Clinical findings and diagnosis of vestibular lesions in patiens with vestibular schwannomem

Summary: The aim of our study was diagnosis and rehabilitation of vestibular loss in patients with vestibular schwannoma. In the first part we focused on analysis of a group of patients before resection of vestibular schwannoma, mainly on evaluation of gain of posturography and on optimalisation of diagnostic algorithm of vestibular pathology. Throughout the second experiment, we studied if rehabilitation of postural gait with visual biofeedback will speed up vestibular compensation in patients after resection of vestibular schwannoma. The group consisted of 44 patients, who underwent surgical removal of vestibular schwannoma. Before surgery each patient underwent clinical vestibular examination, electronystagmographic recordings and posturography. In the second part of the study 17 patients from previous group were chosen. These patients underwent rehabilitation and rehabilitation with biofeedback. For evaluation was used independent samples T-test and cross-tabulation. A great number of variables were reduced by factor analysis. For statistical analysis of the group with rehabilitation was used nonparametric Wilcoxon signed rank test. Effect of deep foot massage on postural stability in patients with spinocerebellar ataxia

Introduction: Spinocerebellar ataxia (SCA) is currently defined as a group of diseases with progressive cerebellar syndrome, which manifests distinctively by ataxic stance and gait. In patients with SCA, deterioration of postural control occurs due to changes of pathophysiological degenerative nervous system. Control of postural stability is significantly influenced by somatosensory afferentation of soft tissue of foot. The work objective is to determine the effect of deep foot massage on postural stability in the group of patients and to compare obtained results with experiments without deep foot massage. Methods: Seven patients of the Clinic Rehabilitation at the Faculty Hospital, Prague Motol (average age 51.9 ± 13.9 years, 5 men, and 2 women) with SCA and clinical signs of cerebellar ataxia were participated in this study. All patients were examined using posturography before and after application of deep foot massage. Measurements were conducted on a firm surface, foam pad with open and closed eyes. Length of COP trajectory, COP area and COP velocity were measured. Options of using physiotherapy at patients with amputation of their lower limb

This bachelor deals with the possibilities of the use of physiotherapy at patients with amputation of the leg. The loss of the lower limb has a major influence on change in postural stability. Despite the prosthetic equipment patient has impaired proprioception, which is reflected to the pathological walk with compensatory mechanism. We are trying to return a person to normal life with minimal disabilities using specific physiotherapy methods with adequate training and stability. In the theoretical part there is a summary description of amputation and its influence on postural stability, further I deal with protetics options, where I am closely describing the use of functional and posturograf tests. In the end I deal with physiotherapeutics prosthetic equipment and methods applied in practice. The aim of the practical part is to assess the changes in postural stability with the use of the instruments with a biological feedback and to map out the qualitative and quantitative parameters of walking of patients with lower limb amputations. For the practical part have been selected qualitative research, which includes case reports of two patients with transfemoral amputations and one transtibial of the lower limb. Both input and output examination includes a complete analysis of kinesiologic, functional tests with posturography examination of postural stability. Each patient attended 10 individual therapies to increase the stability. The results were compared between input and output of tests. All patient's postural stability has been positively affected with a slight improvement in posture. Despite the regular therapy we didn't suceed to change the pathological components. This state can be caused by the prothesis, which was by all patients described as unsatisfing. Bachelor thesis may be used by students of physiotherapy for extension of their knowledge, or they can apply the knowledge during their practice. It can also be used as educational material for patients who are interested in the issue.
Using biofeedback for patient with non-specific low back pain

The Diploma thesis titled "Using biofeedback for patients with non-specific low back pain" in theoretic part gives basic information about motor learning, biofeedback, low back pain and postural stability. In the research part, it deals with influence of postural stability- focused therapy (performed on Nintendo Wii Fit Plus device) on low back pain and postural stability. The work also deals with influence of added feedback on reduction low back pain, postural stability and realizing intrinsic feedback. We demonstrate that this therapy reduced low back pain but has no significant influence on postural stability. We also demonstrate that added feedback has positive effect on persistence of pain and affective part of the Short Form of McGill Pain Questionary, but it does not influence postural stability. On the contrary, absence of added feedback significantly influenced the postural stability. The added feedback does not influence awareness intrinsic feedback.
The influence of music therapy on stability and reactibility of vertebrogenic patients as viewed by a physiotherapist Objectivisation by the posturography

The therapy of vertebral issues is nowadays (at the present time) an inherent part of the physiotherapist's work. The main aim of this study is to find out, if we are able to change stability and reactibility of patients with vertebral issues by using selected elements of musical therapy. As an ulterior aim I chose to evaluate respondent's subjective feelings about changes in their vertebral issues and to compare advantages and disadvantages of integrating musical therapy into therapeutical unit. In the theoretical part I am writing about musical therapy as a branch of science which is trying to find its place in our health care. I am also writing about postural motor skills, posturography as an objectiv evaluating method and about vertebral issues with its etiology. An important chapter in this theoretical part is about psychosomatics, which often causes back pain. For practical part of the theses I prepared 10 musical therapy sessions. Main aim of these sessions was to influent musculosceletal system, reactibility and stability of respondents. To evaluate objectively the results the posturography was used. Materials used to fulfill ulterior aims were taken by a questionnaire.

The Synapsys Posturography System and Nintendo Wii utilization in therapy of patients after brain injury. ABSTRACT The main theme of this thesis is the use of modern gaming console, the Nintendo Wii in therapy of patients after a brain injury. The work focuses mainly on training of postural stability platform with the Wii FitBalance Board. The author objectifies the results of therapy using posturography with the new equipment Synapsys Posturography System. The theoretical part describes the postural motor, its management, ontogenesis, but also its failure and the most common causes of these disorders. The author pays particular attention to the work mechanisms of injury and damage to the brain, which has a direct impact on the perception of body position and movement. A separate chapter summarizes the general knowledge about stroke and its effects and explains main problems of instrumentation in physiotherapy. Affect of standing frames on measuring postural stability

This semestral thesis is focused on postural stability. The aim is to introduce standing frame Balance Trainer and haptic floor Wii Balance Board. The theoretical part describes the postural functions, posturography and subsequently standing frame Balance Trainer and stabilometric platform Wii Balance Board. It is also designed the measurement protocol for affect of standing frames on measuring postural stability on a group of subjects and the measurement is realized. From the collected data are calculated parameters which are statistically processed and the affect of standing frames on measuring postural stability is evaluated.
Methods for the evaluation of postural stability

This study deals with methods for evaluation of postural stability. The main problem of posturography is lack of standardization in measurement and evaluation. There are presented some parameters affecting postural stability that are often taken into account. Those are parameters that might be changed while measuring, for example standing position, sampling duration or visual control. And others that are altered when evaluating is taking place, variability of those parameters, is much greater. Measurement protocol taking into account the most promising parameters was put together based on literary search. Changes in COP of ten subjects were measured with Wii Balance Board, according to the proposed measurement protocol. The data acquired from a particular subject were used to count values of parameters used in evaluation. After that the values were statistically analyzed. To compare each parameter, trajectory with good results according to other literature was chosen to pose as a reference. Final result is comparison firstly between importance of changes in parameters of measuring and secondly between success of detection of those changes of separate parameters.
The impact of non - functional sole of foot on the posture of people with non - sedentary kind of employment

This thesis focuses on the non-functional sole of the foot and its influence on people with sedentary job's body posture. As a non-functional sole of the foot is considered, in the research part of this thesis, the deviation of the standard that can be determined by means of podoscope which displays the sole of foot, or by functional tests. The first aim of this work is to describe the posture disorder of people working in an environment where they burden the sole of the foot in the long term. The second aim consists in the suggesting and applying a physiotherapeutic plan for particular people with a dysfunctional foot sole and not having sedentary job. The target of the research question was to define a particular pattern of the posture disorder as a consequence of the dysfunction of the foot sole. In the theoretical part of the thesis the reader is informed about the development and anatomy of the foot, its function that can be focused on from the point of view of a foot as such as well as the function of the foot as an impact on the posture when sitting, standing and walking. Another part deals with the posture and its modifications in both, the statistics and dynamics. In relation to the research part of this thesis, there are incorporated chapters about the possibilities for the examination of the foot from the point of inspection, palpation, active and passive movements and, last but not least, the functional examination including tests of the sole of the foot and instrumental examination of the posture - posturography. Because of the target of the description of the consequences of the non-functional foot sole, there is a description of the function muscle chains that connect the sole of the foot, and the posture, included. A short part of the theory is dedicated to sanitary footwear. For the research part of the work, three women aged 21 22 with non-sedentary jobs, working eight hours per day as a minimum, with subjective difficulties with the pain of lower limbs and back in the lumbar area, were chosen. The collection of data took place in the form of processing three case interpretations with the use of the method of an interview, comparison and a kinesiological analysis, including the use of objective methods of the examination with podoscope and posturograph. The content of the kinesiological analysis was focused on a chosen test that were after the end of the therapy registered in a chart depicting the pattern of the posture disorder. The examination by posturograph includes projection of the focal point of probands to the supportive base and the measure of their motoric abilities. There was a physiotherapeutical plan conducted and applied in the form of exercise during the therapy of probands. The therapy took place for nine weeks, the brobands exercised every two weeks under the supervision of a therapist with the purpose of eliminating potential mistakes. The exercise was extended or corrected according to the actual condition of probands. From the results of the theoretical part it is obvious that it is possible to create the pattern of the posture disorder in case of the people with non-sedentary working place.
Analysis of stabilometric signals in frequency domain

This work deals with the metods frequency and time frequency analysis of stabilometric signal. In the introroduction is described theory about posturography and posturographic measurment. The work contains describtion of stabilometric parametrs in time domain (1D and 2D parametrs) and in frequency domain. The aim is create review of basic metods used to processing and preprocessing of stabilometric signals and comparing this methods . In work is realized ferquency analysis used Frourier transfrmation and Burg method and time-frequency analysis used Short time Frourier transformation and Wavelet transformation. One part of program is aimed on comparison of this methods.
