The Progress of Nursing in the 21st Century from a Patient's Perspective

Intoduction: Nursing is a human-oriented scientific discipline. Due to various world events, pivotal changes have occurred in this field. These changes have led to improvement in nursing care that is being provided in health facilities today. The Aim of the Thesis: The aim of the thesis was to determine changes to the field of nursing through the eyes of care recipients (i.e. patients) that happened during the 21st century. Furthermore, the goal was also to identify factors that influenced these changes and evaluate nursing care through the eyes of patients. Methodology: The thesis was carried out by means of a qualitative research survey in the form of semi-structured interviews with patients. The selection criteria for choosing an informant were residence in the South Bohemian Region and 18 years of age (or older). These informants were chosen by snowball sampling. The research survey included 20 informants. The interviews were recorded on an answering machine, literally transcribed and consequently analysed. Results: The thesis identified a total of 8 categories and 23 subcategories. The category of Scientific and Technological Progress was recognized as the central category. It is the driving force for most changes, including changes in education. The patients thought that the biggest change was modern equipment, new aids and devices in health facilities. They also mentioned innovations in the education of nurses and they agreed with the changes. The patients rated nursing care as a highly professional, expert and demanding field. They understood that more and more demands were being placed on the nurses. Moreover, the patients described the profession as prestigious, but also said that the public should be more informed about it. Conclusion: It is certain that nursing care will be influenced by further advances and innovations because science and technology cannot be stopped. In this context, the demands on nursing personnel will increase and the patients will be more demanding as well. Therefore, it is necessary to continue working on nursing care. However, it has to be taken into account how these changes and developments are perceived by the patients.
View of doctors on current nurses practise

In this bachelor's thesis we will deal with key aspects of the work of a nurse from the point of view of medical staff and we will also investigate the opinions of physicians on the current system of Czech nursing education. The present time has high requirements on nurses, especially on the work of a nurse and higher education. Today is very often said that nurses should be equal partners with physicians in the care of patients and take a respected position in a multidisciplinary team. The work is divided into theoretical and empirical. In the theoretical part of the bachelor's thesis we will deal with the history of nursing, current nursing and we will also focus on the doctor nurse relationship. In the empirical part of the work we will examine the view of physicians on the issue of education and work of a nurse. For processing of the empirical part was chosen qualitative research together with the method of semi structured interview. The research group consists of 8 medical staff from various clinical fields. Based on the interviews, the results were divided into 2 large categories and 8 subcategories. Gained results showed that physicians perceive current education as complicated and too long, preferring to devise a better system that would precisely define and adjust questions of increasing competencies and the related education of nurses and length of study. Furthermore, doctors stated that university studies bring benefits in the form of obtaining a university degree and higher prestige of the profession. Physicians consider the work of a nurse to be essential and have the opinion that the nurse is an indispensable part of well provided care for patients. We also found out that the most common reasons that affect the emergence of a conflict situation between the nurse and the doctor are stress, overwork, mutual disrespect associated with inappropriate behavior and violation of the competencies of both parties. Based on the statements, we can also state that doctors consider the nurse as their partner in the field of professional patient care and they do not have problem with accepting proposals or recommendations from nurses.
The distribution of medicaments by the nursing sister in the social wealfare institutes

This bachelor's thesis focuses on the distribution of medication in the social welfare health institution. The objectives of the thesis were to focus on procedures of distributing medication by nurses and to introduce the most frequent complications when distributing the medication in the social care institute. The thesis is composed of two parts, theoretical and practical. The theoretical part describes social and health care services, a health and social care system, a general pharmacology, medicaments' distribution and the complications during the distribution, and the competency of nurses. The research part concentrates on individual study and describes a methodology of research and its organization. This research was done via survey, and the results are analyzed and discussed. The lack of general nurses in inpatient care- The fact or myth

The thesis analyses causes of the lack of medical workers providing nursing care in inpatient facilities and observes in broader connections the progress of present staff condition. By means of managerial methods for determining number of nursing staff, it discovers, whether it is possible to define the number of nurses in inpatient medical facilities as insufficient, or whether the present number of nurses is critical with its origin in lack of other nursing employees. This thesis is supposed to show that the lack of nurses is not only a problem in the last few years, but it has been here since the beginning of the 20th century and it has been connected with all social events to present days. Starting point of the evaluation of the situation will be a brief historical excursus with a brief analysis of professional, legislative and social impacts which influenced the number of nursing employees in the past and their consequences up to now. Medication management is important and very complex part of health care, which is ensured by many healthcare professionals. Intensive care specifics include not only extensive pharmacotherapy but also the administration of medication that can directly and very promptly affect vital signs. This fact brings huge demands on safety during providing care. Traditionally, it is assumed the medication is prescribed by physician, and the nurse ensure administration based on physician?s detailed written prescription. However, activities related to this part of healthcare are currently much more complex. The nurses are expected to be fully competent to fulfil their role in this area at the point of graduation, even in the specialized environment. In addition, it seems that the formal nurses? competencies in medication management are not, in spite of heavy regulation, clearly specified. Although the close relationship between medication management and quality of provided healthcare is obvious, the role of nurses including expected and required demands of current practice have not been analysed in detail yet. The aim of the thesis was to analyse the competencies and nursing role in medication management at intensive care units.Mixed methods design was chosen. Quantitative techniques were used to analyse extent of education and nurses? knowledge in this area, and qualitative techniques were used to analyse current practice and requirements on competence of nurses. There is no difference in formal nurses? clinical competencies in the field of medication management in the Czech Republic depending on the level of education. There is also no difference in extent of pharmacology education during qualifying study at diploma and degree level. However, the current practice in medication management at intensive care units is hugely multifaceted. In some respects its demands greatly exceed the expectations that can be derived from formal set of nurses? clinical competencies in medication management. Although the previous medical doctor?s prescription, especially in case of medication with systemic effect, is considered to be the necessity, the experience of administering this medication in certain circumstances without previous medical doctor?s prescription has been also described. Patient assessment and dose titration of certain medication according to its effect or e.g. laboratory values were described as relatively common part of complex nursing care at this type of workplace, as well as adherence to specific procedures during administration. Particular procedures were workplace related and varied among units but the importance of quality of care was emphasized. The administration of topical medication in specific circumstances, such as eye, oral or skin care in patients with decreased level of consciousness, was perceived more as basic nursing care than medicine administration. However further research will be necessary to verify the extent of this practice. The nurses? knowledge in medication management varied according to the specific topic, but also according to the level of education or previous work experience including previous practice at intensive care environment, although formal competencies in medication management do not differ. Due to the complexity of the medication management and increasing demands on the competence of all healthcare professionals including nurses, it is essential to consider a fundamental revision of the formal nurses? competencies to be necessity at least in this area. Consequently, it could be possible to address the scope, form and content of nursing pharmacology education so that set requirements are met.
Role of the nurse in the administration of drugs in the workplace intensive care

This thesis deals with the role of nurses in the administration of drugs in the intensive care unit. The goal is to find consistency between the legal and factual state of drug administration, ie analyze, what are the non-medical healthcare professionals authorized in accordance with applicable legislation and internal regulations of the medical device to do and what they are actually able to perform safely. The theoretical part defines the terms related to the administration of medicines. I deal with the skills of paramedical staff, indications of medicines, their administration, pharmacotherapy, mistakes and evaluation of the quality and safety of health services. The theoretical part deals with the current status of this issue and also provides insight into the history of drug administration. In the empirical part of the thesis I work with the results that I received questionnaires. Respondents are nurses, paramedics, nurses of leading nursing management and physicians. Individual items are analyzed and used to answer defined research questions. Bachelors of nursing - how are they percieved by nurses

The thesis is focused on the university education of nurses from the point of view of nurses with high school education. The theoretical part briefly describes the history of the nursing education in our country, it mentions the most significant changes concerning the education of nurses and outlines certain legislative changes that are being prepared. The theoretical part also presents a survey of the universities and faculties where it is possible to study nursing at the present time. The research for the empirical part of the thesis was carried out in a form of a questionnaire inquiry in a faculty hospital. The targeted group of respondents were high school educated nurses without further specialisation. The resulting group, whose responses are used for this thesis is made up of 107 nurses. The empirical part of the thesis is aimed at stating the opinions of high school educated nurses on university educated nurses and the need for university educated nurses at the patient's bedside. The thesis also attempts to find differences between high school educated and university educated nurses.
