Vestibular rehabilitation efect of patients with postural stability impairment

Disorders of postural stability seriously impair the quality of life. Falls are more common in the elderly, where they can have very serious consequences. However, this issue does not always concern only seniors. These disorders can be caused by a number of reasons. The diploma thesis is devoted to postural stability in patients with vestibular disorders. The aim of the theoretical part of the work is to present comprehensive information on the control of postural stability, its disruption and the integration of sensory systems needed for its maintenance. Further chapters describe postural control assessment and rehabilitation options. The aim of the experimental part was to determine the effect of vestibular rehabilitation on postural control. The evaluation was carried out with the help of a static stabilometric platform Kistler®. 9 patients with postural stability disorder in the chronic stage participated in the research part. A total of 5 men and 4 women, aged from 35 to 78 years, on average 59 with SD ± 16 years, with peripheral (4), central (4) or combined (1) vestibular disorders were exposed to a 3- month rehabilitation program. The use of biofeedback for stability training in patients with degenerative cerebellar disease

The graduation thesis deals with the issue of balance disorders due to a cerebellar damage. It deals in detail with the treatment of patients suffering from degenerative cerebellar ataxia and with the use of biofeedback technologies as means of postural stability treatment. The aim of practical part of this work was to investigate the effectiveness of therapy using biofeedback for patients suffering from progressive ataxia due to cerebellar neurodegeneration. The research sample consisted of 8 patients. The patients underwent a total of 18 therapy sessions within a twelve-day rehabilitative program. The therapy contained a training of stability in various positions with the elimination of visual control. A device provided an additive information about head sways for the patients via a tongue electrotactile stimulation. The effects were assessed by means of posturography, functional clinical tests (BESTest, Dynamic Gait Index) and questionnaires (Activities-specific Balance Confidence Scale, Dizziness Handicap Inventory). Three assessments were performed: immediately before, immediately after, and 30 days after the rehabilitative program. Slackline as a therapeutic possibility to influence postural stability

Objectives: To summarize knowledge about a control of posture, a balance training and a testing on balance platform. I investigate in experimental part of my thesis if slackline training will affect stabilography and differences in quantitative parameters will be found in a training group or a control group. Hypothesis: A specific slackline training positively affects stability which was measured in posturographic selected tests done by the training group. Methods: We test postural stability in control (10 persons) and training (10 persons) group in selected tests of stability. We used a balance platform for testing. The data were statistically analysed. Results: 4 weeks of intense slackline training result in improvement of postural control. That is support by the data of stabilometric examination. The results were just below statistical significance (p > 0.05). Conclusion: The results testify that slacklining can improve postural control in healthy subjects. The results were on the boundary of statistical significance in our intervention group.

Title: Assessment of the effect of CDP training in patient after lower extremity amputation Objectives: The aim of this diploma thesis is to describe the influence of the individually set training programme on the computerized dynamic posturography (CDP) NeuroCom Smart EquiTest System (further referred to as EquiTest) independent of traditional therapeutic exercises in the patient after unilateral transfemoral amputation of the lower limb and to monitor the effect of this training on postural behaviour, ability of functional mobility and balance, frequency of falls and balance confidence in an individual after amputation. Methods: This is an experimental pilot case study that monitors the effect of an individually designed five-week training programme (with a frequency of exercises 2 times a week) on the EquiTest in one patient after unilateral transfemoral amputation of the lower limb. Examination of postural functions was performed by the EquiTest using SOT, MCT, and LOS tests. The ability of functional mobility and balance was tested using the functional Timed up and go test (TUG). The frequency of falls was detected from the proband's medical history. Title: The Effect of Thermotherapy on Postural Stability assessed via NeuroCom SMART Equitest Objective: The aim of this study is ti determine whether a one-time application of positive or negative thermotherapy has a positive or negative effect on the postural stability and/or balance in a sample of healthy young adults. The tool for achieving this goal will be statistical analysis of the results of the computerized dynamic posturography before and after application of thermotherapy. And futher a comparison with the results of the control examination. Methods: In this study we have used one-time application of positive and negative thermotherapy in the form of water immersion. A total of ten women and ten men with average age 24 years (sd = 2,18), average height 174,45 centimeters (sd = 7,72) and average weight 78,10 kilograms (sd = 18,10) took part in our study. Influence of airflow control with vocal cords on dynamic stand stabilization

Title: The effect of airway control on stance dynamic stability Objectives: The aim of this thesis is to find out whether and how will the influence of vocal cords modulation be manifested on the dynamic stabilization of the standing body during translational shifts of the supporting surface of different intensities and A-P directions. Thus, building on the findings of Massery et al (2013). Methods: The thesis has the character of qualitative research. The experiment was attended by 23 healthy probands, of which 7 men and 16 women aged 20-40 years. Spirometry was used to test the objectivity of airway airflow during breathing / phoning maneuvers with different vocal cords positioning and dynamic computer posturography using the Neurocom Smart Equi Test System and its Motor Control Test, which evaluated the effectiveness of automatic postural responses. We connected the posturograph with the spirometer using the Kistler accelerometer (type 8766A100BB). The course of the experiment was simultaneously recorded by a camera (GoPro Hero 7). Changes in posturography in pediatric patients with cerebral palsy after therapy by active videogame Nintendo Wii and Vojta reflex locomotion

Aim: The aim of this study was to determine the impact of therapy by active videogame Nintendo Wii and by Vojta reflex locomotion on a static and dynamic postural control at a group of children with mild cerebral palsy. Methodology: 14 children (from 6 to 18 years old) were randomly divided into two groups. Both groups received both therapies (active videogame therapy and Vojta reflex locomotion) with a 6 months gap, but in an opposite order. Posturographic measures were obtained before the start of the therapy, after therapy and 8 weeks after the end of the therapy. Results: After both therapies, there was a significant increase in the COP sway velocity in the mCTSIB subtest on a firm surface with closed eyes and the COP sway velocity in the mCTSIB subtest was reduced on a foam surface with closed eyes. Further, after both therapies, there was a significant reduction in the time required to perform the turnabout in SQT test. Other tests (LOS, RWS, WA, TW) and subtests did not change significantly. There was not a significant difference between therapies, but in the post- therapy tests, VRL had a greater tendency to maintain its effect 8 weeks after therapy. Development of somatosensory system in the context of the postural - locomotion system and the as sessment possibilities

The aim of the work is to describe the somatosensory system function development in the context of postural- locomotion function in preschool age and to describe the possible assessment of this function in preschool children, on the basis of research. The theoretical chapters deal with the development of the somatosensory system and the postural- locomotion system, also with the physical development of the preschool child. The practical part presents the outcomes of the research, namely how the development and usage of somatosensory signals by central nervous system is influenced by growth of the preschool child's organism. Lastly, the work presents the possibilities how to assess the preschool child's somatosensory system in common physioterapeutic practice. A preschool child's case report is included.
