Shrnutí Hodnota HRCT v diagnostice plicních komplikací maligní hematologických onemocnění. Včasná diagnóza je jednou ze základních podmínek pro úspěšné terapii malignit a jejich komplikací. Zobrazovací metody hrají důležitou roli v jejich odhalování a řízení léčby pacienta. HRCT je metodou, která nejvíce přispívá v diagnostice plicního zapojení, které je velmi běžné v této skupině pacientů. Cílem této práce bylo zjistit potenciál této metody v diagnostice plicních komplikací u pacientů s hematologickou malignitou. Za tímto účelem byly posouzeny 4 cíle: 1) srovnání mezi HRCT a hrudníku X-ray, 2) stanovení schopnosti HRCT, aby konkrétní diagnózu, 3) definice indikačních kritérií pro HRCT a 4) přidělení vhodné vyšetření technika. V období od ledna 2000 do prosince 2005 bylo provedeno 214 HRCT vyšetření plic ve skupině 162 pacientů s hematologickými malignitami a plicní symptomatologie. 176 vyšetření bylo ve srovnání s hrudníku X-ray, která byla provedena 48 hodin nebo méně před HRCT. Plicní léze byly objeveny ve 180 případech (84,1%). 123 nálezy byly potvrzeny jinou metodou (odpověď na určitou terapii, kultivaci, sérologie, BAL nebo histologie). Infekce léze byla diagnostikována v 77 případech (62,6%). Zbývající 46 (37,4%) léze non-infekční. Mezi častými infekcemi léze byly mykózy (58,7%). Nejčastějším mykóza... Value of HRCT in the diagnostics of pulmonary complications of malignant haematologic diseases. Early diagnosis is one of the essential conditions for successful therapy of heamatologic malignancies and their complications. Imaging methods play an important role in their detection and patient's management. HRCT is the most contributing method in the diagnostics of pulmonary involvement, which is very common in this group of patients. The aim of this thesis was to find out the potential of this method in the diagnostics of pulmonary complications in patients with haematologic malignancy. For this purpose were assessed 4 objectives: 1) comparison between HRCT and chest X-ray; 2) determination of capability of HRCT to make a specific diagnose; 3) definition of indication criteria for HRCT and 4) assignment of the suitable examination technique. In the time period from January 2000 until December 2005 were performed 214 HRCT examinations of the lungs in the group of 162 patients with haematological malignancy and pulmonary symptomatology. 176 examinations were compared with chest X-ray, which was performed 48 hours or less before HRCT.
