Generic substitution and generic drugs ? the analysis of experience of patients

The law about Generic substitution was legislatively established in the Czech Republic in January 2008. The following year certain doubts about the quality, safety, and potential adverse effects of generic drugs appeared. The above mentioned facts prompted us to analyze patient's awareness, opinions, and experience with generic drugs. We used a prospective questionnaire method in several selected pharmacies in the Czech Republic. Data were collected in the period from January to May 2009, and then they were analysed by specific statistic methods. MS Excel and statistic software SPSS were used. The total of 501 filled-out questionnaires were collected and evaluated. Average age of patients was 50 and two- thirds of them were women. 75 % of patients agree with generic substitution. Half of the number of the patients was at least once offered generic substitution, and majority of them agreed with it. Generic substitution was requested only by 7 % of patients. Generic substitution was more frequently offered to women, older patients, and patients using more than one kind of drug (p<0.05). The results show that GS is accepted by majority of patients, and pharmacists are generally perceived as guarantors of its safety. A wide range of these situations can be suitably prevented. Aim of study: Identify and evaluate signs of medication errors in the providing of pharmaceutical care. Analyze the factors that may affect the occurrence of these medication errors. Methodology: Questionary with prospective screening of medication errors over the period of 20 days in 7 public pharmacies from Karlovy Vary region. Following questionary focused on the pharmacists' opinions (from same region) on the factors, which contribute to possible mismatch in the drugs dispensation. Data evaluated by descriptive statistics. Results: 20 potentially serious medication errors that could endanger the health of the patient were detected (1 error per 1000 dispensed prescriptions). Tutor: Vlcek J., Dept. of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Kralove, Charles University in Prague,Czech Republic Background: Diabetes mellitus type 2 is a group of metabolic diseases whose common trait is hyperglycaemia. Aim: To assess the group of patients, diabetics with type 2 diabetes mellitus according to different points of view and to assess their treatment. Methods: 106 patients with DM type 2 (49 male, 57 female, mean age 61,8) were retrospectively studied for 23 months. The data were obtained from patient's data in diabetologic consulting room. Results: The concentrations of glycosylated haemoglobin were in 45 % of patients less than 6,5, 13 % of patients had these concentrations 6,5-7,5 and this can be considered as satisfying state of disease. 42 % respondents had higher concentrations of glycosylated haemoglobin. The most common medication in monotherapy were sulfonylureas in 25 % of patients and metformin in 16 % of patients. The most common combination of antidiabetics were metformin with sulfonylureas in 17 % and metformin with insulin in 12 % of patients. Analysis of prescription and potential drug interactions in patients with inflammatory bowel disease.

Background: Concurrent use of multiple drugs often increases therapeutic effectiveness, but very often may also pose a risk for drug interactions. These may result in severe clinical manifestation, if they are not paid an attention and unproperly managed. Aim: To assess percentage of potential drug interactions in the population of patients with diagnosis of inflammatory bowel disease (IBD) and suggest their management. Methods: 177 patients with IBD (84 male, 93 female, mean age 36,9, range 16 - 78, 117 CD, 60 UC) enrolled. Their complete medication was gained by means of an interview and subsequently evaluated and processed with Thomson MICROMEDEX® DRUG - REAX® System to reveal potential drug interactions. All the identified drug interactions were taken as potential. There was no prove if patient was harmed by them. Results: In CD, the most prescribed drugs were aminosalicylates (39 %), corticosteroids (both topical and systemic)(21 %) and immunosupressants (23 %). The other prescribed medicines were proton pump inhibitors, iron containing preparates, probiotics and TNF-α inhibitors. The knowledge and atttitude of mothers to vaccination in Czech Republic in the multiresources system

The childhood vaccination in the Czech Republic represents a very complex area. While the current legislation defines which children vaccinations belong under mandatory, fully reimbursed National Immunisation Program (NIP), there are also possibilities to undergo vaccination as a result of own decision. The role of the State as the only guarantee and payer of children vaccination has been, due to registration of next vaccines, extended by subjects who use opportunity to vaccinate against additional infections diseases and consequently pay for it. Nowadays, financially, non-reimbursed vaccination doubles NIP vaccination reimbursed by the state. Almost 350 million CZK was spent in 2008 on vaccination against human papillomavirus (HPV). That represents the most paid non-mandatory vaccination. National-wide Research amongst mothers of children up to five years of age performed in 2008 showed high confidence in the benefit of vaccination when only 4% of the mothers are in contradiction with this statement. Nevertheless, the research also unveiled need for more information as 28 % of the mothers think that children are receiving more vaccination than is good for them. Pharmacoeconomic and Pharmacoepidemiology Aspects of Allergy Rhinitis Treatment and Prevention

7 SUMMARY Rational pharmacotherapy should ensure a safe and cost-effective treatment of patients with allergic rhinitis (AR). Two main aspects of treatment of AR were addressed (1) comparative benefits and costs of subcutaneous and sublingual allergen immunotherapy in patients with allergic rhinitis, and (2) systemic adverse events - neuropsychiatric disorders and gynaecomastia - during intranasal corticosteroid use. (1) Allergen specific immunotherapy: benefits and costs  Our pharmacoeconomic study on allergen specific immunotherapy showed that sublingual and subcutaneous immunotherapy reduced clinical symptoms and the need for symptomatic medication in adults with grass pollen induced rhinoconjunctivitis compared to standard pharmacological treatment. The clinical efficacy of the sublingual and subcutaneous immunotherapy was not significantly different in the third year of specific immunotherapy. Nevertheless, the SCIT group exhibited slightly better improvement in visual analogue scale and a greater reduction of systemic antihistamines versus SLIT in the third year compared to the baseline year. The aim of the thesis was to analyze the pharmacotherapy of epilepsy in the Czech Republic and quality of life (QOL) of patients with epilepsy as well as identification of factors with main influence on QOL. Each specific issue has been described and discussed in three chapters and published in five original papers. 1. Pharmacoepidemiology of antiepileptic drugs 1a. Pharmacoepidemiological study of a group of patients with epilepsy Aim Aim of the study was to analyze epilepsy treatment in patients from two specialized neurology departments in the Czech Republic, describe seizure type and seizure frequency, duration of the disease, antiepileptic drug (AED) treatment and employability of the patients. Methods and results Epileptic outpatients aged over 16 years with confirmed diagnosis of epilepsy attending the Centre for epileptology and epileptosurgery (CEE) Thomayer's Teaching Hospital in Prague or the Neurology department of University Hospital in Hradec Králové (FN HK) were included (n = 427).
