Analysis of drug-related problems in a healthcare facility IX Author: Anna Makovyeychuk Supervisor: PharmDr. Martin Doseděl, Ph.D. Department of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Kralove, Charles University Introduction: Patient safety culture is one of the basic principles of healthcare provision. One of important focus is the elimination of risks, including problems associated with medicines. DRP (drug-related problem) is defined according to PCNE (Pharmaceutical Care Network Europe) as an event or circumstance related to drug administration that actually or potentially affects the intended therapeutic goals. Therefore, the role of the clinical pharmacist within the multidisciplinary team is key to uncovering the mentioned problems. Aim: The task of the theoretical part was to create a summary of drug problems associated with the group of opioid analgesics-anodynes (according to Anatomical- Therapeutic-Chemical (ATC) classification group N20A). The basis for processing and structuring was the PCNE 5.01 classification system. The aim of the practical part was the analysis of drug-related problems that were discovered in patients during the review of pharmacotherapy in a healthcare facility, its classification according to the PCNE 5.01 system and monitoring the level of...

Analysis of drug administration by nurses in health facility XIV. Author: Sandra Newaldová Tutor: PharmDr. Martin Doseděl, Ph.D. Department of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Králové, Charles University Introduction: Medication errors are one of the causes of adverse events in healthcare facilities, leading to patient endangerment or harm. A study looking at this issue reports that the most common errors occur when a nurse administers medicines in a healthcare facility. Aim: The aim of the thesis is to monitor medication errors of nurses during the preparation and administration of medication to patients and to analyse the prevalence of medication errors, determine their type, their relationship to other influencing factors, and their severity with respect to the patient's condition. Methods: Prospective observational study with direct observation method, conducted in a specialized medical rehabilitation facility. The study included two pre- and post- intervention phases in which nurses' medication and procedural errors were monitored, analysed and evaluated. Rohatec Special Regime Home
Raszyk, Kristián ; Klement, Miloš (referee) ; Boleslavská, Yvona (advisor)
The subject of my design is an architectural study of a new special regime home in Rohatec on Nové řádky Street. It is a healthcare facility offering institutional care, complemented by supportive services. The building will provide institutional care capacity for 90 clients. Staffing is designed for two shifts, each consisting of 50 employees. It is a standalone, three-story building with a basement. All buildings are covered with flat green roofs. Broadly, the facility is divided into a main building, which primarily contains patient wards and directly related services, and a secondary building, which houses facilities used by clients intermittently. The main building includes five patient wards. Additionally, the main building contains supportive services such as administrative offices, a pharmacy, common rooms, technical spaces, and underground parking. The secondary building includes common rooms, a dining area, kitchen operations, and spaces for physical exercise and rehabilitation.
Rohatec Special Regime Home
Velazquez Hernandezová, Natalie ; Klement, Miloš (referee) ; Boleslavská, Yvona (advisor)
The subject of this master's thesis is a study of the construction of a special regime home in Rohatec. The aim was to create a proposal for a healthcare facility for institutional care supplemented by support services. The care home is designed to accommodate 90 clients and two shifts of fifty staff members each. The site under consideration is located on the outskirts of the village of Rohatec within the residential area of family houses. The site consists of three parcels (38/3; 38/5; 37/3) owned by the region and one parcel (40/2) owned by the Czech Republic's Forests. Parcel 37/3 currently contains a family house, which will be demolished. Parcel 38/3 contains a regulatory gas station, which will be relocated off-site for the purpose of the new construction, while ensuring public access. The forest protection zone of 50 meters will be exceptionally reduced to 0 meters with the consent of the Forests of the Czech Republic to enable construction on the site. The proposed building is in compliance with the zoning plan. The proposed new construction is a standalone, basement building that respects and also concludes the street line of family houses. The building has three above-ground floors and one underground floor. The main entrance to the main building is oriented from the northwest at the level of the ground floor from Nové řádky Street. The main entrance to the garages is oriented from the southwest at the level of the basement floor from Cihelny Street. The main exit from the collective garages is on the northeast facade of the building at the level of the basement floor leading to Nové řádky Street. The maximum height of the main building is +15,500 meters (object access to the elevator). Parking is mostly provided in the underground garage of the building. Another 13 public parking spaces are located along the road on Nové řádky Street. Two of these spaces are reserved for disabled and rapid admission or for ambulance access. Along the northeast boundary of the plot adjacent to the forest, there is a cascading garden with four height levels connected by gentle ramps with a slope of 1:20 and stairs. The first level directly connects to the ground floor level and serves as a social outdoor center with a pétanque court, tables for playing games, dining, resting benches, and flower beds with herbs and perennials. The second level consists of recreational stairs with space for various outdoor performances and a section with exercise machines for seniors. The third level is spiritual. There is a small meditation area here. The last level is consistent with the original terrain. It serves as a quick connection to the forest park and operational ramp from the garages. In the garden, there are two stepped ponds around the leveling ramps, which will cool the climate of all levels of the garden. Elevated flower beds with fragrant perennials or herbs and benches with a view of the forest permeate the entire garden.
The issue of falls of seniors in healthcare facilities
BUCHOVÁ, Nikola
This bachelor's thesis focuses on the issue of elderly falls in a healthcare facility. The aim of this thesis was to map the causes of falls in the elderly. Furthermore, the aim was to identify risk factors, describe the consequences of falls, and identify nurses' preventive measures in preventing senior falls in a healthcare facility. The theoretical part describes aging, then the theoretical part mentions geriatric syndromes, different causes of falls and risk factors of falls. The next chapter describes falls prevention, where compensatory aids are included and described. The last chapter briefly described patient education in a healthcare facility. The second part of this thesis is the empirical part. The empirical part was prepared by means of qualitative research, using the method of semi-structured interview. In the research, 2 interviews were created, in one interview 12 questions were created for nurses and in the other interview 15 questions were created for hospitalised patients. Seven nurses and seven hospitalized patients participated in the interviews. Data were processed using open coding, by the pencil and paper method. Five categorization groups were created for the interviewed nurses and four categorization groups for the hospitalized patients. The results in the empirical section show that the most common causes of falls include inappropriate footwear, wet floors, poor lighting and poor use of guardrails (barriers). Risk factors most commonly include associated patient illnesses, age, change of environment, poor accessibility of signalling equipment and use of medication. Regarding sequelae after falls, hematomas, lacerations and pain were the most common. Preventive measures for nurses are most commonly signalling devices, grab rails in bathroom, handrails in corridors. Other preventive measures include, appropriate footwear, proper lighting, bed adjustments, and the use of bed rails (barriers).
