Options of continual glycemia monitoring in critically ill patients and treatment of hyperglycemia with the use of insulin algorithms

Glycaemia control is nowadays one of the most discussed topics in intensive care. The landmark Leuven study, which has brought hyperglycaemia in intensive care to the centre of interest, demonstrated that tight glycaemia control (TGC) targeted to maintain euglycaemia between 4.4 to 6.1 mmol/l reduced mortality and frequency of organ complications associated with critical illness. Although this study was published eight years ago, there is still no general agreement about approach to tight glycaemia control in critically ill patients. Doubts have been raised about both the universality of intensive glycaemia control, and especially its safety in terms of potential damage to patients by episodes of hypoglycaemia. Our studies did not focus on questions of importance or risk of TGC, but we concentrated on key point of hole TGC concept - on glycaemia control in critically ill patients itself. The aims of our studies were the research of subcutaneous adipose tissue as an alternative-site for continuous glucose measurements in ICU patients, and comparison and development of insulin algorithms for hyperglycaemia treatment and normoglycaemia management. Care of the patient with acute respiratory insufficiency

The thesis entitled Care for a Patient with Acute Respiratory Insufficiency was written at an anaesthesiology and resuscitation ward. It is divided into a theoretical and a practical part. The theoretical part provides a fundamental overview of the anatomy and physiology of the respiratory system, as well as patophysiological mechanisms involved and certain nosological units causing respiratory insufficiency. The practical part maps in detail the course of a difficult hospitalisation of a female patient with severe respiratory insufficiency. The treatment process is developed into nursing diagnoses serving as an overview of the aspects that must remain in the centre of focus when caring for patients with such severe conditions. The aim of this thesis is to provide a comprehensive overview of treatment, complications and nursing activities for a patient with acute respiratory insufficiency.

This bachelor thesis focuses on the design, fabrication and testing module 4 independent switching appliances. Switching can be controlled automatically according to the ambient temperature and time, or manually. The equipment includes the measurement of power. Design of permanent magnet synchronous machine

Nowadays in the area of drives, classical direct current machines are replaced for reason of low efficiency and low service life their frictional part by synchronous permanent magnet machines, among that belongs brushless or electronically commutated machines. This machines provide benefits of fine and effective regulation with no limits incurrenced by mechanical parts. Within this work, introduction with properties and construction of various types synchronous machines witch permanent magnets are performed, further the analytical calculation of design are made with effort to the best configuration for requisite characteristics. This design is compared with simulation made by computer technique and in the next step modifications of individual parts and choosen parameters for maximizing efficiency and minimizing volume are made. Comparison of success changes in design is made by simulation in program RMXprt again and the final machine is compared with original design, benefits of changes within efficiency and with considering of quantity used material are evaluated and his charakteristics are checked by Finite Element Method.
