Validity of "Muscle excitability scale" for spasticity assessment in patients with spinal cord injury

The aim of the thesis was to assess a construct validity of "Muscle Excitability Scale" in assessment of spasticity in patients with spinal cord injury. The correlation between this scale and other clinical tests evaluating various aspects of spasticity was investigated. 48 participants with symptoms of a spasticity were included in the study. The Spearman rank correlation coefficient was used for quantification of correlation between each two tests. The most significant correlation was found between the Muscle Excitability Scale and the Modified Ashworth Scale (rs=0,26 - 0,35, p<0,05). Less significant correlation was found in comparison with the Clonus Scale (rs=0,27, p<0,05) and with the SCI-SET (rs=0,25 - 0,26, p<0,05). There was no correlation between the Muscle Excitability Scale and the Penn Spasm Frequency Scale. Additionally every test was compared to each other and then the outcomes was discussed and compared to results of other similar studies. Possibilities of using aquatic therapy for patients after spinal cord injury

The thesis deals with patients after spinal cord injury (SCI) focused on their health problems and possibilities of using aquatic therapy, that can participate in the program of comprehensive rehabilitation. General section summarizes the classification of patients after SCI, the properties of the aquatic environment and the physiological reactions of the organism in the aquatic environment. It also focuses on the issue of sitting of paraplegics and current methods of assessment in clinical practice. The special part contains a case of a patient after SCI at the level of Th8-Th10, an example of a therapeutic unit in the pool aimed at improving the quality of sitting. Trail-O as the means of integration of persons after spinal cord injury

Title: Trail-O as the means of integration of persons after spinal cord injury. Objective: The goal of the thesis is to describe the current situation of the Trail-O event and to highlight its utilization options for handicapped people. Methodology: The research group consists of 18 respondents, of which 10 handicapped and 8 persons without a handicap, 11 men and 7 women engaged in Trail-O. The research was conducted using a public inquiry, the investigation focused on the personal data and information about this sport. Data collection took place at the European Cup in Trail-O in September 2015 and electronically on the server www.survio.com. The data was evaluted by descriptive statistics, the absolute and relative frequencies. Results: Men are more engaged in Trail-O than women and approximately the same amount of handicapped as respondents without a handicap. 17 respondents has previous experience with other sports, 13 of them to the Trail-O received by previous operation of orienteering. The main motives for doing Trail-O are nature, people, the possibility of competition between the handicapped and the healthy as well as the possibility to contest on top level even with a handicap. The evaluation of functional sitting stability of paraplegics

Title: The evaluation of functional sitting stability of paraplegics Objectives: The aim of this work is to evaluate the functional stability of paraplegics sitting through tests. Another objective is to determine what is the effect of fourteen days rehabilitation intervention on the sitting patients. Methods: Designed experiment was processed by the form of quantitative research. There were 10 pacients with spinal cord injuries, especially paraplegics included - 5 men and 5 women. The history was detected with the main emphasis on assessment ASIA score. Measurement of unsupported upright sitting was done thanks to the Plantograf V09. It records the current pressure distribution and movement of the center of pressure (COP). Functional T - shirt test was evaluated by using a stopwatch. Probands responded two questions about their current condition after rehabilitation after the output measuring. The therapist rating was included in the results. All data were further processed in Microsoft Office Excel 2007. Results: Measurements confirmed the reliability of both used tests. Monitoring the activation of selected muscle groups of clients with spinal cord injury

PaedDr. Karel Jelen, CSc The aim of this thesis is to monitor the activation of selected muscle groups (musculus deltoideus pars anterior, musculus deltoideus pars posterior, musculus triceps brachii caput laterale and musculus trapezius pars descendus) during double poling ergometry. Nine subjects with different spinal cord lesions volunteered to participate in the study. The surface electromyography is used to detect the musculus activation. The monitoring of four muscle groups is devided into three phases. Firstly, all the participants double-poled for one minute at moderate intensity according to the Borg scale. Each participant underwent a 10-minute intervention to correct the posture. In the last phase the volunteers exercised again for one minute at moderate intensity. The aim of the intervention is to decrease muscle activation of musculus deltoideus pars anterior and musculus trapezius pars descendus (muscles with a tendency to hypertonia during wheelchair propulsion and activity of daily living) and to increase muscle activation of musculus deltoideus pars posterior and musculus triceps...
Options of electrical stimulation of paretic muscles in patients withh spinal cord injury

Electrostimulation of neuro-muscular tissue is one of the elementary treating methods in physiotherapy. One of the areas of its use is rehabilitation of patients with spinal cord injury. As there doesnt exist enough resources written in czech language, I settled down to the goal of present the overview and options of its use in those patients. As completing this text, I came out of my knowledges drawn through the studies on 2nd Faculty of Medicine Charles University, specialized publications available in czech language and not least from specialized articles, case studies and literature of worldwide authors, because those are majority of informations aviable about this problematics. This work stays from parts about spinal cord with its injuries and elecrtostimulation and its practical use. Big portion of the work deals with usage of electrostimulation to improve the quality of life of spinal patients. Especially in prevention of pressure ulcers, respiratory diseases and osteoporosis. Last chapters are specially devouted to functional electrostimulation of upper and lower limb.

Title: Evaluation of wheelchair adjustments and their impact on proper propulsion pattern upper extremities and sitting posture in patients with spinal cord lesions. Objectives: The objective of this work was to evaluate the adjustments of wheelchair and to determine their impact on the work of the upper limbs and posture while sitting. This work was done on a group of patients with spinal lesions with their own wheelchairs. Another objective was to assess how education of proper propulsion pattern and form and type of a grip of handrim affects the correct propulsion pattern. Methods: In the group of 26 patients with spinal cord injuries in different levels of the spinal cord, I tested the adjusments of wheelchair, in particular the centre of gravity and maximum seat height from the ground. I also investigated what education on correct propulsion pattern the patients received, and observed any home-made adjustments to handrim, as well as patients' techniques of handrim grip. Finally, I examined how the subjects drove his/her wheelchair. During all the tests, I recorded video and photo documentation. The results were statistically evaluated using the Fisher test.
