Management of patients with cerebral palsy.

This thesis is focused on summarising basic knowledge about the diagnosis cerebral palsy (CP), describing the development of an indiviual with CP and on compiling a care plan from birth up to adulthood. In the theoretical part knowledge about historical aspects, risk factors, prevalence, diagnostics, classification and comorbidities of CP is summarized. In the theoretical part are also included the characteristics of each life period of an individual with CP and interventions that are usually indicated during the time, including a comparison with foreign approaches. The practical part is an overview of intervention indications in relation to age compiled from data of patients' medical documentation. These data were processed into a timeline, which is a part of this thesis.

Kateřina Rybářová Title: Problems Coping with Household Care by People with Transversal Spinal Cord Injury from Perspective of an Occupational Therapist Abstract: This bachelor thesis considers problems of people with transversal spinal cord injury coping with household care from perspective of an occupational therapist. The aim of the thesis is to qualitatively compare how men and women with transversal spinal cord injury in C6 level cope with undertaking household chores. The theoretical part describes difficulties of spinal cord injury and household chores and sets out possible occupational therapy intervention. It clarifies options of adapting household chores for those with transversal spinal cord injury in more detail. The practical part is set out by case reports of two client groups (male and female) with transversal spinal cord injury in C6 level. It introduces anamnesis, examination by occupational therapist, appraisal of independence using the SCIM and appraisal of the home environment, by using the methodology SEMAFOR home. Title: Pilot Testing of Usability of the Nine Hole Peg Test in Occupational Therapy in Patients with Rheumatoid Arthritis and in Patients with Hand Osteoarthritis Abstract: Occupational therapists can objectively assess manual dexterity in patients with rheumatoid arthritis (RA) or hand osteoarthritis (OA) using Nine Hole Peg Test (NHPT). It is performed according to a manual that includes precise instructions. The subject is required to place nine pegs from the container into the pegboard as quickly as possible and return them straight away. The time required to complete the test is measured with a stopwatch. The aim of the bachelor thesis is to perform a pilot testing of usability of a new Czech translation of the NHPT manual in patients with RA or with OA. The partial goal is to obtain video documentation during the administration of the Nine Hole Peg Test and to create recommendations for using this test in practice in patients with RA or with OA. With an approval of the Ethics Committee of the Institute of Rheumatology Prague, 19 patients (8x RA, 11x OA; 14 female, 5 male) were tested using NHPT. Title: Pilot Testing of Usability "Action Research Arm Test (ARAT)" in Persons after Acquired Brain Injury Abstract: The bachelor thesis is focused on use of the Action Research Arm Test (ARAT) tool in person after acquired brain injury. The often problem in this population may be decreased or impaired function of upper limbs, which is what the ARAT is dealing with. The aim of the thesis is assessment of the tool and its characteristics. Theoretical part describes different types of brain injury, primarily stroke and their consequences. A chapter about grips and their pathologies follows. Grips are the dominant part of the ARAT testing. The second half of the theoretical part describes testing in occupational therapy and importace of standardised tool. The ARAT is further described in detail. Other tools for testing upper limb function, are briefly described in the conclusion. The practical part deals with pilot testing of the ARAT with the 8 patients. Short case studies of all patients are presented and the results of the individual subtests and total score of ARAT. These results are compared with each other between the patients. Barriers of Public Environment from the Perspective of People with Acquired Brain Injury during Shopping

Author: Lucie Markovcová Title: Barriers of Public Environment from the Perspective of People with Acquired Brain Injury during Shopping Introduction: Removing barriers of the enviroment is one of goals of The Convention on the Rights of Persons with Disabilities. In foreign studies, occupational therapists are also focused on the effects of barriers on the performance of people with disabilities, which limit them due to sensory disorders or their phatic and cognitive functions. Aim: Find out the barriers of public enviroment, that complicate shopping of people with aquired brain injury (ABI). Methods: A questionnaire with 34 questions was made and distributed in electronic version and also in version for printing by emails to facilities for adult people with ABI. Therapists and family members were allowed to help them with filling the questionnaire, so people with malfunction of phatic functions and cognitive skills were not limited. Also involved observation during shopping was made with 5 people with ABI. Results: The questionnaire was filled by 26 people with ABI on average in 12 minutes. 20 of them were after stroke. Occupational therapy is currently an essential part of comprehensive rehabilitation. Occupational therapy is a relatively new field that is continuously evolving. It tries to keep up with time and its demands. The main goal of this thesis was to depiction a real profile of the occupational therapy profession in the Czech Republic. The thesis is divided into two main units, namely the theoretical and practical part. The theoretical part describes the basic aspects of the field of occupational therapy and the work of occupational therapists in the Czech Republic according to the literature search. The practical part of the thesis contains the methodology of the work and the results of the questionnaire survey. Three hundred and fifteen respondents participated in the research. Processed and analysed data are presented using graphs, tables and figures. At the end of the thesis, a profile of the occupational therapy profession is compiled from these results. Information from the practical and theoretical part of the thesis is then compared in a discussion. The output of the thesis is a depiction of the real profile of the occupational therapy profession in the Czech Republic according to information obtained from respondents. Occupational therapy is currently an essential part of comprehensive rehabilitation. Occupational therapy is a relatively new field that is continuously evolving. Title: Occupational Therapy Assessment of People with Guillain-Barré Syndrome in Productive Age Focusing on Possibilities of their Employment Abstract: Guillain-Barré syndrome is an acute inflammatory neuropathy affecting the peripheral nerves. Its most common symptoms are symmetrical weakness, myalgia, paraesthesia or dysesthesia, areflexia, and temporary quadruplegia. The syndrome starts affecting lower limbs. It progresses in the cranial direction which may lead to ventilation insufficiency and autonomic dysfunctions. It may even affect ability to work of people in productive age. Occupational therapists may specifically target the employment opportunities of people with Guillain-Barré syndrome and support them in returning to work as part of occupational therapy interventions conducted in health and social care institutions or prevocational assessment centers. This thesis describes the occupational therapy examination in detail with emphasis on the possibilities of employment of three people in productive age with Guillain-Barré syndrome. Clinical Utility of Occupational Therapy Standardisee Test "Action Research Arm Test (ARAT)"

The Action Research Arm Test (ARAT) is a standardized tool, which is used for fine and gross motor skills evaluation of hemiparetic patients after stroke. It is widely used abroad thanks to its excellent psychometric properties and easy administration. It is also used in many foreign studies to evaluate the function of upper extremity, especially for neurological patients. Unfortunately, so far it has been used only sporadically in the Czech Republic. In the theoretical part of this thesis the ARAT and the Modified Frenchay Scale (referred as "MFS") is described. In the practical part, the MFS is compared with the ARAT. The main aim of this diploma thesis is a pilot comparison of sensitivity of upper limb motor skills evaluation done by ARAT and MFS. Thirty patients after stroke were tested in the Rehabilitation Center Kladruby in three weeks. They were further divided into three intervention groups according to the severity of the paresis (light, medium, heavy). The analysis of the results shows that the sensitivity of both diagnostic tools is statistically comparable. According to the Spearman correlation coefficient, the ARAT and the MFS correlate very strongly in range of 0.96-0.98. Grasp evaluation in patients after stroke

Title of diploma thesis: Grasps evaluation in patients after a stroke Objective: The main goal of the pilot study was to determine minimal clinically important difference in selected tests focusing on improvement in the grasps of patients in the chronic phase post stroke. The main hypothesis was set to prove that the differences between the initial and final assessments based on the Modified Frenchay Scale characterizes patients' improvement more accurately than the Score for visual evaluation of functional task of the hand. Methods: The research group consisted of 40 participants (26 men and 14 women) all in the post stroke chronic phase who were selected based on the criteria for inclusion in the research. Data was collected at the Department of Rehabilitation Medicine and the First Faculty of Medicine. Using video documentation of the Modified Frenchay Scale used during both the initial and final assessment, the test itself was then analysed. A 3rd subtest was also used to evaluate the Score for visual evaluation of functional task of the hand. Using the Spearman correlation coefficient, the statistically significant level was established (p <0,05) followed by calculating the critical value of the Spearman correlation coefficient (rsp (0,05,40) = 0,264), and thus verifying the hypothesis.
