This diploma thesis analyses the supply of gluten-free foodstuffs in the stores in the Domažlice area, based on the demand of the gluten intolerant customers. In the theoretical part, the basic terms related to the topic are explained, i.e. business terminology, marketing research and overall issue of celiakia and gluten. The practical part examines and evaluates the width and quality of supply of gluten-free products compared to the demand. The main purpose, that is removing the deficiencies which are perceived by the respondents mentioned in the questionnaire, will be completed by proposal of specific recommendation for the Domažlice area according to the wishes of the sick.
HLA typizace v klinické praxi - využití k testování predispozic k onemocněním autoimunitního typu

Autoimmune diseases require the interplay of internal (genetic) and external (environmental) factors to develop and erupt. The essential genetic factors include HLA alleles, whose carrying is associated with a significant predisposition to the development of a specific autoimmune disease. This Master Thesis deals with the problematics of autoimmune diseases associated with HLA system. The practical part focuses on the examination of predisposing alleles related to celiac disease as one of the most widespread autoimmune disease, which many scientific teams are dealing with, but even in the Czech Republic there are not enough population studies of this disease. Thanks to data from genetic laboratory GENLABS s.r.o. in České Budějovice, ÚHKT and VFN in Prague it was possible to focus on such population study.
The role of innate immunity cells in the pathogenesis of celiac disease

Celiac disease is an autoimmune disease which occurs in susceptible individuals after ingestion of food containing gluten. Gluten and its monomeric fraction gliadin induce inflammatory damage of the small intestine by activating the immune cells that react strongly to gluten peptides. Gluten peptides have the ability to activate cells of adaptive as well as innate immune system. This work is focused on the production of interleukin (IL)-1 in antigen presenting cells stimulated with peptic gliadin digest. We found that monocytes and peripheral blood mononuclear cells (PBMC) isolated from blood of celiac patients secrete significantly more IL-1α and IL-1β than cells of healthy donors after stimulation with gliadin digest. The gliadin-induced IL-1β expression is controlled by a signaling cascade that includes MAPK kinase family molecules and transcription factor NF-κB. Moreover, we found that the adaptor proteins MyD88 and TRIF as well as Toll-like receptor (TLR) 2 and 4 play a role in the signaling cascade underlying gliadin-induced IL-1β expression by using murine bone marrow derived dendritic cells (BMDC). Quality of patients life with celiac disease

This thesis is dealing with the quality of life of patients with celiac disease. In the theoretical part, I am presenting the characteristics of celiac disease and I am describing its history briefly. For better orientation in the issue, I also joined a description of the anatomy of the small intestine and pathophysiology of celiac disease. In this thesis I am also dealing with incidence of disease, etiology and risk factors participating in the development of celiac disease, symptoms and the different forms, which are related to celiac disease, as well as potential complications and prognosis of celiac disease. An irreplaceable is the role of nurses in diagnosis and targeted screening for celiac disease, therapy and prevention, dispensarization and education of patients with celiac disease about gluten-free diet. I am also describing the problems that patients with celiac disease may encounter in everyday life. Last but not least I am focusing on the general concept of quality of life and also on the quality of life related to health and celiac disease. Celiac disease and issues of kid's eating habits in school canteens

This thesis is focused on catering for pupils with celiac disease in school canteens. The first part, the theoretical one, deals with the disease itself; it gives basic information about the disease, its history, main symptoms, anatomy of healthy digestive system, and the most common health problems occurring when suffering from celiac disease. At the end of the theoretical part diagnosis of the disease is described as well as suitable and unsuitable diet food for celiac and food legislation. The practical part is a survey focused on catering for pupils with celiac disease at selected elementary schools. The survey was conducted in two regions in Valašské Meziříčí and in Prague. In the final part of my thesis special gluten-free menus for schools are suggested. The pedagogical outcome of my thesis is a lecture on celiac disease which can be held at elementary school s to help pupils find out basic information about this disease.
Effect of celiac disease on growth and bone density.

Celiac disease is a chronic autoimmune disease of the small intestine caused by intolerance to gluten. The disease is characterized by impairment of intestinal mucosa, it is associated with malabsorption and maldigestion. The main symptoms are stomach pain, diarrhea, anemia, failure to thrive. Recently, the number of patients with atypical symptoms and a completely asymptomatic form of the disease increases. Prevalence of the disease in Europe is 1 %. The aim of this paper is to describe how celiac disease affects the status of bone tissue and what is its impact on growth in children.
Molecular genetics of celiac disease

Celiac disease is an organ-specific autoimmune disease that arises as a consequence of hypersensitivity to the grain gluten in genetically susceptible individuals. Genetic predisposition are HLA-DQ2 and HLA-DQ8 genes, which are necessary but not sufficient for the emergence of celiac disease; it is involved in approximately 40% of the inheritance. In the course of the time, other genes that might contribute to the pathogenesis of celiac disease are being discovered. Among these so-called candidate genes, which are sought on the basis of known knowledge of molecular mechanisms of innate and adaptive immune responses, are for example: MIC, TNF, CTLA-4, CD28, ICOS, MYO9B, MMP, TLR and PTPN22. Immune response triggered by gluten peptide penetration into the lamina propria leads to mucosal damage. Different gluten peptides are involved in the pathology of celiac disease in different ways, some peptides trigger an adaptive immune response, while others, such as peptide p31- 43, triggers an innate immune response.
The possibilities to influence the response of innate immune cells to gliadin

Gluten sensitive entheropathy - celiac disease is a lifelong, genetically predisposed, immunologically mediated susceptibility to dietary wheat gluten, most frequently demonstrated by small-bowel damage and malabsorption syndrome. Strict adherence to gluten-free diet is the sole rational therapy of the disease. One of the possible therapeutic strategy for the treating of celiac disease is to utilize the synthetic polymer P(HEMA-co-SS). This polymer is capable specifically bound gliadin in gastrointestinal tract and by this way to neutralize the damaging effect of this alimentary protein on mucosa of small intestine in celiac patients. The in vitro study on human PBMC and specimens of small intestinal biopsies of celiac patients in our laboratory demonstrated that putative therapeutic ability of P(HEMA-co-SS) is substantially influenced by degree of proteolytic processing of gliadin and P(HEMA- co-SS) and also by different timing of per os administration of both components in organism. Another putative adjuvant therapy of celiac disease is employing of the beneficial probiotic bacterial strains.
