Occupational initial examination from the perspective of International Classification of Functioning. Disability and Health

The focus of this Bachelor's dissertation is International Classification of Functioning, Disability and Health (ICF) and its applicability to occupational therapy initial examination. This dissertation is divided into sections: theoretical and practical. Basic terminology as well as the ICF framework, goals, philosophy, development and the overview of its components can be found in theoretical section. Moreover, it describes the links between rehabilitation, occupational therapy and the ICF. The end of the theoretical part gives information about occupational therapy examination. Practical section explores possible use of the ICF in occupational therapy initial examination. The main goal of practical part is to create an occupational therapy initial examination form based on the ICF and to apply it on patients. This form ought to be practically used by The Department of Rehabilitation Medicine, 1st Faculty of Medicine, Charles University in Prague and General University Hospital in Prague.
Professional faster care abroad

Bachelar thesis is devoted to the topic of professional foster care abroad. The target of the thesis was to show the way of the functioning this form of the substitute family care in two different countries. The first part describes the characteristics psychological aspects of the foster care. Another part talks about the professional foster care in Slovakia. After separation of Czechoslovakia, the system of the substitute family care in Slovakia was developing differently than in the Czech Republic. There is a literature dealing with these issues of the professional foster care in Slovakia so I could well study it and wrote complexly about this issue. There is also a part in my thesis about the civil association "Návrat", which supports the comeback of abandoned children in to the family. The important part of the process of comeback of the abandoned children in to the family is the social work with dysfunctional families. The final part of the thesis informs about the professional foster care in Norway. I was studying in Norway at the Erasmus program so I had the great opportunity to get the information about the system of the professional foster care from the people working in such a organizations in Oslo. Comparison of need for cooperation in a medical team at a unit of intensive care and standard department

Téma: Porovnání potřeby sounáležitosti ve zdravotnickém týmu na jednotkách intenzivní péče a standardních odděleních. Cílem diplomové práce je zjistit míru uspokojení potřeby sounáležitosti ve zdravotnickém týmu na jednotkách intenzivní péče a standardních odděleních. Ve své teoretické části se zabývám podrobným rozborem potřeby sounáležitosti. Charakterizuji problematiku lidských potřeb a jejich důležitost při působení v pracovním kolektivu na jednotkách intenzivní péče a standardních odděleních. Poukazuji na příčiny a projevy frustrace této potřeby. Zabývám se fluktuací zdravotnických pracovníků a pohlížím na narušení potřeby sounáležitosti, jako na jeden z důvodu proč sestry mění pracovní místa. Praktická část práce je zpracována formou kvantitativního výzkumu. Metodou dotazníkového šetření jsem získala data od 142 respondentů (všeobecných sester, sanitářů, fyzioterapeutů ve fakultních a krajských nemocnicích). V dotazníku jsem sledovala tyto oblasti: základní faktografické údaje, frustraci potřeby sounáležitosti a její projevy, důvody fluktuace, motivaci v pracovním kolektivu. Zpracováním dat jsem došla k závěru, že sestry znají význam výrazu potřeba sounáležitosti. Subjektivně více vnímají frustraci sounáležitosti zdravotničtí pracovníci ze standardních oddělení než zdravotníci z jednotek...
Social Worker as a Member of the Multidisciplinary Team in the Therapeutic Community

The present thesis is preliminary focused on the mapping and description of the scope of work of a social worker and the definition of his/her position within a multidisciplinary team of a therapeutic community. A description of the cooperation between a social worker and other members of multidisciplinary team within a therapeutic community shall be understood as a partial goal of the thesis. The bachelor thesis is divided into the theoretical part and the practical part. The theoretical part contains definitions of basic terminology hereof: a social worker, a therapeutic community and a multidisciplinary team. This way, the comprehensive theoretical frame of the issue in question has been elaborated, which establishes a starting point for the following practical part of the thesis. In accordance with the main objective of the bachelor thesis the practical part contains a definition related to the following three survey questions: 1) What is the core role of a social worker within a therapeutic community? 2) How the other members of a therapeutic community perceive the importance of social work? 3) What is the position of a social worker within multidisciplinary team of a therapeutic community? In order to perform the survey, the qualitative research strategy was applied, as well as interviewing method and a technique of sub-structured interviews. The sample group has been formed by five members of a multidisciplinary team of one selected therapeutic community. The sourced data had to be analysed using the method of cluster analysis. The survey has revealed the work of a social worker within a therapeutic community lies in particular in social agenda addressing. These are mainly the cases when a client is provided with help in the field of social contributions and benefits and also in the field of debts. A social worker shall also instruct and teach a client what are the right approaches to his/her own matters and shall help him/her to come back to ordinary life successfully. Based on the survey, it has appeared that the other team members perceive the social work as day-to-day and integral part of activities within a therapeutic community, which is then being reflected also in a therapeutic process. The social work performed at the beginning of a treatment is understood by all the workers as the most important activity. It was also confirmed that a social worker represent an integral part of multidisciplinary team of a therapeutic community. The other team members appreciate his/her contribution especially to the field of social work expert knowledge which they do not have. A social worker's role within a team is to work as a coordinator and lecturer who helps to bring an explanation related to the social work towards all the other members of a team. The contribution of the present bachelor thesis lies in the increase of up-to-now awareness related to the activities and roles of a social work within a therapeutic community. The present thesis may also contribute to extended awareness of expert and general public. The bachelor thesis may be also useful as a background document for the elaboration of a diploma thesis.
Home hospice care from the perspective of family members of the incurably ill.

The goal of my thesis I have chosen is to identify the way hospice care is perceived in the Telč region by family members of terminally ill patients. I based my thesis on available specialised literature, current legislation and internet sources dealing with home hospice care. In the first theoretical part, I deal with the definition of hospice care trying to explain the term of general and specialised care that have a common objective, namely dignified and tranquil dying. The second section deals with family in the context of palliative care that plays the key role in the patient care. If the family decides to care for the incurable patient at home, it is vital to support it. Moreover, in this section I define needs of family members summarised in three core topics. Including the care for the patient family that I included to the hospice care as not only the patient suffers from the illness but all his/her family does. That means that each family member needs palliative care. The end of this section deals with mourning of the survivors. The core point is not leaving them alone in their mourning, to have them accompanied by someone in their grief. The third chapter focuses on the role and position of the patient. It changes when a seriously ill human becomes dependent on third party´s help losing his/her capacity to fulfil his/her professional and family role. This enormously affects his/her physical and emotional condition. Moreover, I describe needs on terminally ill people that change when the patient approaches his/her death depending on his/her family and social situation. It changes also during his/her adaptation to adverse diagnoses and forecast and emergence of difficulties and complications. I describe four groups of needs: biological, psychological, social and spiritual. The fourth chapter characterises home hospice care that is described as specialised palliative care provided in the home environment amidst patient´s family and friends. I describe the history of Czech hospice care stating that before the WWII it was quite normal to care for terminally ill patients at home till the end. This trend emerges again in the 90s. I define objectives of the home hospice care and related activities. The fourth chapter includes the description of a multi-disciplinary team and funding of home hospice care. In the fifth chapter, I characterise the home care organisation that try to expand the range of services by including the home palliative care and also deal with the home care history, objectives, delimitation of the scope of activities, agency human resources management and home funding methods. In the sixth chapter, I represent the organisation Sdílení, o.p.s. Telč, services provided by it and basic duties according to the Act No.108/2006 Coll. as well as the mission and objectives of this organisation concentrating on help and support of seriously ill people and their family members/friends in such difficult life situation. The key objective of Sdílení is to preserve maximum possible self-sufficiency and dignity of seriously ill people and support of families that are able to find the courage and force to accompany a terminally ill family member to the end of his/her life. In the practical part of my thesis, I analyse data collected by means of qualitative research based on an inquiry in the form of semi-standardised and open-code processing. Who was included to the basic file were family members of terminally ill patients who were divided to two parts. The first group consists of 3 respondents who were entrusted to the care of the home hospice Sdílení. The second group are 3 respondents entrusted to the Home Care hospice. In the practical part, I describe the process of collection of data, record results of the interview process by the open-code method.
Multidisciplinary Cooperation and the Role of a Nurse in Patient Care Affected by Blast Syndrome

Blast injury is a very serious lesion caused by the shock wave resulting from an explosion. Further secondary injuries can arise from burying under debris, pieces of shards from the explosive cover, at burning, poisonous gases created by chemical reactions during detonation. The injuries often happen to be fatal and the percentage of people who survive it depends on the fact how far from the epicentre they were situated. If the individual is very close, there is almost no chance to survive, if, however, the individual is situated further from the epicentre, a few tens of meters, the chance is higher. It is so called mass disaster, which includes more injured people at an explosion. Therefore it is necessary for nurses and the hospital staff to know what the procedures are at this incident and how to take medical and nursing care of higher number of such patients suffering from blast injury. The care itself is extremely demanding, since the injured are often in critical condition. The aim of this diploma thesis, the topic of which is multidisciplinary cooperation and the role of a nurse when taking care of the patients suffering from blast injury, was to find out the information based on the latest Czech and foreign literature and create a compact overview of the knowledge of this issue, for it is necessary to have this knowledge to carry out nursing care of such patients. The second aim of this diploma thesis was to map the role of a nurse in details when taking care of a patient suffering from blast injury. This aim was stated for the reason of the demanding character and complexity of nursing care at urgent reception and anaesthesiology and resuscitation unit because the nurses at these units are placed demands on high qualification and requirements. This diploma thesis was purely elaborated as theoretical thesis based on Czech and foreign sources. The given pieces of information were obtained from a great deal of professional and scientific publications, monographs, professional journals and internet sources for each chapter. The first part of the thesis was dedicated to the description of the given issue, pre-hospital care and classifying algorithms that are used at mass health losses in such way so that the injured people have a higher chance to survive. The crisis management at hospitals, the reception of the injured, the care at the urgent unit and finally the follow-up care at the anaesthesiology and resuscitation unit are worked out in the following part. Thereafter the theoretical part focuses on nursing care at the critical care department where the nursing care of a patient suffering from blast injury is described as well as the duties of a nurse. The needed competences of a nurse working at the urgent unit, anaesthesiology and resuscitation unit are also defined in this chapter. According to the valid Czech legislation, only specially trained nurses for intensive care should take care of the patients with blast injury because the extent of their competence for intensive care is many times higher than general nurses have. The last chapter deals with team and multidisciplinary cooperation among the doctors and hospital staff as well as the cooperation concerning the pre-hospital care of the emergency services at emergency incident, for their active and quality communication are the keys for good and well done work with the least consequences.
A multidisciplinary team in the care of woman with the breast cancer

I have chosen the following topic for my thesis "The multidisciplinary team in the care for women suffering from breast cancer" not only due to the topicality of the issue, but mainly because I work in a surgery ward where I take care of women suffering from this disease. It is a sad fact that during my tenure in this department, the number of women with this disease has been increasing and their age has been dropping. Modern diagnostic methods determine the diagnosis and a multidisciplinary team suggests the most suitable and efficient treatment. The theoretical part of the thesis deals with the definition of the term 'multidisciplinary team', with its tasks and it also specifies which members of the medical staff belong to the team and what their role is. The theoretical part of the thesis further describes breast tumors, methods of their identification and their treatment, complex nursing care for women suffering from breast cancer and last but not least the importance of prevention. Two objectives were determined within the thesis. The first one was to discover whether a multidisciplinary team is involved in the care for women suffering from oncological breast disease. The other objective was to discover what consequences does the oncological breast disease have on women´s lives in connection with nursing care. Based on these objectives three research questions were identified: How does the multidisciplinary team cooperate in the care for women suffering from breast cancer? What roles does a nurse have within a multidisciplinary team dealing with breast cancer from patients' point of view? What is the most frequent impact of breast cancer on women´s life from the point of view of patients? The thesis was made as a qualitative research. For the purpose of obtaining data, the method of a semi-structured interview was applied. Interviews were recorded in a voice recorder and subsequently converted into writing. The interviews form a significant research base from which categorization and sub-categorization fields are created. The research group comprised eight women in whom breast cancer had been diagnosed and who were hospitalized in the surgery ward in February 2014.
Social Worker as a Member of the Multidisciplinary Team providing Zootherapy

The goal of this thesis was to map the possibilities of a social worker´s participation in multidisciplinary zootherapeutics. I was interested in whether the social workers are able to define the multidisciplinary team itself and whether they are able to choose which coworkers in their facilities are placed in such teams. Multidisciplinary team is hugely discussed topic in all fields beginning with health services. The thesis is divided into two parts into the theoretical part and the part dealing with the research. Theoretical part is then divided into next three subparts. In the first one I escribed basic definitions of social work, role and content of social worker´s work according to law n. 108/2006 about social work and specifics of social work with seniors. I devoted the second subpart to the definition and different kinds of zoothereapeutics, to focus groups and to its benefits. In the last subpart I did a summary about all workers who belong into the multidisciplinary team and their activities according to chosen laws. In the research part I tried to map the social worker´s involvement in providing zootherapeutics. I stated two main research questions. In the thesis I used approach based on quality, the method of asking questions and the technique of half structuralized interview. The interview contained 25 opened questions which were supplied by subquestions. The research showed that in the chosen facilities the social worker figures rather as a coordinator of zootherapeutics, communicates with zootherapist, organizes terms and chooses subjects that are suitable for the zootherapeutics. I also found out that the most often used type of zootherapeutics is a group form of dog therapy. The social worker participates during the therapy only minimally because it is mostly organized by the therapist himself / herself. The thesis may be used as a feedback for social facilities using zootherapeutics and also as a source for lectures at the university.
Introduction of home hospice care from the perspective of social work

The work is focused on introduction of home hospice care from social work´s point of view. The thereoretical part deals with characteristic of hospice, hospice care, then home hospice care and valid legislation in a more detailed way. Emphasis is put on the social work in hospice care and the position of a social worker within hospice care. Research follows the theoretical part, research has the goal to find out if home hospice care with a social worker succeeds in phasing to a large extend. Research file consists of four facilities of home hospice care in South Bohemian Region. The research concerns Home hospice Jordán in Tábor, Saint Kleofáš´s Home hospice care in Třeboň, Home hospice Athelas in Písek and Saint Jan N.Neumann´s Hospice in Prachatice.
