Construction and technological project of Retirement Home Pržno

This thesis describes the implementation of the proposed project, which concerns the reconstruction of the existing building of shopping center and restaurant. To built-in an atrium area, outdoor terraces and extension of the second storey superstructure of the building will be realized the project of the Retirement Home. This work contains a summary technical report, processing of technological regulations, report building equipment, design of the main building machines and mechanisms, assessment services, occupational health and safety plan, itemized budget and schedule subject to Retirement Home.

The aim of this master‘s thesis is to design a retirement home in Skrbeň. The building has three floors above ground with wall support system. In the first basement floor is a technical background. The 1st floor is rehabilitation, kitchen and dining room with facilities. In the 2nd floor and 3rd floor are the bed wards.

The projected retirement home is located in the cadaster of the town Velká Bíteš. The building is designed with respect of resident's needs of retirement age. It is a four-storey building without basement. There are designed external doctor's offices on the first floor. There are lounge with library and technical facilities on the first floor as well. Besides that, we can find accomodation spaces from second up to fourth floor. The structural system is a longitudinal wall and ceilings are made by monolithic reinforced concrete. The roof is shed. The building is detached with built chambers

It handles about a „Retirement home“ named project (3 floors, with a partial basement, building on a slight slope). The base structure is of reinforced concrete strips. The walls of the basement are made with the application of hidden formwork type system BEST. The bearing and non-bearing walls are of majority used silicate bricks. The staircase structure consists of monolithic reinforced concrete 2x brokem slab. For ceilings slab panels, are used Spiroll. The building is finished with a single layered flat roof.

The theoretical part of the thesis defines the old age, describes social services for seniors,and general matters relating to this period of life. This theoretical part is followed by the research part. For the research a qualitative method of narrative interview was used. The aim of this work is to find out, which leisure activities does a specific retirement home offer to seniors, to understand how big importance do seniors attach to offered activities, how they enjoy them, and whether they fulfill their dreams of leisure time and if they can bring them closer with other seniors.
Evaluation of life satisfaction of pensioners (healthcare institutions,homecare agencies, retirement houses)

Theoretical concepts: The central topic of my thesis is the life satisfaction of the elderly, although this concept is perceived very subjectively. Moreover, it varies even in one person at different life stages and it depends on various circumstances and events that accompany people on their life journeys. However, experiencing satisfaction always requires the saturation of one's needs, physical ones as well as mental, social and spiritual ones. Most of the times, one is able to provide for their needs, or at least affect them, through their activities and their will, but the older ones grows, the more one becomes dependent on others in this respect. This task is usually taken in part by one's family members who are willing to help and take care of their close relative, but there also arise situations where one has to entrust themselves into the hands of health professionals, especially nurses. The nurses' tasks do not just include staffing surgeries, but it is necessary for them to pay attention to the individual needs of their clients. Importance of both these components of care is entirely equivalent, because treatment success depends largely on the psychological state, defined by the satisfaction of the clients' life needs, and, on the other hand, the measure a patients' well-being is strongly influenced by the state of their health depending on successful treatment. Professional care that their clients receive is often linked to the placement of the elderly in some health institutions; possibly there may be a situation when an elderly person stays in their home and receives care from field medical staff for only a few hours a day. In any case, the objective of such care, through whatever methods it is provided, should be to achieve the highest possible degree of satisfaction of elderly individuals. Objectives of the thesis: To find out about subjective evaluation of life satisfaction of the elderly depending on their age To find out about subjective evaluation of life satisfaction of the elderly depending on their sex To find out about subjective evaluation of life satisfaction of the elderly depending on institution type Hypotheses: H1: Men are more satisfied than women H2: Satisfaction of the elderly decreases as they grow older H3: The most satisfied are the elderly who use the services of ADP Methods: The method of quantitative research was used in the practical part. For this purpose, the reduced SQUALA standardized questionnaire was used. It had been transferred to interview form due to the age of the target group of the elderly. The obtained data was processed with Microsoft Office Excel and some calculations were made with the Statistica programme. Answers from one hundred respondents were obtained as part of the survey; the respondents came from South Bohemia, particularly from the Český Krumlov and České Budějovice regions. Three types of health care institutions were contacted: a home care agency, retirement homes and hospital aftercare wards. Fifteen elderly persons were obtained for cooperation in the home care agency, 39 persons in retirement homes and 46 persons in hospitals. Results: The results of the survey show that satisfaction of the elderly is most affected by the type of health institution used, i.e. by the environment in which they are provided with health care. Definitely the most satisfied are the people who use the services of the home care agency. On the other hand, age or sex did not play a significant role when evaluating life satisfaction. Conclusion: The importance of this thesis may lie mostly in the opportunity to use the acquired knowledge, regarding the satisfaction of the elderly and the factors that influence this satisfaction, to improve the quality of care provided by nurses and other health staff.
Differences in the sight of home for the elderly before and after placing a client

The diploma thesis Perception of a Retirement Home before and after Client's Admission deals with elderly people's decision to go to a retirement home and their subsequent life in a retirement home. The diploma thesis is divided into two parts theoretical and empirical. The theoretical part focuses on aging and old age, quality of life, autonomy, modern families and elderly care at home as well as in institutions. The chapter Aging, Old Age and the Elderly explains the terms "aging" and "old age". Next, it also describes the changes that occur during old age, how the elderly manage to adapt to these changes and what is their attitude toward old age. From the nursing perspective, the senior citizens can be divided into following categories: able, fragile, dependent, completely dependent and dying. According to this division, the level of care for the elderly can be determined. However, it is vital to maintain acceptable life quality among the elderly as well as respect their real needs. Not only basic physiological needs have to be satisfied, but also higher-level needs cannot be overlooked, such as safety, security, love, acceptance, belonging, respect, recognition and personal fulfillment. One of the next main aspects that also should be taken into account is personal autonomy. Personal autonomy includes independence and freedom. It is self-sufficiency of an individual related to his or her possibilities, abilities and willingness to seize the choices and opportunities that life offers. People have the right to make independent decisions and establish or carry out them in their social environment. In general, people are considered to be autonomous individuals when they manage and organize their own activities without help of anyone else, are able to satisfy their physiological needs, stand up for their opinions and do not enforce approval, closeness and emotional support. They should not excessively fear other people or even feel guilty. If elderly people are no longer self-sufficient due to any reason, it is necessary to provide them with comprehensive care. Elderly people should be entitled to choose their preferred form of care. Elderly care can be provided in clients' private environment, by their family or by field social work. If the elderly do not have a family or it is not possible to take care of them in their home, a housing facility called a retirement home can be used. A retirement home helps its clients with following: accommodation, meals, assistance with daily self-care tasks, personal hygiene or arrangement for personal hygiene, maintaining contacts with social environment, activating tasks and enforcing rights and interests (for instance assistance in dealing with authorities, communicating with various companies, etc.). The aim of the empirical part of the thesis was to determine how the elderly perceive the retirement home before their admission and after adaptation. A secondary aim was to identify the views of family members on changes in the behavior of the elderly after their placement in a retirement home. With regard to the objectives of the thesis, a qualitative research method has been chosen which allows to examine the area in depth. The research was conducted by a survey method (a semi-structured interview). The research was conducted in two retirement homes in South Bohemia. The research group consisted of 10 respondents aged from 60 to 88 years. The target group was selected by quota sampling. The respondents consisted of clients without loss of cognitive abilities placed in a retirement home who could remember their idea about the retirement home before entering it. Another target group were family members of the individual interviewed clients. The diploma thesis states questions related to the research areas.
The role of nurse in the client's adaptation in elderly house

Theoretical outcomes Admission of elderly people to a retirement home changes their lives in many ways. With regard to these changes, it is necessary for the senior citizen to adjust effectively to this situation and thus prevent complications related to the so-called maladaption syndrome. Adaptation can be defined as a basic human trait which allows people to survive even extremely difficult situations. It can be divided into several phases. Every elderly person goes through individual phases which sometimes might be linked together and of different length. The whole adaptation process needs to be handled responsibly since the person's future life depends on its progress. Aim of the thesis Aim of the thesis was to determine how nurses can contribute to the course of adaptation process in retirement homes. Research questions How is the nurse's role perceived by the client during the adaptation process? How do the nurses understand their own roles in the adjustment process? Which factors influence the course of the client's adaptation process? What is the role of family in the client's adjustment process to a new environment in retirement homes? What problems does the client newly admitted to a retirement home have to deal with? Methodology The practical part of the Bachelor's thesis was conducted by means of qualitative research survey. The method of collecting data consisted of two variants of a semi-structured interview. The first interview variant was aimed at registered general nurses working in a retirement home. The second interview variant was focused on elderly people living in a retirement home. The first group consisted of eight respondents, seven women and one man. The second group was comprised of seven respondents, five women and two men. All the collected interviews were transcribed and the results were subsequently processed using axial and open coding. Results Based on individual research questions, the results can be divided into several areas. The first part of the results focuses on the role of nurses in the adaptation process as perceived by the client. The results show that clients consider the nurses' role to be very important. The second part informs about the role of nurses in the adaptation process as seen by the nurses themselves. The outcomes indicate that nurses regard their role as vital too. Next, the third part sheds some light on factors that influence the course of adjustment process. Factors that were mentioned most often include changes after admission to the retirement home such as new environment and home regulations, family, nurses, interpersonal relationships and favourite things brought from home. Furthermore, the fourth area describes the importance of family during client's adjustment to the new environment of retirement homes. In this part, the respondents mostly agreed on benefit of family visits. Last, the fifth area identified problems which new clients in retirement homes have to face. A new way of life was described as the most difficult problem, followed by interpersonal relationships (including a new roommate), loss of home and privacy. Conclusion The thesis offers a complex overview of problems related to clients' adjustment to an environment in retirement homes and role of nurses during this process. The results of the thesis can be used as a teaching resource for students of nursing as well as students of similar backgrounds. Next, the outcomes could be also presented to nurses, caregivers and other workers in a seminar focused on issues related to new clients' adaptation to retirement homes. Moreover, this thesis can also serve as a basis for further research. Last but not least, gerontological nurses might also benefit from information provided in this Bachelor's thesis.
Home care versus institutional care for seniors from the perspective of nursing

For many years, great attention has been devoted to the care of the elderly due to global aging of population. Almost all people want to spend their old age at home in their environment. This idea is supported by many experts who deal with the issue of old age and point to the advantages that it entails for both the senior and for the society. Therefore, it is paradox that many old people are placed in various health and social care facilities despite a wide range of various external services that enable seniors to stay in their home environment. That is why this master thesis aims to compare the nursing care provided to elderly people in their home environment with the care provided in institutions, both from perspective of a nurse who provides the care and from perspective of the elderly person or members of their family. The thesis is divided into two main parts. The theoretical part focuses on explaining the old age and the changes that come along with the old age. It also points out how old age is perceived in contemporary society. Further, it deals with different types of senior care that is most frequently divided into family, health and social care. The practical part is focused directly on individual families of seniors who live at home supported with home care, and to families who have a member in the department for the chronically ill (DCI). In the practical part I also focused on nursing staff, particularly nurses who come into contact with the seniors every day. A qualitative research method was used for the research survey. The technique of in-depth interview was used for data collection. The first part of the research group consisted of nurses working in DCI and nurses working in home care. The second part of the group consisted of seniors who reside in DCI and their families and the elderly who use home care services and their families. Interviews with the nurses, seniors and their families are divided into individual categories: awareness, reason for placement to DCI, satisfaction with the care provided, the advantages and disadvantages of home care/DCI, differences in care in DCI and at home, in foods and drinks, positioning and incidence of pressure sores, hygiene, administering medicines, re-bandaging, in bowel habits, behavior of staff/family, facilities for a senior. The investigation shows that families consider hospital care and home care to be very good, but it can be seen that home care nurses enjoy greater trust of the family and have much closer relationship than those in a hospital. Both groups of nurses think that the care they provide is of high quality and professional, but point to the lack of funding that a hospital and a home care agency have. A very surprising fact emerged that families that have an elderly person in institutional care are eager for the placement since they believe that they would not be able to care of them properly themselves. The investigation cannot clearly assess which type of care is best because both methods have their positives and downsides for every party involved and there are different variables, such as the health condition of the elderly, their family or social background. Quantitative research over a longer period of time would be required for thorough mapping of the entire situation which could monitor the provided care in the long run and also take into account the above variables that may change over time.
