Specifics of speech and language therapy intervention in adults after stroke
Králová, Tereza ; Horynová, Jana (advisor) ; Klenková, Jiřina (referee)
The thesis focuses on the specifics of speech therapy intervention in adults after stroke in the acute and subacute phase of the disease. The thesis is divided into two main parts, namely the theoretical part and the empirical part. The theoretical part of the thesis defines the concept of stroke and summarizes the knowledge in the field of etiology, symptomatology, diagnosis and prevention. It also defines the individual communication disorders associated with stroke (i.e. aphasia, dysarthria, dyspraxia, dysphagia and cognitive-communication disorders), their classification, aetiology, symptomatology, diagnosis and therapy. The empirical part of the thesis is focused on a qualitative research investigation, the main aim of which is to identify and describe the specifics of speech therapy intervention in people after stroke in the acute and subacute phase of the disease, when the patient is hospitalized in a hospital. The research investigation is carried out by direct observation of the work of a clinical speech therapist in a hospital setting in patients after stroke in the acute and subacute phase of the disease, as well as by analysis of medical reports and documents. From the results of direct observation and document analysis, five case studies of patients hospitalized in the hospital just...
Pre-hospital and intensive care for patients after a stroke
KULHAVÝ, Jakub
The thesis is divided into two main parts (theoretical and empirical) and deals with prehospital and intensive care for patients with stroke. The thesis has a total of three objectives. The first objective was to map selected aspects of the process of providing prehospital emergency care to patients suspected of having an acute stroke. The second objective was to describe the course of intensive nursing care for patients after a stroke, and the third objective was to determine the differences in nursing care between patients with ischemic stroke and hemorrhagic stroke. The empirical part focused on studying medical and, primarily, nursing documentation. The obtained data were processed using a quantitative research method based on retrospective data analysis, which was then interpreted. The data was recorded in record sheets created specifically to adhere to the predetermined objectives and hypotheses. The contribution of the thesis is primarily its unusual approach to the issue, addressing at the same time both prehospital and intensive care for patients after both ischemic and hemorrhagic strokes. The results indicate that crucial aspects of prehospital care include correct and rapid diagnosis, assessment of vital functions, consultation of the condition of a patient with a stroke physician and rapid transportation to the nearest appropriate medical facility. The course of intensive care focuses primarily on continuous monitoring of vital functions and their potential stabilization combined with comprehensive nursing care, which depends on the level of self-sufficiency of a patient and includes all care for basic human needs, primarily aimed at restoring the patient's ability to manage basic daily activities. In summary, it could be said that patients with hemorrhagic stroke required more demanding and comprehensive nursing care compared to patients with ischemic stroke.
Ischemic thrombus analysis in multiphasic brain stroke CT data
Mikešová, Tereza ; Holeček, Tomáš (referee) ; Jakubíček, Roman (advisor)
This master thesis deals with analysis of ischemic thrombus in brain CT scans. In theoretical part, a review of methods, especially thrombus segmentation, is developed. Furthermore, the anatomy of cerebral arteries and acute ischemic stroke is summarized. Selected methods from the field of image processing are briefly described. The practical part results in a comparison of thrombus segmentation methods. The segmentation itself was preceded by data preprocessing, which is described in the theses, and the creation of a manual annotation database. The best implemented method was found to be the adaptive thresholding method, which achieved a Dice score of 0,4555. By combining the methods appropriately, a final Dice score of 0,5145 was achieved. Thrombus parameters were then calculated from the segmented volumes. The median intensity value was 51,55~HU, the median length was 15,16 mm, and the median volume was determined to be 65,34 mm3. Subsequent correlation analysis showed no significant relationship between the derived parameters.

Title: Case Study of Physiotherapy Treatment of a Patient After Stroke Objective: To evaluate the effects of physiotherapy care on the patient. Methods: The first general part provides the description of the desease itself, its pre- valence in the Czech Republic, etiopathogenesis, clinical features, prognosis and current therapeutic approaches in acute, sub-chronic and chronic stages. Recommendations for primary prevention of cerebrovascular accident are described. Post-stroke patients with permanent disabilities run high risk of falling. Some selected methods (tests) are presented to evaluate the patient's balance ability (e.g. Berg Balance Scale, BESTest). The special part of this work presents a case study of a post-stroke patient, the course of the therapy and an evaluation of its effects. Results: The physiotherapy care has led to improvements of gross and fine motor skills and increased the patient's independence in everyday living. Balance ability of the patient is evaluated using the BESTest. Conclusion: It is important to maintain optimal health, independence and self- suffi- ciency of patients in the chronic stage. Specially developed functional tests can be used to evaluate the patient's condition and to choose an effective therapy. Keywords: cerebrovascular accident, brainstem stroke, falls...
The Application of Robot-Assisted Therapy of Hand in Patients after Stroke in Chronic Phase: Follow-up Trial of Application of AMADEO Instrument
Oktábcová, Alice ; Uhlířová, Jaromíra (advisor) ; Kotková, Karla (referee)
Name: Alice Oktábcová Supervisor: Mgr. et Mgr. Jaromíra Uhlířová Oponent: ……………. Diploma thesis: The Application of Robot-Assisted Therapy of Hand after Stroke: Follow-Up Trial of Application of AMADEO Instrument Key words: stroke, robot-assisted therapy, Amadeo Instrument, follow-up study, upper extremity In recent years, western countries noted a gradual increase in stroke survivors. Stroke can cause motor activity impairment and also senzoric and cognitive disorders which lead into restriction of participation of activities of daily living and also reduction of social and community activities. Functional improvment of upper and lower extremity after stroke is a very complex proces which has to be solved in multidiscplinary and multifactorial approach in result of every stroke survivor to live in the most independent way as possible. Robot-assisted therapy for motor recovery could be a successful way to improve motor activity of stroke survivors. Amadeo Instrument is one of robot-assisted therapy for hand rehabilitation of stroke survivors. This instrument can be described as final effector without exoskeleton which allows functional motor therapy for fingers and also a motivational feedback. Amadeo therapy can be passive, assisted, active and also interactive. The aim of this thesis is to...
Neurological manifestations of Fabry disease
Reková, Petra ; Kemlink, David (advisor) ; Karetová, Debora (referee) ; Kešnerová, Petra (referee)
Neurological manifestations of Fabry disease Abstract The presented thesis deals with cerebrovascular manifestations of a rare X-linked disease, Fabry disease (FD). Screening programs can detect unrecognized disease and increase awareness of the disease with the goal of early diagnosis and initiation of therapy before developing an irreversible organ damage. Identifying vascular abnormalities and the influence of comorbidities on the cerebrovascular phenotype of FD patients may help to promote the knowledge of the pathophysiology of cerebrovascular involvement. The main objectives of the studies underlying this thesis were to determine the prevalence of FD and to evaluate the relevance of FD diagnosis in an at-risk group of patients with acute ischemic stroke, to analyse the cerebrovascular phenotype of patients with FD using ultrasound, and to identify predictors of significant cerebrovascular involvement. In this thesis we showed a relatively high prevalence of FD in adult patients with acute stroke, proposed to reclassify the G325S variant, and identified a novel R30K variant. We noted that the combination of aseptic meningitis with lacunar stroke in a young person should lead to suspicion of FD. We further demonstrated multiple differences in neurosonological parameters between patients with FD and a...
