Normotenzní hydrocefalus (NPH) představuje důležitou součást diferenciální diagnos- tiky příčin kognitivního deficitu, poruch chůze a inkontinence u pacientů vyššího věku. To platí především pro normotenzní hydrocefalus idiopatický (iNPH), kterému je tato práce dominantně věnována. Průběh onemocnění ovlivňuje řada faktorů, proto komplexní pří- stup hodnotící více proměnných doprovázející toto onemocnění, onemocnění komorbidní či zvažované v diferenciální diagnóze, ale i komplexní přístup k terapii, která v sobě skrývá mnohem více než jen implantaci derivačního systému pro odvod mozkomíšního moku, může zlepšit kvalitu života těchto pacientů. Pilířem předkládané disertační práce jsou dvě prvoautorské práce. První z těchto studií se zabývá morfologickými parametry získanými při MRI vyšetření a jejich schopností predikovat zlepšení v chůzi po implan- taci zkratu u iNPH pacientů. Zhodnotili jsme prediktivní výkonnost skóre pro hodnocení hydrocefalu s disproporčně zvětšenými subarachnoidálními prostory (DESH skóre), úhlu corpus callosum a příznaku cingulárního sulku oproti referenčnímu standardu v podobě funkčního vyšetření - lumbálního infuzního testu (LIT) a vnější lumbální drenáže (ELD). Ve druhé práci jsme sledovali změny ve všech hlavních symptomech iNPH pomocí pestré palety vyšetření u pacientů,... Normal pressure hydrocephalus (NPH) is an important part of the differential dia- gnosis of the causes of cognitive deficit, gait disorder and incontinence in elderly patients. This is especially true for idiopathic normal pressure hydrocephalus (iNPH), to which this thesis is dominantly devoted. A number of factors contribute to the course of the disease, therefore a comprehensive approach evaluating multiple variables accompanying the disease, comorbid diseases or considered diseases in the differential diagnosis, as well as a comprehensive approach to therapy, which contains much more than just the im- plantation of a derivation system for the drainage of cerebrospinal fluid, can improve, not only, the quality of life of these patients. The mainstay of the dissertation thesis are two first-author articles. The first study deals with morphological parameters obtained during MRI examination and their ability to predict improvement in gait after shunt implantation in iNPH patients.
