The Importance of Verification Images during IMRT in the Area of Head and Neck.

Tumours of the head and neck are the fifth most common cancers in the Czech Republic. Despite easily available medical examination and observable symptoms most patients come for treatment with various types of tumours in advanced phases which lowers their chance of being cured. The technique of IMRT has recently become the most often used curative method for the tumours of the head and neck, enabling better treatment of healthy tissues and critical organs in this area. The thesis is divided into the theoretical and practical part. In the first part, the areas of head and neck and the types of cancers found in these areas are analysed. The chapters concern symptoms, aetiology, diagnostics, prognosis and, of course, their treatment. Further, the common course of planning has been included and at the end of this part the history and presence of verification images is mentioned. The second part of this thesis includes objectives and a hypothesis. The objective of the thesis is to explore and evaluate portal verification images of target volume in tumours of the head and neck and compare these images with referential verification images as well as to state deviations in irradiation position and to prove the significance of verification images in the area of head and neck. This part also includes chapters describing methodology (with resources), and the course planning process in a patient using IMRT technology. Further, it includes the research result of the a collection of 50 patients with the tumours of the head and neck who were irradiated by IMRT technology and whose verification images of target volume were evaluated. The predicted objectives were fulfilled and the hypothesis was disproved. Verification portal images are made in oncology department once a week. I would suggest making verification portal images with a fixation mask at least twice a week in order to achieve better accuracy and control of the irradiation position.

Prostatic cancer and possibilities of prediction of early success of radical prostatectomy

Classification of adenocarcinoma prostatae is very important for the treatment. I have described the disease, its treatment and classification in the introduction. I have analysed a sample of 52 patients from the Urological Clinic of General Faculty Hospital. Simple statistics was used for comparing and analysing of relation of preoperative classification and postoperative development of PSA. I have compared preoperative and operative Gleason's score. Preoperative Gleason score underscores the operative score. PSA decrease is significantly higher in patients with high initial PSA and in patients with classification T2N0M0. PSA one month after surgery does not differ in patients with classification T1C and T2 and it does not correlate with the preoperative characteristics of patients. The decrease of PSA is perhaps dependent only on the surgical treatment. Effect of operation cannot be predicted by Gleason score and by age. These results are valid only for patients undergoing radical prostractomy

Thesis on "Expert systems analysis of insurance fraud in vehicle area" deals with insurance fraud in the Czech Republic, including the introduction of the legal process and the general division of insurance fraud and the realities of dealing with insurance losses. The practical part of the thesis deals with expert analysis of solution of claims, including detection and prediction of insurance fraud. The aim of this thesis is to provide statistical analysis of insurance fraud, ie. Evaluation of variations and correlations and statistical dependencies of various markers of insurance fraud in the Czech Republic.

The importance of tumor markers and markers of angiogenesis for colorectal cancer prognosis

Despite extensive progress in understanding of the molecular pattern of the tumor, growing possibilities of chemotherapy and surgical treatment, colorectal cancer remains the significant cause of death worldwide. Surgical treatment alone has already reached in oncologic radicality its limits, and therefore the surgeons turned to the molecular base of tumor growth in an effort to find markers that allow to operate patients at lower stages of the disease and thus improve the results and prognosis of the disease. And also to find markers, which, after currative operation will find a possible relapse in early stage to enable the surgeon to perform second curative intervention. Tumor markers are substances that often arise in connection with the changed metabolism transformed tumor cells, and therefore their levels increased in the presence of malignancy. Serum tumor markers are important parameters that may facilitate the prediction of the disease, its progression or regression (remission). Assessment of the Cathetrization Programme in a Haemodialysis Centre

This diploma thesis deals with complex quality assessment of catheterization programme run at Strahov Internal Medicine Department of the General Faculty Hospital in Prague. The evaluation was done according to K/DOQI 2006 international guidelines. The aim was to evaluate primary success of catheterization, probability of maintaining catheter functionality in time, the risk of its thrombotization and presence of infection. The evaluation was performed using Kaplan-Meier method. Moreover, the effect of additional enhancements for maintaining catheter functionality (TEGO Connector, Biopatch, heparin and Citra-LockTM) and the predictive value of selected hematologic parameters of long-term functionality of the catheters (hematocrit, plasma D-dimers) was assessed. The latter was performed on a small sample of patients. The possibility of detection of the catheter point position by measuring recirculation and its effect on dialysis was investigated by comparing X-ray control of the catheter point position to the value of recirculation measured on catheters connected either normally or inversely to the extracorporeal circulation device. Genetic Aspects of Sporadic Ovarian Cancer

SUMMARY Molecular biological parameters, including genetic alterations, present new and perspective direction in diagnostics, prediction of prognosis, monitoring and possible therapeutic approaches in oncological disease. The results presented have to do with the project, which is concentrated upon chromosomal rearrangements in ovarian and cervical cancer and their correlations with available parameters of both molecular biological and clinical characteristics. Sixty patients with ovarian cancer and twenty patients manifesting cervical cancer were included into the study. The histological type and grade, MIB-1 and p53 (using immunohistochemical method) were estimated by histopathologist. Both conventional karyotyping and molecular-cytogenetic methods (fluorescent in situ hybridization and comparative genomic hybridization) were applied to reveal chromosomal aberrations. The results were subjected to statistical evaluation, using analysis of variances and χ2 test. There were correlated parameters of quantitative and qualitative character - age, stage, histological type, grade, CA 125 before and after treatment, MIB-1, p53, surgical residuum, lymphadenectomy, response rate, chromosomal rearrangements. Molecular Signature as Optima of Multi-Objective Function with Applications to Prediction in Oncogenomics

Náplní této práce je teoretický úvod a následné praktické zpracování tématu Molekulární signatura jako optimální multi-objektivní funkce s aplikací v predikci v onkogenomice. Úvodní kapitoly jsou zaměřeny na téma rakovina, zejména pak rakovina prsu a její podtyp triple negativní rakovinu prsu. Následuje literární přehled z oblasti optimalizačních metod, zejména se zaměřením na metaheuristické metody a problematiku strojového učení. Část se odkazuje na onkogenomiku a principy microarray a také na statistiku a s důrazem na výpočet p-hodnoty a bimodálního indexu. Praktická část je pak zaměřena na konkrétní průběh výzkumu a nalezené závěry, vedoucí k dalším krokům výzkumu. Implementace vybraných metod byla provedena v programech Matlab a R, s využitím dalších programovacích jazyků a to konkrétně programů Java a Python.

Possibilities of preoperative determination of local extent of prostate cancer

Rheumatoid arthritis (RA) and osteoarthritis (OA) represent the most common forms of musculosceletal disorders that affect diarthrodial joints, lead to joint damage and disability. Extra-articular manifestations accompanied the joint disease only in RA. Diagnosis of both conditions most commonly bears on the conventional radiography. Mostly in OA, radiographic changes often occur late in the disease and are largely irreversible. Molecular markers could reflect joint damage, inflammation, or immune response. Current investigation revealed potential uses of molecular markers, ranging from understanding pathogenesis of the diseases to predicting and monitoring the outcome of the treatment. The aim of the thesis was to analyze several biochemical markers in serum, synovial fluid and synovial tissue samples from patients with RA and OA. and to evaluate their diagnostic and predictive values as well as their contribution to the pathogenesis of the diseases. This thesis analyzes Frentech Aerospace s.r.o. company since the year 2005 to 2010. Analysis of the company is divided into two parts. First part is dedicated to financial analysis with selected economical markers. Second part is dedicated to the analysis of selected datas for the monitored time with statistic methods. In this part is also a prediction of company development in the years 2011 and 2012. Thesis includes designed solutions for improvement of the company's financial situation.
