Possibilities of physiotherapy in patients with hypermobility

This bachelor´s thesis deals with the issue of joint hypermobility. Hypermobility does not constitute a disease in the true sense, but a certain level of ligament quality. It is manifested by increased range of movements in joints over the physiological limit. Above all, it should be noted that especially the ligament fulfills the strengthening and fixing function of the musculoskeletal system. In cases of hypermobility there is increased laxity of ligaments and looseness of joint capsules, moreover, it is associated with low muscle tone. As a consequence of the insufficiency of ligament, hypermobility is accompanied with joint instability, which leads to the development of functional disorders. Therefore, physiotherapy constitutes a crucial role in the compensation of the hypermobility signs in order to avoid that functional disorders grow into structural ones. The aim of this work was mainly to explain the problems of hypermobility and to propose an appropriate physiotherapeutical treatment of hypermobile individuals. In the theoretical part of this work, I define the musculoskeletal system focused on the description of ligament and joints. It is followed by the explanation of functional disorders and their relationship with structural diseases. Then, I continue with a characteristics of hypermobility and the associated joint hypermobility syndrome, which I subsequently elaborate with the description of epidemiology, etiopathogenesis and clinical signs. Later on, I present the classification of hypermobility and diagnostic tests with a broader description of the most commonly used ones in the Czech Republic and abroad. In the final part, I come to the differential diagnosis and the characteristics of the treatment of hypermobile individuals. In the practical part of my thesis, I did a qualitative research. The subjects of my research were two adult men with confirmed hypermobility. I took the anamnesis of the two probands and performed an initital check-up including the examination of hypermobility according to Janda and also using goniometry. Based on the examination, I proposed a therapy under my leadership for the period of 2-3 months with 60 minutes sessions once a week. Probands were recommended to do exercise for about 15-20 minutes every day. For one of the probands we planned a total of 10 sessions. After the completion of the therapy, we conducted an output assessment and made a conclusion that the overall physical condition had improved, but due to the length of the therapy we failed to influence the hypermobility itself because it was impossible to influence the quality of the ligament in such a short time. Compensation of hypermobility is a question of a very long-term exercise. Unfortunately, we had to finish the therapy of the second proband before our seventh session because of sudden health complications. That was the reason why we did not reach any positive results. However, this case would also require a long period of time to compensate the hypermobility.

Integrated multichannel audio amplifier for home cinema

This paper describes complete design of amplifier for home cinema system. Described amplifier has 2 digital inputs (TOSLINK and S/PDIF) and single analog input. With this inputs it is possible to connect amplifier to a source of surround audio 5+1 signal. Paper further discuss design of correction circuits, preamplifier, power amplifier and power source. The whole is controlled by 8-bit microprocessor and has a graphics output implemented through LCD display. The goal of this paper is to design and build high quality amplifier.

Switching driver for high power LEDs with higher efficiency

Problems in swithing converter and power losses in this circuit describe these master’s theses. Power losses have the primar influence on the total effectivity circuit. The first chapter deal swith principle of basic parts switching converter (buck, boost, buck-boost). The power losses on main elements used in switching circuit (diode, transistor) are explained in the next part this thesis. Theory of synchronous rectifiction for reduction of losses. A choice of two integrated circuit useful for future design of LED driver working in buck-boost mode is on the end of the theoretical part. The practical part is fosud on the design of circuits (with integrated circuits chose in the theoretical part) including schematic diagram, calculation of parts and PCB layout. The build samples are evaluated by set of measurements in the field of effeciency, temperatures of components, stability output LED current, EMC and soon. Finally the particular parameters of both tested circuits are compared

Odhad potenciálneho produktu a produkčnej medzery v Českej republike a na Slovensku

This thesis has two goals. The first goal is to estimate potential output and output gap in the Czech Republic and the Slovak Republic by using Hodrick-Prescott filter. The second goal deals with dependence of stock markets, repre-sented by stock indices, and real output, represented by Gross Domestic Product in the Czech Republic and the Slovak Republic by using correlation analysis and Granger causality test.

Corporate Reporting Solution Implementation, Basic Principles and Solutions in Cognos 8 BI

This thesis is focused on problems of reporting. Its goal is to provide a basic overview of concepts and technologies associated with reporting and demonstrate its unusual use on real-world example of a betting agency. The thesis is divided into two parts - theoretical and practical. The theoretical part is focused primarily on getting insight into the subject. Concepts, basic principles and technologies used for reporting are defined here. In this part the optimal reporting structure is described and also an analysis of current state of the market of commercial reporting tools is prepared. The real-world problem of reporting is described in the practical part. Two exemplary reports are the output. The first analyzes the bet ticket and the other helps determine the courses of bets. On these practical examples there is shown the development consecution of the report from its business submission, business structure definition, ETL arrangements in IBM InfoSphere DataStage, development of the report using IBM Cognos 8 BI toolkit, till the analysis of the final report.

Potential output. Econometric application for Czech Republic.

I summarize different methods of potential output and output gap estimation including advantages and disadvantages in this thesis. I also applied two published models on real data for Czech Republic. Concerned models are Hodrick-Prescott filter and so called Production Approach. Both approaches are simultaneously used by ČNB. This thesis offers comparison between HP filter and production approach and comparison of Czech, Austrian and common EU-15 potential output and output gap. Potential output of Austria and EU-15 was obtained from OECD database. Comparison result refers to very similar progress of estimate obtained by univariate and multivariate method. It also shows different trend behavior of domestic economy against more developed EU countries, which is starting to be similar at the end of observed period.

Genetic polymorphism of selected loci in relationship to technological traits of meat

Within the framework of this diploma, a genotypization of 162 beef cattle of the ČESTR commercial breed from was performed in order to analyze the potential influence of gene coding calpain I (CAPNI) on the resulting beef tenderness. CAPN1 present on the BTA29 chromosome, was selected in previous studies as a candidate gen for QTL. Polymorphism of CAPNI gene was studied using PCR/RFLP method and CAPN530 marker. Genotype identification resulted from this procedure. A 341 bp long fragment was present in homozygotes AA, two fragments of 195 and 146 bp were present in homozygotes GG, and three fragments of 341, 195 and 146 bp were detected in heterozygotes AG. The genotypization output was subsequently statistically evaluated. 11 homozygotes AA, 62 homozygotes GG and 89 heterozygotes AG were detected in the analyzed beef cattle population. More frequent G allele occurred in the set with the frequency of 0,657 and A allele with the frequency of 0,343. Warner-Bratzler shear test was employed to determine beef tenderness based on the shear force attribute. Samples of raw and grilled beef aging 1, 14 and 28 days after the slaughter were analyzed. Statistical methods were used to evaluate relationship between genotype and detected amount of shear force. In case of raw beef, only the samples aging 1 day after the slaughter showed significant difference in the shear force. The most positive results for the given parameter were achieved in case of AG genotype. Samples aging 14 and 28 days after the slaughter showed no difference in shear force and related beef tenderness, indicating no genotype influence. In case of grilled beef, no significant difference in sheer force indicating possible genotype influence was detected at any day of analysis. Genotype influence on grilled beef tenderness within the test animal population is minimal and statistically inconclusive.

Generally, the uncertainties in model outputs come from inaccuracies of initial and boundary conditions, further from the necessity to parameterize the small scale processes, and the structure of the model, e.g. the choice of numerical schemes or spatial resolution. In case of the simulations of future climate, another source of uncertainty arises. It is the unknown development of forcings that influence the climate system. The analysis in this work focuses on two multi-model ensembles, that come from two international European projects PRUDENCE and ENSEMBLES. The simulated 30-year mean seasonal air temperature and precipitation amounts are used, for the reference period 1961- 1990, and several future time periods. Two techniques were employed to assess the uncertainties. Influence of accreditation on patient

Nowadays the accreditation of hospitals is very current topic. During the last years there has been a change in the way of life of society. Within the provided services, the number of patients seeking higher quality medical facilities with specialist care rises. The basic lead for patient is the information about accreditation of the medical facility. The accreditation of the hospital is guarantee for patient that procedures used in the hospital comply with the prescribed standards. Standardization of care provides a unified approach to the patient and leads to safer and higher quality care. Thesis objectives Objective 1: To find out how the selected hospitals introduce "Program of improving the quality and safety of the services provided in a hospital accredited (JCI)" Objective 2: To explore the impact of accreditation (JCI) on patient satisfaction and quality of feedback. Objective 3: To define the benefits of accreditation at the hospital with JCI against the facility without JCI from the patient's perspective. Research questions 1) How does patient perceive the level of care, its accessibility and continuity in the accredited facility? 2) How does patient perceive the complex care of team of trained personnel (doctors, nurses, educational nurses, nutritional therapists)? 3) How does patient evaluate the awareness about all care related to his/her health during hospitalization? 4) What difference in care is there for patient at the hospital with accreditation from the other facility without it? Methodology To obtain the necessary information, a qualitative research was carried out and the methods of questioning with the technique of semi-structured interview on a pre-prepared set of questions were used. A total of 6 patients hospitalized in Hospital Na Homolce were interviewed. The hospital management was informed about the research in writing. Results and Conclusion The interviews revealed that the patients were provided by nursing care according to general or functional method during the hospitalisation in JCI accredited hospital. The care was available according to the patients? needs in terms of time and organisation. In one case a bed wasn?t arranged. The situation was resolved by agreement of staff with the patient. An important part of patient admission to the accredited hospital was staff presentation to patients, which was done, except for one patient. Furthermore, all patients agreed that a physician in the department had attended them every day. The cooperation of the multidisciplinary team worked well according to the patients, they particularly appreciated the work of a psychologist. Patients saw differences in the care in the security measures and in terms of availability, the interviews revealed that two respondents had experienced a long waiting time to scheduled surgery at a hospital without accreditation. The patients evaluated positively the level of provided nursing care. The results of the survey show that most of patients were aware of the differences in provided care. There is surely an influence of education of the nurses in the communication with patients, when was found out that it is needed to develop the communication more. The output of this thesis is a proposal of practical seminar for nurses, which should contain basic information about an effective communication with the patient, particularly aimed to feedback.

Modification of synthetic jet actuators for heat transfer

Synthetic jets as means of heat transfer are well known from literature. Aim of the paper is to summarise options how to set synthetic jet actuator so that the heat transfer is enhanced. The paper concerns two ways how to do it. Increasing positive velocity component at the synthetic jet actuator output and adjusting right actuator frequency are those possibilities.
