Therefore it is very important to focus on its prevention and early diagnostics before the stage of health complications shows up. Objectives: The aim of this thesis was to realize and evaluate consulting activities in pharmacy focused on patients who have high risks of arterial hypertension or have been already diagnosed with it. Methodology: Data were collected in a public pharmacy in Prague from the end of April 2013 until the beginning of April 2014. The actual blood pressure measurement was preceded by obtaining of answers to selected questions through controlled interview. Acquired statements were written into the printed forms. The aim of this thesis was to carry out an research which was focused on attitudes, opinions and experiences of patients (general public) regarding generic drugs and generic substitution and to analyze extracted results. Metodology: Data were collected since February to June 2016 in a workplace practicioner in Jičín. We asked all patients who came to the doctor's office during that period. Data were collected through printing anonymous questionnaire. The questionnaire was piloted. Patients got a cover letter with questionnaire together. Respondents were categorized based on the cover letter and the questionnaire was filled out by the respondents who the survey was focused on. Analysis of drug-related problems in a health facility I. Author: Michaela Bahnerová1 Tutor: Josef Malý1 1 Department of Social and Clinical Pharmacy, Charles University in Prague, Faculty of Pharmacy in Hradec Králové Intoduction: Drug-related problems (DRPs) lower the quality of health care and present a risk factor in patient's safety. Objectives: The aim of this thesis is identification, classification and further analysis of DRPs in health care facility. Methods: Revision of pharmacotherapy was performed in health care facility in term 6. 8. - 10. 8. 2012. The pharmaceutical team which contained two pharmacists and diplomant examined health documentation of patients hospitalized in the facility. We collected information from personal, allergic, family, and labor-social anamnesis of patients. This data were noted into electronical database and after revision were DRPs classified and evaluated. The results of DRPs identification were evaluated by frequency analysis. In ten randomly selected patients was described DRPs resolution. Results: In total we noticed medical therapy in 175 patients (61 % women). Average patient's age was 58 years. 482 DRPs were identified. Of the patients, 84 % had DRPs and an average of 2,75 DRPs were recorded per patient. Methods: Data were collected via questionnaire survey in 2012 and 2013 in three districts. The survey was conducted using two questionnaires, which were filled at pharmacies with possible help of trained interviewer. Experience shows that a prerequisite for the rational use of both instruments is sufficient awareness among both health professionals and patients. Objective: The aim of this study was to analyze the opinions, knowledge and experience of patients regarding generic drugs and generic substitution. Methods: Data were collected between 1 March 2015 and 31 March 2015 in a workplace practitioner in Prague. It was an observational survey. We contacted all patients who visited a doctor during that period. Patients had to agree to their inclusion in the investigation and also meet "inclusion criteria" (e.g., at least 18 years old, literate in Czech, long-term use at least one medicine). Anna Králová Thesis Supervisor: PharmDr. Martin Doseděl, Ph.D Charles University in Prague, Faculty of Pharmacy in Hradec Králové, Department of Social and Clinical Pharmacy BACKGROUND A drug-related problem (DRP) is an event or circumstance involving drug therapy that actually or potentially interferes with desired health outcomes. AIM The aim of this thesis is to analyse and evaluate the incidence of drug-related problems in a long-term care facility. METHODS This research was conducted in the long-term care facility of the Military University Hospital in Prague between March and August 2014. Data were gathered from medical records and healthcare workers. In particular, the patient case history, laboratory and physical parameters, and drug therapy data were collected. The modified PCNE-DRP Classification (Pharmaceutical Care Network Europe), version 5.01, was used to classify the detected DRPs. Recommendations to eliminate DRPs have been suggested by a pharmacist and presented orally during ward rounds. The results of these recommendations have then been recorded.
