Effects of proprioceptive neuromuscular facilitation on the upper limb function of people with C4-C5 spinal cord injury

Title: Effects of proprioceptive neuromuscular facilitation on the upper limb function of people with C4-C5 spinal cord injury The main objective: The aim of the work was to find out what effect the use of the proprioceptive neuromuscular facilitation method will have on the function, muscle volume and ranges of movements of the upper limb in people who are affected by a spinal cord lesion in the C4 and C5 region. Furthermore, the work dealt with the subjective feeling of the patients, whether the upper extremity improved after the therapy and if it was possible to involve it more in ADL activities. Methods: 5 probands took part in the study - all men with an average age of 41 years. A total of 3 measurements were performed for the study participants - approx. 2 months before the start of the intervention, before the start of the therapy and after the end. Values such as the volume of the arm muscles in contraction, the range of motion of abduction in the shoulder joint and flexion in the elbow joint were recorded. Another measured subject was the forearm endurance test. Participants also filled out the SCIM form before and after graduation. Title: Cooperation between Ocupational Therapist and Physiotherapist in the Training of Personal Activities of Daily Living with Patients with Cervical Spine Lesion Abstract: This thesis explores some aspects of collaboration between occupational therapist (OT) and physiotherapist (PT) in rehabilitation of patients with complete spine lesion in level C6-C8. This group of patients has the functional potential to cope with activities of daily living with the use of compensation aids and strategies in an adapted enviroment. To fulfil this potential intense intervention of OT and PT is needed. The goal of this thesis was to map this cooperation in training of self-sufficiency in daily tasks during subacute and chronic phase of rehabilitation. As a representative sample 3 institutions were chosen, where patiens are treated during the spinal program. These institutions are: Spinal Unit in Faculty Hospital Motol, Rehabilitation Centre Kladruby and NGO Centrum Paraple. Information gained through observation in the practical trainings in these institutions and data collected from the survey were systematically processed and offer a view of some aspects of interprofessional cooperation. Training for autocatheterization in patients with spinal cord lesions from the perspective of an occupational therapist

This bachelor thesis deals with the rehearsal of self-catheterisation by spinal lesion patients from the occupational therapist's point of view. The aim of the thesis is to create a selfcatheterisation manual for patients after spinal lesion, which might be beneficial for paraplegic as well as tetraplegic patients. The manual contains information about the self- catheterisation procedure and offers instructions, methods and alternatives of compensatory aids and strategies for becoming independent in one of the ADL activities - urinary bladder emptying. The thesis is divided into a theoretical and practical part. The theoretical part clearly explains the problems of tetraplegia and the level of disability and analyses the effects of spinal injury to urinary bladder functioning. The practical part is concerned with the benefits of the Self-catheterisation Rehearsal by Spinal Lesion Patients from the Occupational Therapist's Point of View manual which were examined through a qualitative research. Kateřina Rybářová Title: Problems Coping with Household Care by People with Transversal Spinal Cord Injury from Perspective of an Occupational Therapist Abstract: This bachelor thesis considers problems of people with transversal spinal cord injury coping with household care from perspective of an occupational therapist. The aim of the thesis is to qualitatively compare how men and women with transversal spinal cord injury in C6 level cope with undertaking household chores. The theoretical part describes difficulties of spinal cord injury and household chores and sets out possible occupational therapy intervention. It clarifies options of adapting household chores for those with transversal spinal cord injury in more detail. The practical part is set out by case reports of two client groups (male and female) with transversal spinal cord injury in C6 level. It introduces anamnesis, examination by occupational therapist, appraisal of independence using the SCIM and appraisal of the home environment, by using the methodology SEMAFOR home. Influence of cervical spinal cord lesion on cardiovascular fitness

Traumatic damage to the cervical spinal cord has a dramatic impact on determinants affecting cardiovascular fitness. In tetraplegia, in addition to diminished skeletal muscle function, impaired sympathetic cardiovascular innervation together with a dysfunction of catecholamine release is typically present. Individuals with tetraplegia are at a high risk of long-term inactivity, which increasingly contributes to the development of common civilization diseases. The aim of the current dissertation project is to investigate the cardiovascular fitness in males with cervical spinal cord injury (n=20) as compared to able- bodied males (n=27). Both groups completed exercise testing on an arm-crank ergometer. Arterial blood pressure response to exercise, peak oxygen consumption and peak heart rate were the primary outcome variables. In the first part of the study, a graded maximal arm- crank ergometer test to the subjective exhaustion was performed for the inter-group peak exercise comparison. The peak power dramatically differs between the groups and could be an important confounding factor limiting the evaluation of a physiologic response related to the equivalent physical loading. A subsidiary aim of this thesis is a survey of the Czech market with electric wheelchairs, the selection of several different models suitable for tetraplegics, their assessment by an occupational therapist and recommendations to certain groups of clients. When it comes to the case studies, this theoretical information is applied in the clinical practice. The Influence of upper-limb key muscles strength on self-sufficiency of tetraplegics

This bachelor thesis concerns the influence of upper-limb key muscles strength on self-sufficiency of tetraplegics. The paper is divided into a theoretical and a practical part. The theoretical part contains observations on spinal cord damage as well as health consequences and complications of tetraplegia, which were subjected to closer scrutiny. Furthermore, the paper focuses on self-sufficiency of tetraplegics and on the influences which affect it. It offers a thorough description of the available research on the relationship between muscle strength and self-sufficiency. The practical part describes and compares case reports of two probands of the same motor level of tetraplegia and of a similar strength of upper-limb key muscles. Self- sufficiency of the probands is attested by SCIM and a detailed description of the strategy of transfer from a wheelchair to a bed and back. The use of surface electromyography for functional diagnostic of upper limb in spinal cord injury patients an occupational therapy perspective

The aim of this thesis is to determine the use of surface electromyography for functional diagnostic of upper limb in spinal cord injury patients from an occupational therapy perspective. Functional diagnostic is focused on personal activities of daily living (pADL). So far, no one has commented this topic from an occupational therapy perspective in the Czech Republic. The thesis should offer another possibility of objective evaluation. The theoretical part deals with the description of upper limb, including kinesiology, the types of grasps of quadriplegic patients and the muscles involved in performing personal activities of daily living. There are problem areas of activities of daily living in spinal cord injury patients and their classification. At the end of the theoretical part is described electromyography, interconnection with occupational therapy and use of surface electromyography. There is also described interdisciplinary team. The practical part consists of six case reports, in which subjects were measured muscle activity during performing personal activities of daily living using surface electromyography.
