Possibilities of pulmonary rehabilitation for oncological patients after lung resections

The lung carcinom is one of the most common types of cancer in male population of the Czech Republic. The usual signs are chest pain, stuffiness, hoarseness and coughing. Lobectomy is usually done to the patients with smaller tumors. It is important to ensure the right preoperative education of the excercises. The priority after the operation is pain therapy, removal of the increased amount of phlegm, support of expectoration, improvement of the ventilation parameters and prevention of post-operative complications. The selected objective of this thesis was to map the problematics of pulmonary rehabilitation of oncological patients after lung resections and also to suggest an approach to the pulmonary rehabilitation of oncological patients after lung resections. The theoretical part describes the anatomy of primary and auxiliary respiratory muscles, the characteristics of the lung carcinoma and its chirurgical therapy and the techniques of pulmonary rehabilitations including different physiotherapeutic concepts like soft and mobilization techniques, the Brügger concept, proprioceptive neuromuscular facilitation and kinesiotaping. The practical part is consisted of qualiative research in which three oncological patients after the lung resetions participated. The research includes trigonometric and anthropometric examinations, muscle test, measurement of maximal exhaling and maximal inhaling pressure, the Borg's scale of percieved exertion and saturation of hemoglobin from oxygen. The therapy began the day before operation and ended before discharge on the seventh to eighth day. The results went back to previous values before the operation in some of the parameters and there were no negative effects of the therapy. It is safe to say that the therapy had a positive effect on the patients without any complications.

The theoretical part of the bachelor thesis gives a description of Covid-19 within its overall context, including pulmonary rehabilitation, the importance of early physiotherapy intervention and rehabilitation options in specific stages of the disease. Title: Case study of physiotherapeutic care of a patient diagnosed with pneumonia Aims: The aim of this thesis is to briefly summarize the basic theoretical knowledge of pneumonia from professional sources and to apply selected physiotherapeutic methods used in the diagnosis of pneumonia and to assess their effectiveness. Summary: In the first part of the theoretical part, the basic theoretical knowledge of pneumonia is briefly summarized and ARDS is also marginally described. The second part outlines how physiotherapists can contribute in the treatment of these conditions and what evidence-based medicine has to say about physiotherapeutic approaches in the ICU and in pneumonia. The special part of this bachelor thesis is based on a continuous professional practice at the Department of Anaesthesiology, Resuscitation and Intensive Care at the Institute of Clinical and Experimental Medicine in Prague between 10 January 2022 and 4 February 2022 and is presented in the form of a case report of a patient with a diagnosis of pneumonia. Sensorimotor training - part of comprehensive treatment of chronic obstructive pulmonary disease

The thesis deals with the possibility to influence the values of the lung function in patiens with chronic obstructive pulmonary disease (COPD) after exercise carried out according to the Methodology of sensorimotor training. The thesis contains a summary of theoretical knowledge about the pathophysiologic expresion of COPD, including their consequences not only for the respiratory system, but musculoskeletal system as well. Furthermore the thesis includes the information about the Methodology of sensorimotor training, that was used to influence the posture correction in our research. Methods: The spirometric examination of flow-volume curves monitor the changes prior to and after the exercise according to sensorimotor training in patiens with COPD (mean age 60, 733 ± 11,817). Results: The using of sensorimotor training can provide correction of the position of the whole body, of the chest, and change a chest mobility and in total to set up conditions for more economical muscle activation. This enable the changes in the values of the lung functions in the patiens with COPD, that we measured. Not all changes showed statistically significant change. Conclusion: Using sensorimotor training have an objective effect on the values of the lung function in patiens with COPD.
The Effect of Airway Clearance Techniques on Patients with Chronic Obstructive Pulmonary Disease

Title The effect of airway clearance techniques on patients with Chronic Obstructive Pulmonary Disease. Objective The main purpose of this study is to expose and evaluate the efficacy of airway clearance techniques on patients with Chronic Obstructive Pulmonary Disease by using pulmonary functions and Quality of Life measurements from recent studies. Methodology This thesis is literature review. The selected studies were according to inclusion and exclusion criteria. The selections of recent studies were published from the year of 2000 until 2017 in English language. The following research databases were selected to identify the relevant topic: PubMed, MEDLINE, Embase, Cochrane, PEDro, and CINAHL. The parameters' results from the articles have been also selected by evaluation the statistical differences according to the P-values. Results Twenty-three articles out of 117 have been found according to inclusion criteria, exclusion criteria, and outcome measures. The influence of extrinsic motivation on physical condition in lung transplant candidates

Rehabilitation takes an important part in the care for lung transplant candidates with chronic lung disease. The theoretical part of this thesis summarizes current knowledge about motivation and adherence to rehabilitation, about telerehabilitation options (telemonitoring physical activity using pedometer), and their use in lung transplant candidates. In the practical part it was investigated whether extrinsic motivation (education, pedometer, communication with physiotherapist via e-mail) leads to an improvement in the six-minute walking test outcome (6MWD) or in adherence to rehabilitation in lung transplant candidates. It was found no statistically significant improvement in 6MWD nor in adherence to rehabilitation in extrinsically motivated patients waiting for a lung transplant. However, data suggested increasing trend of 6MWD after intervention.

Work placement: Institute of Rheumatology Prague Objectives: The aim of the thesis was to investigate the various physiotherapeutic techniques in improving the physical state of a 36-year-old, male patient with Interstitial lung disease related to Antisynthetase syndrome with the presence of AntiEJ, AntiRO52 and, AntiRO60 antibodies. Methods: PIR, DNS, soft tissue techniques, joint mobilization, physical exercises, IMT threshold, sensomotoric exercises. Results: Improvement in breathing capacity and breathing pattern, improvement in posture, improved mobility of soft tissues. Conclusion: This study was divided into 2 parts, the general part, which discusses the anatomy of the respiratory system and the nature of the disease, and the special part, which represents the course of the treatment and its effect on improving the general well-being of the patient. Title: Case study of the physiotherapy treatment of patient with MODS and Covid-19 in case history Abstract This bachelor's thesis deals with the issue of acute and postacute physiotherapeutic care in patients hospitalized in the ICU. The primary aim of the thesis was to conduct the specific case study and provide theoretical basis for diagnoses and physiotherapeutic interventions related to the given case study. Secondary objective was to briefly summarise the current available knowledge about Covid-19, post-Covid syndrome and highlight the importance of this issue in physiotherapeutic practice. This report is divided into three main parts. The first two chapters are theoretical. They focus on detailed descriptions of selected diagnoses and possible therapeutic interventions. These include systemic inflammatory response syndrome, sepsis, septic shock, multiple organ dysfunction syndrome, acute respiratory distress syndrome and post-Covid syndrome. Martina Havlová Title of bachelor thesis: Physiotherapy Focused on Physical Activity Intoleration in Patients with Chronic Obstructive Pulmonary Disease Abstract: The thesis is divided into theoretical and practical part. The theoretical part is written in the form of literary research. It begins with a brief introduction to COPD, etiology, pathogenesis, epidemiology, and classification. Substantial part is devoted to clinical manifestations of the disease. Furthermore, it deals with physical activity in patients with COPD, which is the central topic of the work. Last but not least, lung rehabilitation and treatment options are mentioned. The practical part of this work is focused on influencing the intolerance of physical activities of a patient with COPD. I set up a therapeutic unit to increase physical exercise tolerance. I verify the effectiveness of the therapy in the form of a case report, which contains the input and output examinations. The result is not only a positive effect of this therapy on physical exercise tolerance, but also on other symptoms that are often associated with this disease. Rehabilitation in lung transplant candidates

Lung transplantation is the method of treatment for patients with end-stage lung disease. The main theoretical part of this bachelor's thesis is focused on the possibilities of rehabilitation in lung transplant candidates. This rehabilitation concept includes patient's education, individually designed exercise program, techniques of respiratory physiotherapy, nutrition therapy and psychological support. The practical section of this bachelor's thesis incorporates a case report of a lung transplant candidate who underwent 4 months of preoperative rehabilitation program. Tha main difference after completing preoperative rehabilitation was found in maximal inspiratory muscles strength, which increased of 17,71 cmH2O.
