Pilot Testing of Usability "Action Research Arm Test (ARAT)" in Persons after Acquired Brain Injury

BACHELOR THESIS ABSTRACT Name, Surname: Ludmila Hysková Supervisor: Bc. Olga Nováková M.Sc. Title: Pilot Testing of Usability "Action Research Arm Test (ARAT)" in Persons after Acquired Brain Injury Abstract: The bachelor thesis is focused on use of the Action Research Arm Test (ARAT) tool in person after acquired brain injury. The often problem in this population may be decreased or impaired function of upper limbs, which is what the ARAT is dealing with. The aim of the thesis is assessment of the tool and its characteristics. Theoretical part describes different types of brain injury, primarily stroke and their consequences. A chapter about grips and their pathologies follows. Grips are the dominant part of the ARAT testing. The second half of the theoretical part describes testing in occupational therapy and importace of standardised tool. The ARAT is further described in detail. Other tools for testing upper limb function, are briefly described in the conclusion. The practical part deals with pilot testing of the ARAT with the 8 patients. Short case studies of all patients are presented and the results of the individual subtests and total score of ARAT. These results are compared with each other between the patients. This bachelor thesis deals with garden therapy and therapeutic gardens problematics. In theoretical part (literature review) explains garden therapy, for us compared to other countries scarcely used, describing the fundamental principles of the therapy garden construction, including barrier-free usage and introduces us to the obtained brain damage diagnosis. The goal of the research part of the paper is an ideal garden space for patient’s therapy based on studied literature and modeled area analysis. The chosen model object was the Department of Rehabilitation Medicine of the General University Hospital in Prague and the First Faculty of Medicine Charles University in Prague because of the prevailing diagnosis of that facility-the suffered brain damage. In the process of designing a therapeutical garden specialized garden therapy elements were created such as barefoot walking practice, wheel-chair riding practice as well as walking in an ordinary environment and special adjusted bench for handicapped construction. The described draft is documented by an architecture analysis.

Title: The Proposal and Implementation Assistive Devices for Patients with Acquired Brain Injury Using 3D Printer Abstract: Background: Acquired brain injury cause disability, which influences human self-sufficiency. Self-sufficiency can be increased by assistive devices. 3D printing technology seems to be appropriate alternative for creating tailor-made compensatory aids. Foreign organizations are forming the trend. Goal formulation: The main goal is to create tailor-made assistive devices for patients with acquired brain injury using a 3D printer. The partial goal is an assessment of the financial and technical demands of the technology for ordinary occupational therapy practice. Methodology: For five patients with acquired brain injury was created a tailor-made compensatory aid as a part of the bachelor's thesis and printed using 3D printing. The aids were focused on activities of daily living that the patients were not able to perform without the aid. Results: Six self-feeding aids have been created. One aid had to be redone after testing. One patient needed a soft aid that 3D printing cannot produce. The created aids allowed four patients to engage the paretic upper limb in self-feeding. The aim of this work is to map the course of application of each instrument and their comparison in selected areas. Furthermore, the thesis focuses on the use of the instruments in patients depending on various consequences of acquired brain injury. The theoretical part describes various types of acquired brain injury, their consequences and possibilities of rehabilitation. The utilization of CogniPlus system in cognitive-functions training during the occupational therapy for people with acquired brain injury

Aquired brain injury is still a current topic and a serious problem nowadays. After a treatment, a lot of consequences appear which cause a long-term inability to work and invalidation of many people. One of the serious problems is a disorder of cognitive functions which disables people to get back to a normal life. One of the options how to affect the cognitive functions impairment is a system called CogniPlus. Theoretical part of this work presents aquired brain injury, its dividing, basic classification and the consequences. Furthermore, this work dedicates to rehabilitation of cognitive functions using both computers and conventional methods. It also shows the work of the occupational therapist and the possibility of cognitive function testing. The aim of the research is to find out if CogniPlus cognitive function training is more effective than the conventional pencil-paper method. 20 people were tested in this study. Unfortunately, one of them had to quit due to health problems. The experimental and the control group had a therapy once or twice a week. All the participants went through Addenbrooke cognitive test and through a questionnaire of cognitive mistakes at the beginning and at the end of the testing. Home environment should be accesible, barrier free and safe for people with disabilities. There is one possibility how to achieve these conditions the possibility is to let an occupational therapist to assess your home environment. The main parts of occupational therapy are: evaluation of home environment, suggesting of reconstrucitons needed and recommendations about helping aids. Environment is according to the International Classification of Functioning, Disability and Health one of the factors which are very important. The main goal of this diploma thesis was to design new evaluation of home environment of people with acquired brain injury made according to the International Classification of Functioning, Disability and Health. Partial goals were: to identify barriers of home environment, to determine facilitators which are already existing and to suggest helping aids and reconstructions if it is necessary. This diploma thesis deals with monitoring the movement of the upper limbs in patients after acquired brain injury using an accelerometer. The diploma thesis is processed from the perspective of occupational therapy. The thesis is divided into the theoretical and practical part. The main objective of the theoretical part is to collect specialist literature about monitoring the movement of the upper limbs by accelerometer in rehabilitation, especially in the occupational therapy. The practical part of the thesis has three objectives. The first objective is to find out whether monitoring of the upper limbs using the accelerometer in patients after acquired brain injury will lead to an objective improvement of the activity of daily living (ADL) in the areas of eating, washing and dressing. The second objective is to find out whether monitoring of the upper limbs using the accelerometer in patients after acquired brain injury will lead to subjective improvement in the same ADLs, and the third objective is to compare whether objective and subjective improvement is related. Sensorimotor Functions Activation for Patients after Brain Injury from Occupational Therapy. Subtitle: Using Therapeutic Garden

Garden therapy is relatively well known abroad. Even though there are gardens in the Czech Republic that can be used for this kind of therapy, the field itself is not known at all. The goal of my thesis was to create a design concept of a therapeutic garden for patients with brain injury. Part of the design concept are fragments of tactile, olfactory and locomotive training or wheelchair training that a therapist can use for therapy. In order to create a design concept, I evaluated not only the literature but also the answers from a semi-structured interview answered by 3 occupational therapists who have a garden in their facilities. I partially modified the concept using the collected answers. The result of my thesis is a therapeutic garden that corresponds to my ideas and knowledge of occupational therapeuthic field. Two specific sensory aids from the garden environment complementing it's therapeutical use are also included. These aids are for smell and touch training. The main contribution of my thesis is to point out another possible therapy method that occupational therapists might use in practice or multidisciplinary collaboration with garden therapists.
