The thesis focuses on the area of diagnostics in preschool children in kindergarten in the Hradec Králové region. The main aim of this thesis is to set up a pedagogical support plan, the first level of support measures according to the WELCOME IDEA application for premature children. Another aim is a practical demonstration of how to work with the pedagogical support plan according to the WELCOME IDEA application, analysing its possibilities and limits. Last but not least, the aim is to underline the need of diagnostics in preschool children as a tool for an appropriate pedagogical support plan, which is not only a means for teachers to create optimal learning opportunities for children. The thesis has two parts. In the theoretical part we meet the concepts of family and parenting, psychological needs of parents of premature children, ontogeny of the premature child, psychomotor, communication and socialization in preschool children. Health literacy of mothers with premature infant

Health literacy represents the cognitive and social skills which determine the motivation and ability of individuals to gain access to, understand and use information in ways which promote and maintain good health, according to WHO. Common health literacy level may be sufficient in the case of smooth pregnancy and natural childbirth. However, if there are some health problems, it is necessary to support an increase in health literacy level of mothers. Main objective of this thesis is to describe the objective health literacy level of mothers with premature baby and to identify areas that are for these mothers most problematic. The theoretical part deals with demographic data related to premature births, the basic definition of prematurity, an essential characteristic of neonatology and organization of care, premature baby, the most common medical complications and prognosis. This Master's thesis deals with the topic of impaired communication skills in premature infants. The thesis is divided into theoretical and practical parts. The theoretical part consists of three main chapters devoted to the development of children's speech, premature babies and care for premature babies. The first chapter describes speech and language, the ontogenetic development of speech, language levels in the ontogenesis of speech and impairment of communication skills in childhood. The second chapter defines a premature baby and discusses the most common postpartum complications. The third chapter is devoted to the care of premature babies in the Czech Republic, describes the logopaedic care regarding such children and their prognosis. The practical part is based on the methods of qualitative research. A triangulation of research methods was chosen - document analysis, observation and interview. The main goal was to describe how impaired communication skills are manifested in premature infants. The partial goal was to find out what postpartum complications occur in premature infants. Management of breast-feeding premature infants with the application of classification systems NANDA International, NIC, NOC

In a theoretical part of the paper, the main focus is on characteristics and terminology of premature babies, breastfeeding, classification systems NANDA International, NIC and NOC, about also management which is an integral part of nursing process. Both quantitative and qualitative researches were used to obtain and process all data. In quantitative method was used a method of document was used, content analysis and quasiexperiment. Results of quantitative research were processed in SPSS software, version 16.0 Statistical Package for Social Sciences, where a method of nonparametric correlation was used. A pen and paper interview with open questions was used in qualitative section of research. Before the research itself, a deputy of nursing care in hospital Nemocnice České Budějovice, a.s. was addressed. The research was implemented between February 2013 and April 2013. First research cluster consisted of nine mothers and eleven premature babies in which two of the mothers had twins. Basic criteria for entering the research, was a necessary hospitalization of both the mother and the child, which the hospital Nemocnice České Budějovice, a. s. offers. The major criterion was an initiated lactation. Second research sample consisted of nurses working at Stanice intermediární péče II. (IMP II ? rooming) of Nemocnice České Budějovice a. s. First of goals was to create files from classification systems NANDA International, NIC, NOC for development and support of breastfeeding for premature babies. The main goal of this work was to create a comprehensive documentation mediated by classification systems NANDA International, NIC, NOC for growth and support of premature babies. The theoretical content of the paper itself was used to create documentation and its usage in praxis. It was not mentioned to create new, but to apply already well known, verified and proven classification systems, which can make not only breastfeeding, but also the daily routine of a nurse more efficient. The suggested documentation solves the issues of breastfeeding of premature babies to help mothers, but also to increase a professional prestige of nurses. Documentation was compiled to include all the necessary information needed for the correct nursing process method. Significant part of documentation is a preliminary case history of mother and child, which offered valuable information. Further attribute were nursing diagnoses dealing with breastfeeding issues of premature babies and intervention realized during hospitalization. It was necessary to address each entry concerned with premature babies individually, based on complex evaluation of overall condition. Many pieces of knowledge from specialized literature on the topic of premature babies and personal experience of doc. PhDr. Mária Bolendovičová, PhD. were used to create the documentation. Second aim of the study was verification of selected specimens on clinical praxis. Third aim was to monitor nurses in Stanice intermediární péče II. (IMP II ? rooming) and their attitude to usage of classification systems NANDA International, NIC and NOC. Based on interviews with nurses it was proven that nurses have better knowledge of NANDA Internationals classification systems than knowledge of NIC and NOC. Final documentation was tested in the perspective of medical staff and its exploitability in clinical praxis. It was found that nurses sense the classification systems NANDA International, NIC and NOC as an appropriate tool, but only with more staff present to work on station. From statements of nurses was clear that establishing a diagnoses is a responsibility of each nurse taking part in nursing process.
Caring for High-Risk Newborns after Release from Hospital Treatment: A Parent's Perspective

My Bachelor?s thesis focuses on the issue of children with a development risk, and deals mainly with the care provided to these children after they are released from hospital. The thesis is divided into a theoretical and practical part. The theoretical part defines important used terms. In the Perinatology chapter, general provisions are described as well as the system of perinatologic centres. To understand the topic, it is necessary to know the classification of infants and understand the manifestations and consequences of premature birth, so the second chapter provides basic information about neonatology. A separate chapter deals with the definition of chronological age and corrected age. These are important to secure proper monitoring of the child?s development. The development of prematurely born children differs from the development of children born in the standard term, and this fact must be taken into consideration. Nutrition is an important aspect for proper development. Recent trends especially stress breastfeeding of the child by its own mother. Information about nutrition and breastfeeding and the specifics of nutrition for prematurely born children are described in a separate chapter of the thesis. The following chapter deals with the issues related to releasing children with a development risk from hospital, and the care following their release. The care provided by a development care centre is emphasised. The last chapter provides a selection of the most common diagnoses experienced by prematurely born infants. The practical part contains qualitative research carried out by means of a semi-structured interview. The interviewed sample contains ten mothers of prematurely born children or children with other development risks. Respondents were women that I know, who regularly visit a development care centre with their children. A key selection criterion was the corrected age of the child, which had to be less than 24 months. The goal of the research was to evaluate the activity of the development care centre from the parents? perspective. A single research question was asked: Which deficiencies do the parents see in the activity of the development care centre. The collected data was analysed and lead to a clear conclusion and answer to the research question laid down in the thesis. The results of my research clearly justified the activity of development care centres, which are usually part of large maternity wards. The centres are accepted very positively in all aspects. The interviews also showed some deficiencies, which were described in the discussion and summarized in the conclusion. Since neonatology care is constantly improving, the ratio of surviving children with an extremely low or low birth weight is much higher than in the past. It is necessary to take into account that care does not end after the child is released from hospital. Premature birth carries many risks, which threated the child?s development. It is thus necessary to secure continuous care and monitor the child after release from hospital into home care. This topic has recently been often discussed. The mental development of the child is as important as the child?s physical development. It has been proved that prematurely born children may have some psychological problems. The conclusion summarizes the deficiencies that mothers find in the activities of development care centres, and describes recommendations that should lead to improvement of this care. These include a need for greater engagement of nutrition and breastfeeding counsellors, improved education of nurses employed in community care and improvement of the quality of cooperation with children?s physicians. I would recommend my thesis to the expert and general public. It could also be of use to students of nursing ? midwives, children?s nurses and others who are interested in the issues of children with a development risk.
Nursing care and nourishment of premature baby

NURSING CARE AND NOURISHMENT OF PREMATURE BABY A newborn baby isn´t a little adult, a newborn baby is basically a defenceless creature demanding a very complex care that aims at creating secure conditions for its life. It requires love, tenderness and a lot of devotion for the whole period of 24 hours. The bachelor´ s dissertation paper is dealing with the subject `` Nursing care and nourishment of a premature baby{\crqq} In the theory section the difference between a physiological and high risk newborn baby, the principles of care on the intermediary ward, the nourishment problems and the favourable consequences of breast feeding and breast milk not only on these children are described. Some of the aspects of the care are pointed out aiming at satisfying the basic needs of a baby. Dissertation objective: The paper itself is engaged in finding out something about how well mothers of high risk newborn babies, who are hospitalized on intermediary wards in Jindřichův Hradec and České Budějovice, are informed on a suitable care and nourishment procedures in their children. It should find out how far they have gone with their knowledge of the way their children should be taken care of before they leave the hospital. It is supposed to highlight the drawbacks of the staff , to remove these drawbacks and thus to improve the quality of care. Hypotheses: Two positive hypotheses were formulated referring directly to the particulars goals. 1/ The mothers who are hospitalized on the intermediary wards for newborn babies in Jindřichův Hradec and České Budějovice are aware of the necessity of properly nourishing the child. 2/ The mothers who are hospitalized on the intermediary wards for newborn babies in Jindřichův Hradec and České Budějovice are sufficiently informed about the proper care leading to meeting the needs of their children. Data Collecting: The research mentioned above was carried out on the intermediary wards in Jindřichův Hradec and České Budějovice. A questionnaire was composed to collect the relevant data. There were 22 questionnaires handed out in each ward and the research group was set up including 37 mothers of the hospitalized children. The questions included in the questionnaire referred predominantly to how well the mothers are informed about the child´s care and nourishing problems and they were supplemented with some identification. The gained information was properly processed and evaluated. The results were put in graphs. To get a more distinct picture of the results a pie chart was selected. The research results are assessed in the discussion and confronted with the hypotheses. Conclusions: Both the nourishing care and the nourishment of a premature baby are a subject involving a lot of medical, psychological and social problems. A maternity ward nurse plays a very important and definite role in the care of a premature baby. Supported by her profesonal abilities she is able to initiate such a method of work that reacts immediately to the needs of a child in a positive way. The educational contact with the child´s parents is eqally important and plays a very precise role. The fact how well mothers are informed is influenced by their age, by how much they are interested in getting information and its resources and how much they are willing to be educated by the medical staff on the intermediary wards in Jindřichův Hradec and České Budějovice where they are hospitalized with their children. The research results are very good.
