Physiotherapy in children with cystic fibrosis

The theme of this thesis is children´s cystic fibrosis and the impact of using the methods of physiotherapy. The thesis consists of theoretical and practical part. In the theoretical section is summarized the findings of cystic fibrosis, its history, diagnosis and treatment. Next, in theoretical parts are included the methods of respiratory physiotherapy and other methods of physiotherapy that can positively affect the lives of individuals with cystic fibrosis. Cystic fibrosis is a serious life-threatening disease. Currently there is no effective treatment leading to a complete cure. This disease is characteristic of excessive formation of thick mucus, which most frequently occur in the organs of the respiratory and digestive systems. Thick mucus is the basis for the emergence of various microorganisms which an individual is suffering from cystic fibrosis threatened. The main symptoms of cystic fibrosis are chronic respiratory diseases manifested by cough, expiratory dyspnea and a feeling of inability to breathe. Another typical symptom is a high concentration of electrolyte salt in sweat that causes patients salty taste of sweat. An important aspect is the early diagnosis of disease and early treatment. The key is to combinate pharmacological therapy with daily implementation methods of respiratory physiotherapy. Complementary forms of treatment may be the use of physiotherapy; it can significantly boost the quality of life of the individual. In the practical part is used qualitative research method, case study. The research was conducted in the home of the patient. The research sample was consisted of one patient diagnosed with cystic fibrosis, which was monitored over a period of several months. In the beginning was made entry kinesiology analysis, from which unfolded subsequent individual therapy. This was mainly focused on defective posture and partially covers methods of respiratory physiotherapy. At the end of therapy was conducted final kinesiology analysis. The results are processed in the form of case studies.

Role of Respiratory Physiotherapy for Patients suffering for adenoid hypertrophy

The bachelor's thesis deals with the adenoid hypertrophy problems. It is divided into a theoretical and a practical section. The theoretical one consists of a general and a special part. The theoretical section summarizes anatomical knowledge related to upper airway, describes some of the respiration physiology aspects and it also deals with the most frequent upper airway pathologies. The special part goes into details about the adenoid hypertrophy problems. It describes symptomatology, diagnostics and therapy possibilities. It widely investigates physiotherapy possibilities, breathing re-education in particular. The practical part contains outputs of a questionnaire research which took place at the ORL children clinic of the 2nd Faculty of Medicine, Charles University. It gathers information about each aspect of the problems. Based on the questionnaire research, an examination and a training of a selected group of patients suffering from adenoid hypertrophy, it compiles a possible training methodology and offers an information brochure to parents

The effect of physiotherapy on cardio-respiratory parameters in patients with chronic heart failure indicated for cardiac surgery intervention

The bachelor thesis deals with the preoperative physiotherapy in patients with chronic heart failure who are indicated for cardiac surgery intervention and with the description of physiotherapy procedures utilized before and after the operation. The cardiovascular disease is in developed countries still the main cause of morbidity and mortality. (UNIFY, 2008) The improved treatment of acute conditions allows so that more patients reached a stage of the chronic heart failure. (Špinar, Vítovec, Hradec, 2012) The chronic heart failure is a sign for a number of symptoms that are caused by disruption of the heart work. It is a disorder in which despite adequate ventricular filling drops a cardiac output and the heart is unable to cover metabolic needs of the tissues (supply of oxygen and nutrients and removing of carbon dioxide and metabolic waste products). (Štejfa a kol., 2007) In the theoretical part is described the anatomy of the heart, the physiology of the heart and the blood circulation, the chronic heart failure, the response and the adaptation of the organism to physical load, the cardiovascular rehabilitation and physiotherapy procedures carried out before and after the operation. The practical part of the bachelor thesis is performed by method of the qualitative research with the using techniques: the anamnesis, the kinesiology analysis and the data analysis. The research set was formed by 2 patients of the cardiac surgery department of Hospital České Budějovice a. s. who were indicated for cardiac surgery intervention. The total time of the research was 4 weeks. The first purpose of this thesis was chart the effect of the preoperative physiotherapy on cardio-respiratory parameters and the condition of the patient. The second purpose was describe physiotherapy procedures carried out before and after the operation. I chose 2 research questions for the research: 1. What effect has the preoperative physiotherapy on cardio-respiratory parameters in patients with the chronic heart failure? 2. What are possibilities of physiotherapy procedures before and after the operation? The preoperative physiotherapy was carried out 5 times a week for 1 month in the home of the patient. It included a physical therapy, a respiratory physiotherapy and relaxation methods. Selected cardio-respiratory parameters were taken before and after each training unit. Results of therapy are processed through case studies. The component of the bachelor thesis is also a list of references, key words and abbreviations. Next it includes image attachments and graphs. The thesis can be a benefit for physiotherapists, general and professional public and for possible further research in this issue.

Physiotherapy for recurrent respiratory diseases of infants in home environment

This thesis deals with physiotherapy for recurrent respiratory diseases of infants. Respiratory diseases belong to the most common childhood diseases. Therefore it is necessary to pay attention to this issue. The first two goals of this thesis are to cover an issue of respiratory physiotherapy in infants, both theoretically and practically. The third aim is to create a visual guide for parents of infants with respiratory diseases. To achieve these goals, it was necessary to study literature and to become familiar with the therapy in practice. Methods of qualitative research including direct observation techniques, casual interview with mothers, secondary analysis of data and taking photographs were applied in this research. The research group consisted of three infants with respiratory diseases, who come into the care of the pediatrician prof. MD. M. Velemínský, PhD., Dr. H. C., prof. H. C. Indirect anamneses were taken and through aspecting I assessed their postural activity. During three visits over the time horizon of 10 days, mothers of infants were gradually familiarized with the application of techniques of respiratory physiotherapy in order to realize the therapy in the home environment. On the basis of briefing I wrote a manual. I would like to inform the general public about the possibilities of this treatment. I tried to trace the responses of the infants to the application of the techniques of respiratory physiotherapy, to find a correlation between the character of their breathing and the postural activity and, last but not least, I focused on the problems that appeared during the training. This thesis can be beneficial not only for parents of the infants with respiratory diseases but also for the students of physiotherapy. I believe, that the manual I made could above all facilitate the parents of the ill children to participate on their treatment.

The problems of Pediatric Thoracic Surgery from the view of respiratory Physiotherapy

The bachelor thesis is focused on pediatric surgery in thoracic area and its dilemma. The theoretical part contains knowledge about anatomy, physiology and kinesiology of developing organism. It is followed by overview of the most common diagnoses indicating thoracic surgery, describing each of those surgeries individually. The biggest issue of entire pediatric thoracic surgery is the risk of postoperative complications, which is also discussed in the thesis. This part is then followed by discussing means of physiotherapy. The informations are divided into several parts, each describing means of physiotherapy used in different time sequences. Practical part contains general means of examination and physiotherapic approach depending on patient's age. The last part is devoted to casuistry, which clearly demonstrates issues related to pediatric thoracic surgery from the physiotherapeutic point of view.

BASAL STIMULATION IN A COMPREHENSIVE REHABILITATION

This bachelor thesis deals with the system of comprehensive rehabilitation and specifying its importance for physiotherapy. The theoretical part describes the system of comprehensive rehabilitation. It also focuses on the concept of basal stimulation, introduction of its origin, its current state in the Czech Republic and abroad, its nature, idea, general principles and its place in the system of comprehensive rehabilitation. Then the subject of interest of this concept is characterized, which is to promote the perception, mobility and communication of the client. When describing the elements of basal stimulation, those techniques are emphasised which, according to the obtained information and documents, might be used in physiotherapy. A separate chapter is devoted to physiotherapy for selected diagnosis, in which the basal stimulation is applied. Principles, procedures and applied concepts and methods are briefly summarised here. The practical part describes the process of research carried out in the day care centre Arpida o.s. in České Budějovice. The aim and the objective at the same time were to describe basal stimulation as the concept of comprehensive rehabilitation and to specify its importance for subsequent physiotherapy. This was done by qualitative research and created case studies of individual respondents, when information were taken from recorded medical history, biographical history, kinesiological analysis, evaluation of spasticity and examination of respiratory stereotype. Also information from the area of basal stimulation in the day care centre and information about the progress of physiotherapy were obtained. Subsequently the basal stimulation techniques were interconnected with the physiotherapeutic methods and concepts and the results were thoroughly recorded and evaluated. The results confirmed the positive influence of this scientific pedagogical ? nursing concept and the appropriateness of combination of its techniques with physiotherapy.

Possibilities of physical therapy in patients with chronic form of respiratory diseases at the pulmonary disease sanatorium

The aim of the thesis is to prove that active techniques of respiratory physiotherapy together with aerosol inhalations contribute effectively to treatment of acute and chronic respiratory problems and also to prove that this therapy increases patient's motivation to realize the importance and effectiveness of respiratory exercises . Benefits of above-mentioned therapy influence patient's professional life. Quality of life has also impact on family and social relationships of respective patient. The influence of aerobic training and respiratory physiotherapy upon pulmonary functions of the patiens with cystic fibrosis

The theoretical part of the dissertation covers the theoretical information about Cystic Fibrosis and an overview of anatomy and physiology of respiratory mechanism. Since the dissertation is rather aimed at the practical work, it further describes exercise physiology and the techniques of respiratory physiotherapy. The purpose of the dissertation is to find out the effect of a specific training scheme upon respiratory functions in selected individuals with Cystic Fibrosis, who are diagnosed with different levels of obstructive ventilatory defect. The training scheme includes an aerobic exercise on bicycle ergometers in combination with respiratory physiotherapy. The spirometry test results have shown that the respiratory functions in all pacients have improved.

Respiratory physiotherapy - part of a comprehensive treatment of Duchenne muscular dystrophy patients

Duchenne muscular dystrophy is a progressive muscular disease. This serious disease attacks not only the musculoskeletal system, but also the respiratory system. Respiratory system is one of the most influenced systems. The risks of respiratory problems increase with deteriorating condition of the patient due to muscle weakness. This work is focused on treatment of respiratory system. The aim of treatment is early prevention and solving of complications. These complications include not only scoliosis and cardial problems, but mainly inefficient cough, night hypoventilation, irregular night breathing and failure of respiratory system even during the day. The aim of the respiratory physiotherapy is to keep the right functionality of lungs for the longest time and to choose the correct method of the rehabilitation following the patient's condition deteriorating. Interventions suitable for the patients with the DMD are techniques that leads to increasing lung volume, manual and mechanical techniques of assisted coughing, and noninvasive ventilation. NIV is one of the preferred methods of treating nowadays. Another method is continual invasive ventilation and ventilation with tracheostomy. PHYSIOTHERAPY OF PREGNANCY AND PUERPERIUM

The bachelor thesis is focused on physiotherapy during the time of pregnancy and puerperium. Both pregnancy and birth have a significant influence on the expectant mother. During the nine months, woman undergoes a great amount of changes both from the physical and the mental side. This bachelor thesis deals foremost with changes that affect the physical side of the expectant mother. They include changes on genital organs, on cardiovascular, blood and respiratory system, gastrointestinal tract, skin and last but not least on the locomotory system. With exercise in gravidity, we try to influence these changes and, therefore, avoid any occurrence of eventual difficulties. Exercising during gravidity has not only a great importance to prevention but also to the preparation of an expectant mother to the delivery and for acceleration of return of the mother into the shape before conception. The expectant mother should be careful when choosing a suitable sport and she should always consult her attending gynaecologist. Recommended sports and methodologies include gravid yoga, pilates, belly dance, Moses method, exercise on a big ball, Alexander's technique, hydrokinesiotherapy and gynaecological gymnastics. The theoretical part includes a description of anatomy of genital organs and pelvis, creation of an individual, description of physical changes during pregnancy and puerperium and also characteristics and selection of individual suitable sport activities and methodologies. The goal of this bachelor thesis was to find such suitable physiotherapeutic procedures and methods, apply them to particular expectant mothers and verify their effects. For the practical part of research, the method of qualitative research and personal cases studies were used. Three pregnant women participated in the research. Therapy took place once per week for the period of several months. While having free conversation with the clients during the said period, I obtained the information I have incorporated into the individual casuistries. The therapy was completed by the final examination, which proved a positive effect of the exercise not only on the health conditions of expectant mothers, but also on their mental well-being, which was especially important for mothers.
