Influence of pressure-pain and vibrotactille sensitivity by isometric relaxation in a location with myofascial trigger point
Medunová, Kateřina ; Čech, Zdeněk (advisor) ; Otáhal, Jakub (referee)
Objective: The aim of this study was to evaluate the effect of post-isometric relaxation (PIR) on pressure pain treshold and vibrotactile theshold in the area of myofascial trigger point. Methods: The change in pressure pain threshold and vibrotactile treshold was taken from the area above myofascial trigger point number one trapezius muscle - pars ascendens, and the vibration treshold at dorsum of metacarpal bone of the index finger. Fifty myofascial trigger points were examined (twenty-four from a group of probands with chronic cervicalgia or cervicobrachialgia syndrome, twenty-six in the control group). Results: The pressure pain threshold and vibrotactile threshold did not change after the PIR in the group of patients. But the threshold of vibration sense significantly decreased (nearly by 18%, p = 0.001). The result was opposite in the control group. Pressure pain threshold increased by 11% (p < 0.001) and vibrotactile threshold fell by almost 16% (p = 0.001). Vibration threshold did not change. Conclusions: The study confirmed that PIR can affect not only the pressure pain treshold, but also vibrotactile and even vibration treshold. PIR revealed an alteration to pain perception, vibrotactile and vibratin sensation in the group of patients. This alteration had not been obvious (visible) before...

Ion attachment and ion emission of dust grains
Vyšinka, Marek ; Němeček, Zdeněk (advisor) ; Wild, Jan (referee)
Small dust grains could be found in many environments. Because of their size, their charge and the electric forces acting on them play an important role in their dynamics. The dust grain charge is influenced by many different processes. Among other methods, charging processes can be studied by trapping a single dust grain inside an electrodynamic quadrupole where it is put into specific environment (ion or electron beam, UV radiation). The progress of the charging process can be inferred from the changes in the grain charge. Our work contributed to the building of a new laboratory set-up for study of the charging processes. Specifically, a new Faraday cup for measurements and stabilization of the ion and electron gun currents is designed and its function verified. It solves the shortcomings of the previously used Faraday cups. Further, an amplifier for the ion gun current measurement is designed and constructed along with the design of digital stabilization of the current. The stabilization is neccesary for the interpretation of measurement results and its digitalization will allow more flexible adjustment and easier connection with the control computer. Finally, data acquired during the charging of dust grain with different ion species are evaluated in order to study the effect of different ions on surface...

Early diagnosis of occupational asthma is important especially for the prognosis of this disease. The confirmation of the diagnosis of occupational asthma is sometimes difficult using diagnostic methods available nowadays. That is why searching new methods is very important. Analysis of exhaled breath condensate (EBC) by liquid chromatography combined with mass spectrometry enables the separate detection of cysteinyl leukotrienes (LT) - LTC4, LTD4, LTE4; LTB4 and 8-isoprostane. If patients with occupational asthma and controls were compared, only LTC4 was significantly higher among all EBC parameters studied in asthmatics (despite corticosteroid treatment). This marker could be used in the future diagnostics. Monitoring of 24-hours variability of EBC parameters in patients, in whom occupational asthma is suspected, showed relatively high intraindividual and interindividual variation. It is evident therefore, that if only one daily measurement in asthmatics would be possible (which is common in articles of several authors), it should be collected in the same period of day in all persons. In negative bronchoprovocation tests significant changes of EBC parameters were not found. The evaluation of positive bronchoprovocation tests was limited by small number of patients, however in five persons from six, the...

Endoscopic laser-assisted cordectomies classified by Remacle and Rudert (typ I-V cordectomies) comprise a vast range of procedures from simple vocal cord stripping or submucosal cordectomy (type I) through more extensive surgery (subligamentous cordectomy- type II, transmuscular chordectomy- type III) to complete (type IV) or extended cordectomies (type V) used when tumour involves the anterior commisure, arytenoid region, subglottic region etc. Endoscopic laser-assisted surgery of the larynx is usually indicated in early stages of infiltrative malignant disease of the glottic region (Tla-b, T2) or in preneoplastic conditions (laryngeal intraepithelial neoplasia - LIN I, II or III). Radiotherapy is also believed to be an equivalent type of oncological therapy for these diseases. Both surgery and radiotherapy can worsen one of the main functions of the human larynx- the human voice. Because the overall survival rates and local control rates are quite similar in both types of therapy, one should always have in mind something that is beyond the main goal of achieving radical removal of the tumour- patients quality of life (QoL) and quality of voice. It is very interesting to compare QoL and voice quality in different types of treatment. The possibility to conserve the radiotherapy for the possible recurrence...

Molecular factors of artherogenesis in chronic kidney disease with experimental model of chronic inhibition of Nitric Oxide Synthesis and its Effect of HMG-CoA reductase inhibitoin
Lecian, Dalibor ; Matouš, Bohuslav (advisor) ; Tesař, Vladimír (referee) ; Zadražil, Josef (referee)
The experimental part of the thesis covered the markers involved in nitric oxide deficiency model in rats. The analogue of arginine N-nitro- L -arginine-methyl ester ( L -NAME) was used. L -NAME administration resulted in sustained elevation of BP, decreased GFR, and in higher albuminuria compared to control animals. The administration of hypolipidemic drug - atorvastatin to Wistar rats was studied. The statin used caused lower albuminuria and prevented decreases in GFR in L -NAME rats without having an impact on hypertension. CAV1 protein expression was similar in all groups of rats. In contrast, VEGF expression and RhoA activity was increased in L -NAME-treated animals, and normalized with coadministration of atorvastatin. Clinical part of the thesis focused on the analysis of markers of oxidative stress in obese and non-obese patients with CKD and after kidney transplantation. The patients with CKD had elevated serum concentrations of AGEs, sRAGE and ADMA with the maximal concentrations of AGEs in the patients on hemodialysis. The successful kidney transplantation decreases the AGEs, sRAGE and ADMA concentrations but not to normal value according to general population. Patients with obesity exhibited lower concentrations of sRAGE in CKD as a protective factor for cardiovascular disease. Concentrations of...

Biomarkers in Myocardial Damage Diagnostics
KOPŘIVOVÁ, Olga
Anotace (abstract) Biomarkers diagnosing myocardial damages have recently been a very dynamically developing part of clinical biochemistry. Implementing of new biomarkers is very prompt; they are mostly implemented into practice without wider clinical and analytical experience. During a few years these are new methods to determine myoglobin, troponins (cTnI, cTnT), CK-MB mass, BNP etc. The effort of clinical biochemists and cardiologists is to cover all phases and levels of myocardial damage in acute coronary syndromes by means of cardio markers. These investigations are dramatically changing the view of diagnosis, determination of prognosis and therapy in acute coronary syndromes. In my thesis I covered retrospective data analysis in patients with the pain in the chest, presented at an inpatient clinic. The value of myocardial damage indicators and their dynamism was analysed from the definitive diagnosis point of view. I performed my own analysis at OKB Nemocnice Písek, a.s. (Hospital Písek, Inc.). The aim of my thesis was to cover the significance of particular myocardial damage markers and find out if investigated set of markers is sufficient for correct and prompt diagnosis of acute myocardial infarction. In terms of this thesis I tried to find out if the panel of cardiac markers contains excess values. In OKB Písek, a.s. the panel of myocardial damage markers includes: myoglobin, troponin I, CK-MB mass and the marker of heart failure BNP. In the samples of fifty-six patients I analysed the presence of the above mentioned markers of myocardial damage and heart failure. I examined only the markers which were determined by a physician. In the group of investigated patients with chest pain, measured values of troponins proved myocardial infarction in sixteen patients, values of CK-MB mass in sixteen patients and values of myoglobin in twelve patients. In the control group of twenty patients with chest pain the boundary values of troponin I were measured in two patients, without the increase of other myocardial damage markers. High value of troponin I was measured in two patients in non-ischemic myocardial damage (acute pericarditis), which agrees with data in scientific literature. The thesis shows that the level of troponin I reflects myocardial damage relatively accurately; troponin I is sufficiently cardio specific but is not a sufficiently early marker. For early management of acute myocardial infection it is important to determine myoglobin, which is detectable within two hours since the beginning of troubles but is not sufficiently cardio specific. The course of CK-MB mass elevation wave copies the course of troponin I elevation wave. That is why the investigation of this marker appears to be excessivet at current investigation of troponin I and myoglobin to prove myocardial damage, which is in agreement with scientific association recommendations.

Metrological characteristics of electrocardiographs
Brožík, Stanislav ; Hozman, Jiří (advisor) ; Beneš, Jiří (referee)
In measuring of electrical activity of hearts, the control of accurate function of electrocardiograph and elimination of the measurement deviations is essential. This diploma thesis describes the procedure of putting together and use of nine gauging fixture according to international recommendation R 90 (Edition 1990). These devices are directly connected to the tested electrocardiograph. When following the recommended procedure of measurements and equations it is possible to find out if the parameters of electrocardiograph don't exceed the allowed measurement deviations.
