Modern therapy of chondral defects

The thesis describes the application of cultured bone marrow stem cells in the therapy of focal chondral defect of the knee joint. In the experimental part of the work, the goal was to quantitatively and qualitatively compare two sampling sites of bone marrow monocytic aspirates, the iliac crest bone and the proximal tibia, in order to determine a suitable cell source for advanced cell therapy. The sample analysis showed that the amount of monocytic cells and the yield of stem cells from the aspirate obtained were significantly higher in the bone marrow from the iliac crest. We did not find significant qualitative differences between the two sources of stem cells. In the clinical part of the work, I present a description of the surgical procedure and the results of a 1-year follow-up of patients after the implantation of cultured stem cells from the bone marrow, under the name BiCure® orthoMSCp (Bioinova, Prague, Czech Republic), fixed on a commercially available 3D scaffold Chondrotissue® (BioTissue AG, Geneva, Switzerland) using coagulated autologous platelet-rich plasma. The effect of the deep stabilization system on the prevention of knee injuries in soccer

This bachelor's thesis examines the deep spinal stabilization system and its effect on knee stability of soccer players for future injury prevention. Joint instability is cited as a major risk factor for non-contact knee injuries. Therefore, this thesis explores the possibility of improving its stability by improving the activation of the deep stabilization system of the spine. Soccer is a collective sport with a large player base and with this comes a high number of injuries. Among the most common of these are knee injuries. The movements performed in football require precise coordination of the activity of the muscles of the lower limbs. These movements are discussed in this thesis, along with the most common football-specific injury patterns. The functional aspects of the deep stabilization system of the spine are also presented. This is a set of muscles of the trunk, diaphragm and pelvic floor that, when properly coordinated, provide spinal bracing during posturally demanding positions. Its proper development begins in early childhood and its involvement depends, among other things, on the correct positioning of the segments. Furthermore, familiarization with methods of injury treatment and subsequent rehabilitation. Summary: This bachelor's thesis is completely theoretical and is divided into eight parts. Title: Case study of physiotherapy treatment of a patient after reconstruction of the Anterior Cruciate Ligament. Thesis Aim: The aim of this thesis is to understand the theoretical background of anterior cruciate ligament ruptures, surgery and rehabilitation. Further, it aims to analyse the effect of therapeutical procedures that were applied to the patient. Clinical Findings: This study evaluated the state of a 34 year old professional male football player after reconstruction of the anterior cruciate ligament on the right knee. The examination showed findings of reduced range of motion, reduced mobility of joints, differences in the conditions of muscles and soft tissue and decreased stability in the right lower extremity. Methods: The methods used for this study were based on the knowledge gained and the literature that was provided to me by the Faculty of Physical Education and Sport, Charles University in Prague. Result: The patient that I have worked with progressed very well after the 10 therapy sessions. Progress was seen in terms of his range of motion, muscle strength and stability in the right lower extremity. The aim is also to document the physiotherapy care of the patient after knee atroscopy for anterior cruciate ligament lesion, cartilage defect and plica syndrome. Methods: In the general part of the thesis I summarize the theoretical knowledge concerning the knee joint, including anatomy and biomechanics, mechanism of injury and treatment of some injuries of this joint. iv Abstract Author: Adam Viktora Supervisor: PhDr. Lenka Žáková, Ph.D. Title of the thesis: Case study of physiotherapy treatment of a patient after Total Knee Replacement Objectives: The goal of this thesis is to gather and understand the theoretical knowledge of the knee joint, osteoarthritis, total knee replacement surgery and its rehabilitation, and then evaluate the therapeutic sessions that were applied. Clinical findings: This study evaluated the condition of a 73-year-old patient after total knee replacement of the right knee. The examination showed a reduced range of motion, reduced mobility of joints, muscle imbalances and differences in conditions of soft tissues. Methods: All methods used for this study were based on the knowledge learned and the literature provided by the Faculty of Physical Education and Sports, Charles University. Results: Following the eight therapeutic sessions, the patient had progressed well overall. Progress was seen in terms of pain, range of motion, muscle imbalances and condition of soft tissues. The second part of the thesis is written based on a physiotherapeutic methods used during the rehabilitation process including initial kinesiological examination, day to day therapy, and final kinesiological examination, and evaluation of the therapy process to understand the therapy effect on patient. Methods: All methods used were noninvasive. The information in the theoretical and practical part of the thesis are based on knowledge gained in Faculty of Physical Education and Sports in Charles University. Possibilities of physiotherapy after knee arthroscopy

This bachelors thesis talks about the possibilities of physiotherapy after knee arthroscopy. The thesis is divided into many parts. The first theoretical part summarises important information about the anatomy, kinesiology and biomechanics of the knee joint. Then the mechanisms of knee injuries are described with their following arthroscopy treatment. The theoretical part ends with the list of possibilities of physiotherapy after knee arthroscopy where specific methods and concepts are explained. The research questions and goals of this thesis follow in next chapter. The first goal is to propose treatment and rehabilitation plan after knee arthroscopy. The second and the main goal is to focus on the problem of knee instability connected with this surgery. The third goal is completed by creating an information brochure with adequate exercises after knee arthroscopy. For my thesis I picked qualitative research. The research group was formed by 3 women between 35 and 58 years which underwent knee arthroscopy surgery. The reseach was done by regular meetings for 2 months. The methods of obtaining data were entrance and output kinesiology analysis. The results are presented in the form of case reports. The case reports contain the progress of therapy, short-term and long-term rehabilitation plan and the summary of all therapies. The final chapter contains evaluation of the research in the form of discussion where I list the strong and weak sides of this research. This thesis can be used in physiotherapy, by the public or patients after knee arthroscopy.
Pressure pain threshold evaluation in relation to the knee joint position - pilot study

The diploma thesis deals with the relationship between the level of stress of soft tissues and their pressure pain sensitivity. It aims to find out whether the level of tension in soft tissues, given by the position of neighbouring joints, affects the pressure pain threshold. The thesis assumptions were verified on a model situation with the sensitivity of the tissues of the ventral and dorsal sides of the thigh depending on the angular adjustment of the knee joint flexion. The theoretical part deals with the findings about biomechanical properties of soft tissues in relation to mechanical load and in the sense of tensile and compressive forces. It describes the mechanisms of tissue nociceptor activation and afferent signal transmission for processing by neurvous system. It distinguishes between the concepts of nociception and pain and deals with methods of their quantification, especially pressure algometry. A group of 30 (15 men and 15 women) healthy probands of younger adult age participated in the research. The values of the pressure pain threshold were measured for each person with an algometer from the ventral and dorsal side of the thigh with angular settings of 0ř, 45ř and 90ř flexion of the knee joint, with a constant position of the hip joint.
