Analysis of selected foreign national eHealth strategies and a concept proposal for the Czech Republic

The diploma thesis is focused on the analysis of national strategies in the area of eHealth. The main aim of this thesis is to analyse the foreign national eHealth strategies and subsequent draft concept for the Czech Republic based on achieved results. The first chapter contains theoretical introduction into the field of eHealth, the main sources of research, history and definition of eHealth. The second part is devoted to the analysis, comparison and critical assessment of national eHealth strategies based on criteria in the countries of Denmark, New Zealand and the Slovak Republic. Third and final chapter is focused on the assessment of the current state of eHealth in the Czech Republic and the draft concept of eHealth strategy in the Czech Republic, which is based on the conclusions reached in the second chapter. The main contribution of this work is the analysis of selected national strategies and draft concept for eHealth strategy in the Czech Republic.

Bachelor thesis is focused on IS/ICT organizations, which contribute to the development of electronic healthcare in the Czech Republic.Purpose of this thesis is to present individual organisations which partake in the development of electronic healthcare and to analyse relationships between these organisations.Introductory part contains a classification system. Every studied organisation will be classified into one of the categories that are contained in the classification system. Following part ad-dresses introduction of individual organisations and their partners with whom they cooperate. Last, and most important, part is focused on analysing how organisations are tied through pro-jects and conferences which they attend.
Project of Electronic Nursing Documentation in the eHealth Context

The dissertation on Project of Electronic Nursing Documentation in the eHealth (worldwide concept of electronic healthcare) Context analyses use of information technology in healthcare focusing on electronic healthcare documentation and also analyses means of information technology implementation using project management. The dissertation goal is to create a plan of the Project of Electronic Nursing Documentation at University Hospital Plzeň using standardized methods of the IPMA Project Management focusing on risk analysis and management and use of acquired knowledge in the eHealth context. Software development and implementation of the new clinical information system module utilizes agile methods of software development. The dissertation states the definition of project and project management within its standardized methods, presents means of healthcare documentation in compliance with current legislation and terms of healthcare documentation use in its electronic form, describes the hospital information systems, data security policy and mentions the National Healthcare Information System.
The Economic Potential of Existing eHealth Systems in the Czech Republic

The purpose of this dissertation is to explore the contemporary area of information technologies employed in health care (eHealth). Focusing on the systems already employed in the Czech Republic, it aims to analyse them, to quantify expenditures of their introduction and maximum benefits derived from these, as well as to evaluate their real current profit. Out of the three existing national systems, this dissertation concentrates on the two only (eRecept, ePACS), as it became impossible to evaluate the third -- IZIP system due to scarcity of the relevant information available. In the field of expenditures not only generally published numbers are taken into consideration, but this dissertation also evaluates the expenditures of other subjects involved (IT systems producers, health care providers, etc.). As the first dissertation in the Czech Republic it quantifies benefits derived from the existing eHealth systems and unlike foreign theses it greatly emphasizes just the expenditures of other subjects involved. Included in this dissertation is also the evaluation of achievements of the eHealth systems in the Czech Republic complete with supposed grounds for this situation. This dissertation also recommends courses of action for contractors of these systems to ensure future basis for more economical and successful eHealth systems. These recommendations are mainly based on analysis of development of the existing national eHealth systems. For the complex evaluation of the costs, maximum of possible and real benefit of the individual eHealth systems the research approach adopted in this dissertation includes descriptive method and SWOT analysis, extrapolation and abstraction, comparative and factor analysis.
Measuring the effectiveness of e-government

This paper describes problems of eGovernment and proposes way to measure e-Government services. Described is the state of the Czech eGovernment in detail to selected projects and in international comparison. The description shows that the development is still ongoing, but in a much slower pace than in the electronically advanced countries. Main identified problem is inconsequent strategic management and resortism, which is constraining deployment of cross-resort projects. Based on the description of the current situation and on the international experiences, author offers specific recommendations to future development of the electronization of public administration in the Czech Republic. In response on management problems is in this paper proposed and tested methodology for measuring the effectiveness of e-Government services. This methodology is an objective procedure of measuring the effectiveness of e-services which is providing feedback for service providers. Testing of methodology evaluates three services from taxation agenda and highlights their weaknesses, including the proposal of corrective solutions.
e-Health - Electronic health care in EU countries

This thesis deals with the problems of e-Health in the Czech Republic within the European Union. The thesis is divided into five main parts. The first of them describes the definition and concept of e Health together with other selected topics. It also describes the reasons why e-health needs support. The second part is focused on the computerization of health care and describes the recommendations of the EU standards and documents for support of e-health. The third part describes the state of e-health in selected EU Member States. The fourth part is focuses on e-health in the Czech Republic. The final part suggests possible ways how to solve problems of IZIP project in the Czech Republic to be more useful. The first goal is to analyze the e-Health environment and current trends in this area in the Czech Republic. The goal is to analyze the EU initiative on data interoperability and support from European Union to member states. The third goal is to analyze the situation of e-health in at least three other EU countries (excluding the CR). All the objectives will be achieved by studying available electronic materials issued by the EU, national government agencies, private experience and consultations with professional public. The benefit of this work is the current view of the state of e-Health in the Republic and in selected EU countries, according to available resources. Another benefit is the proposal addressing some of weaknesses IZIP project in the CR, which I chose as the most important representative e-health solution in the country.
Application integration in healthcare domain - Electronic prescription in Czech Republic using HL7 standards

This thesis is concerned with electronic prescription of drugs in the Czech republic. The objective is to find a proper standard for electronic medical data exchange and use it for building a new data communication interface for the e-prescription system. This work attends to the evolution and actual situation of the national e-prescription system. The system is based on one central storage for e-prescriptions, however, currently it isn't being used. On basis of valid legislation, electronic prescription and drug dispense processes are analyzed. Furthermore, present central storage data interface is explored. Considering advantages, that are bound to utilization of electronic data exchange standards, main objective of this thesis is given, to design a new interface for transmission of e-prescriptions and drug dispense notifications. New interface shall adhere to standards. Czech standard DASTA and europian norm EN13606 were examined. After all, set of specifications HL7 (Health Level Seven) has been chosen, which suits the requirements best. Data interface design, with HL7 standards, is based on exchange of documents. Documents (prescription, dispense notification) are expressed in the Clinical Document Architecture (CDA) format, and exchanged between applications by means of the Medical Records domain messages. The structure of wrappers, forming a message, results from common HL7 Specification Infrastructure. Presented design has been inspired by Finnish solution eResepti. The design tries to transform present interface of central storage to format, which is defined by selected HL7 specifications. It can be used in both local and central e-prescription systems. It may also support the development of systems for electronic health(care) records exchange.

This diploma work analytically examines data resources and conditions for the implementation of an electronic identifier for an insured person in the Czech Republic with relation to the Europian Union. It describes the concept of a single electronic identification method and the possibilities for the utilization of different technologies and extreme situations that may arise in connection with the deficiencies of the judicial code, that would clearly work manipulation with the personal data, its security and access to it. Here compared in this work are the merits and disadvantages of the implementation or adversely the non-implementation of the electronic identifier via a cost-benetfit analysis that gives an answer to the economic question about the implementation of the project, and also describes the benefits for each group of beneficiaries.
Managing the Quality of Health in the Czech Republic

A large number of attitudes and methods of dealing with health service, or, more precisely, health, occur in theory and practice. The aim is to examine the efficiency of the implementation of the current projects, i.e. efficient application of tools of managing the quality of health. Theoretical rationales characterize understanding of conception of the quality of health especially in the sphere of health service. Moreover, this part introduces selected tools of managing the quality of health. The analytical part is focused on the institutional environment in connection with current challanges of eHealth implementation. The Central Repository of electronic prescriptions as the tool of managing the quality of health and the basis for ePrescription is one of the mentined challenges. The final part concerns with future trends in the Central Repository of electronic prescriptions and international projects, e.g. epSOS project. The EU is established on the principle of benchmarking, among other things. Possible future advances need to be considered in the context of foreign projects. For this reason, the Danish eHealth situation is included in the conclusion.
Contribution to economic and policy evaluation of implementation eHeatlh solutions into health services

This thesis is dealing with problems of eHealth specified on particular conditions on the territory of the Czech Republic in context of national economy. Positive and negative aspects of implementation of eHealth solution into health services is evaluated in theoretic part of thesis as well as dominative problems which faces development of eHealth in the Czech Republic. Fundamental plans of support eHealth in the European Union and Czech Republic are described as well. Practical part of thesis is concerned with particular projects of eHealth in the Czech republic. Evaluation of project IZIP is performed due to questionnaire inquiry among the doctors. The example of contribution of implementation eHealth solution into health services is shown on Oblastní nemocnice Mladá Boleslav, a.s.
