Peripherally Inserted Central Catheter - a role of nurse
Rosypalová, Marta ; Michálek, Pavel (advisor) ; Hocková, Jana (referee)
This diploma thesis is related to peripherally inserted central venous catheters (PICC), the role of nursing staff in inserting and care of these systems. The aim of the work, which is partly based on PICC project organized by the Department of Anaesthesiology and Intensive Care Medicine, 1st Faculty of Medicine of the Charles University and General University Hospital in Prague, was to investigate the quality of life in patients with PICC after their insertion, to evaluate the incidence of complications (venous thrombosis, bacteraemia, sepsis, local infections, decubitus, disconnection of the system, mechanical complications, etc.) and compare the economic efficiency of PICC systems with other available methods. The aim of the second part of the thesis, which is unrelated to the previously mentioned project, was to reveal the awareness of nurses/paramedics on PICC catheter. This thesis has two traditional parts - theoretical and empirical. The theoretical part deals initially with the central venous catheter in general and then it describes main features of the PICC systems. The following part reviews indications, insertion techniques and potential complications associated with the PICC systems. The role of nursing staff in the insertion and maintenance of these systems is also highlighted. The...
Aplication of drugs based on telephone orders
Ptáčková, Eva ; Prošková, Eva (advisor) ; Hocková, Jana (referee)
The topic of this work is the administration of medicine following a face-to-face and telephone madication-related orders at hospitals. The methods used in the theoretic part of this work included both discussions with representatives of health care providers and the analysis of documents. Based on the description and analysis of the set-up of verbal madication-related orders as featured in the accreditation standards of four healthcare accreditation organisations and internal rules of five hospitals, the author of this work seeks to identify any possible risk elements of verbal orders, and to propose ways how to modify such parts of standards and rules so as to make the verbal ordination less risky. Whether or not the verbal medication-related orders at hospitals is a lege artis procedur depends first of all on the situation in which the verbal order is issued, on the authorization or qualifications of health employees who are involved in the implementation of such verbal order, and on the strict observance of procedures concerning the communication (including its recording into the medical documentation). Although verbal madication-related order represents risk arising from the ordinance's communication, forensic, or specialist consequence, no greater attention has been paid to them so far. There...

The thesis describes the metodology and organization of the research investigation. This was a qualitative research, the method of data collection was to create three case reports or case studies with follow discussion. The incidence of postoperative infectious complications in neurosurgical ICU
Brindová, Jana ; Mrákava, Vlastimil (advisor) ; Hocková, Jana (referee)
This thesis examines in detail the nature of postoperative infectious complications in patients who underwent brain tumor surgery. It deals with the incidence of these complications and their severity, depending on various factors and suggests the most appropriate nursing procedures used in the care of a patient in whom these complications occurred. For the structure of the work is divided into two parts - theoretical and empirical. The theoretical part deals with the issues examined in terms of the medical field Neurosurgery (closer diagnoses, conditions, operations and other procedures, and complications to occur after these operations or expected performance), analyzes a nursing practice and patient care on neurosurgical ICU, and finally closely examines infections and nosocomial diseases, including a description of the most common types, their agents, diagnosis and treatment. The second, empirical part focuses on the problems from a practical point of view. With the help of set of case studies it closer which specific complications are most commonly found on the neurosurgical ICU in patients after surgery of brain tumors, what are the most common methods of treatment and nursing procedures which are most used in the care of patients with these complications. In conclusion summarizes the...
Issues of communication between nurses and patients with secured airways at Resuscitation Department
Cvrkalová, Lenka ; Hocková, Jana (advisor) ; Podaná, Jana (referee)
Purpose: Interpersonal communication is one of the most important abilities and skills in everyday life. But when patient's communication ability is limited due to endotracheal or tracheostomy tube insertion, the patient, who had communicated verbally, is not able to convey their feelings, needs and wishes anymore. The purpose of this thesis is to help improve communication skills of nurses in the area of communication with conscious, unconscious and analgosedated patients whose airways are intubated. Methods: The survey has form of quantitative research, the questionnaire method was used. Research data was obtained from a total of 144 nurses working at resuscitation departments at four hospitals in Prague. Results: By studying this issue, it has been found nurses have trouble communicating with intubated patients. The most difficult communication is with conscious patients, who are intubated by endotracheal tube. Communicating with patients with impaired consciousness is also very problematic. Conclusion: Despite the fact, that communication with intubated patients is inherently difficult, more than half of the respondents have never been educated about how to communicate with these patients. Not only but also to address this issue, the outcome of this thesis is information brochure with...

Nursing Care for Patient after Surgery of Valvular Defects
Beranová, Veronika ; Burišková, Klára (advisor) ; Hocková, Jana (referee)
This thesis is focused on the nursing care for patient after valvular defects surgery. The aim of this final paper is to ascertain the principles of specialized nursing care for patient after valvular defects surgery, analyse the condition of written standards for providing aftercare to patients who underwent valvular defects surgery, or the state of the nursing protocols in specialized nursing care. The purpose of qualitative - observational research in providing specialized nursing care is to find an answer to the question of whether the specifics of nursing care in Prague cardiac centres are significantly different. The theoretical section contains chapters describing cardiac centres, the history of surgical treatment of heart valves, and the most common valvular heart defects in adulthood. In the theoretical section of this study, I focus in detail on the nursing care for patients following cardiac procedure, ensuring not to forget the monitoring of physiological functions, artificial pulmonary ventilation, and immediate position after elective cardiac surgery. The empirical section is divided into quantitative and qualitative research. The quantitative empirical investigation is focused on an anonymous questionary survey that has been applied in three Prague cardiac centres. Approximately 150...

Permanent Vascular Access for Haemodialysis with Regard to Diabetes Mellitus Diagnosis
Sajdlová, Zuzana ; Eislerová, Iva (advisor) ; Hocková, Jana (referee)
This bachelor thesis deals with the issue of permanent vascular access for hemodialysis with regard to presence of diabetes mellitus (hereinafter DM). It focuses on peripheral hemodialysis vascular access, which include the arteriovenous fistula (AVF) and arteriovenous fistula created using synthetic material, such as graft (hereinafter AVG). The main objective is to highlight the fact that the vascular system is often affacted by systemic diseases like diabetes or atherosclerosis and creating AVF in such patients is often a complicated matter. By intensive postoperative monitoring imminent complications may be detected and prevented. The research is focused on peripheral HCP (AVF, AVG) and on detecting differences in dependance on their individual representation and their functional characteristics at 40 patients treated in Department of Internal Medicine Strahov, General Teaching Hospital Prague. The average age was 70 years. The first group consisted of 21 patients with DM and a the second group 19 non-diabetic paients. AVF is preferable to AVG even in patients with DM. The actual values of blood flow were measured by a team of nurse and clinical engineer and calculated using special formulas from reagular measurements in both groups of respondents. The obtained values of acutal blood flow have...
