Peripherally Inserted Central Catheter - a role of nurse

This diploma thesis is related to peripherally inserted central venous catheters (PICC), the role of nursing staff in inserting and care of these systems. The aim of the work, which is partly based on PICC project organized by the Department of Anaesthesiology and Intensive Care Medicine, 1st Faculty of Medicine of the Charles University and General University Hospital in Prague, was to investigate the quality of life in patients with PICC after their insertion, to evaluate the incidence of complications (venous thrombosis, bacteraemia, sepsis, local infections, decubitus, disconnection of the system, mechanical complications, etc.) and compare the economic efficiency of PICC systems with other available methods. The aim of the second part of the thesis, which is unrelated to the previously mentioned project, was to reveal the awareness of nurses/paramedics on PICC catheter. This thesis has two traditional parts - theoretical and empirical. The theoretical part deals initially with the central venous catheter in general and then it describes main features of the PICC systems. Physiotherapy in patients after stabilization of the fracture of proximal part of femur using the dynamic hip screw

The theoretical part of my bachelor thesis is processed anatomy, kinesiology and biomechanics of the hip joint and the proximal femur. Another chapter consists of the distribution of fractures of the proximal femur, prevention, diagnosis, treatment and socio-economic aspects. The final chapter provides physiotherapy and their use in the postoperative period. The practical part consists of two case study of patients with a fracture of the proximal end of the femur treated by osteosynthesis DHS. These patients were hospitalized after the surgery department of traumatology surgery clinic in Hradec Králové. Both case studies include input and output kinesiology examination, determination of short-and long-term treatment plan, record progress and evaluation of treatment outcomes.

Variation of abnormalities of foramen obturatum and retropubic space and its relation to complications of tape surgery

Introduction: The knowledge of anatomy is crucial for introduction of new surgical methods. It is also of especial use while dealing with surgical complications during surgeries with limited surgical field, where the way of approach limits the management of complications. It was assumed that common anatomical variations would influence different efficacy of surgeries and would explain potential complications. Methods: During anatomical dissections fifty female cadavers were dissected and tension-free vaginal tapes TVT-S H, TVT-S U, TVT Abbrevo and Ajust were studied. A novel descriptive system for localisation of the tape was created. During the dissection the tape was located and its localisation and fixation was described. Results: Common anatomical variation in the sample was corona mortis with frequency of 72 %. Preperitoneal fatty plug, which is recognized by some authors as the first stage of obturator hernia, was found in 40 % in obturator canal. The proper fixation of TVT-S H was achieved in 53.6 %. In 10.5 % urinary bladder was injured. In case of TVT-S U the proper fixation occurred in 63.8 %. In two cases the inserter was nearby corona mortis. The Problems of Application of Intramuscular Injection

This bachelor thesis deals with application of intramuscular injections that are carried out by nurses in all areas of health care as their frequent daily activities. The objective of this study was to determine whether nurses have sufficient knowledge of the intramuscular injections use, what needles they choose for application on cachectic and obese patients, what injection site they uses mostly, the degree of their knowledge of the Z - tract method, their experience with complications in practice, and finally whether they observe all the principles of the safe and effective application of the intramuscular injections. Parenteral drug injection, needles, preparing doses from bottles of medicines and ampoules are mentioned in brief in the theoretical part. The applications of the intramuscular injections, stab points, the patient positions for application, application procedures, the method of Z - tract, specific applications and special attitudes to children and possible complications are described in the following chapters. The methodology and organization of the survey are depicted and the research findings are drawn in the empirical part of the thesis. General conclusions and proposals how to remove identified deficiencies are summarized in the end.
