in the Czech Republic. It focuses on the causes, treatment, prevention and the consequences that the Alzheimer disease imposes on the patient and also on the family and surroundings. I will try to describe the nature of the Alzheimer disease, its stages, care and treatment options for patients and the subsequent effect as closely as possible. The aim of this thesis is also to approach individual forms of treatment available for the patients with the Alzheimer disease in the Czech Republic.

Nonverbal factors in psychotherapy of adolescents

The thesis deals with one of the areas of children's psychology - psychotherapy of adolescents. The study mainly focuses on different non-verbal moments, or factors, occurring at psychotherapeutic sessions held with adolescent patients, and it outlines the possibilities of application of these factors in the psychotherapeutic process. The objective of the thesis is to show that sometimes seemingly insignificant non-verbal moments may play an important role, especially in therapeutic work with adolescents, and therefore therapists should pay increased attention to these symptoms. This also applies in case that a patient was originally indicated a psychotherapy, the main medical treatment medium of which is predominately an interview (for example a family therapy). The thesis is based on one of the assumptions that in general non-verbal methods, in this case at least some of their elements, may work as facilitators of an interview, for example if the patient fails to express certain personal experience of his or her, his/her feeling or emotion with the help of words, or when a therapist fails to enter into verbal communication with the patient.
