Vestibular rehabilitation efect of patients with postural stability impairment

Disorders of postural stability seriously impair the quality of life. Falls are more common in the elderly, where they can have very serious consequences. However, this issue does not always concern only seniors. These disorders can be caused by a number of reasons. The diploma thesis is devoted to postural stability in patients with vestibular disorders. The aim of the theoretical part of the work is to present comprehensive information on the control of postural stability, its disruption and the integration of sensory systems needed for its maintenance. Further chapters describe postural control assessment and rehabilitation options. The aim of the experimental part was to determine the effect of vestibular rehabilitation on postural control. The evaluation was carried out with the help of a static stabilometric platform Kistler®. 9 patients with postural stability disorder in the chronic stage participated in the research part. A total of 5 men and 4 women, aged from 35 to 78 years, on average 59 with SD ± 16 years, with peripheral (4), central (4) or combined (1) vestibular disorders were exposed to a 3- month rehabilitation program. Markéta Křivánková Year of the presentation: 2023 Objectives: To compare the results of available studies using balance boards and neurocentric approach in restoring stability and to explain their principles. Methods: Search of Czech and foreign literature using the FTVS library, PubMed, Google Scholar and ScienceDirect. The practical part uses a method of comparison. Results: The effectiveness of balance boards in restoring stability after ankle distortion has been demonstrated in several studies. On the other hand, its influence is questioned by two of the selected studies which did not show any positive results from using balance boards on stability. Some of the results have been evaluated by questionnaires or stability measurements on an unstable surface, so it is not possible to determine exactly whether the use of balance aids improves stability on a stable surface and is therefore appropriate when returning to sport. Use of Mobile Applications in Vestibular Rehabilitation

The bachelor's thesis focuses on the use of mobile apps in vestibular rehabilitation. The main purpose of this thesis is to provide market research of mobile apps currently available at Google Play Store. Additional goal is to provide an overview of current literature relating to the use of the mobile apps in vestibular rehabilitation. Theoretical section summarizes knowledge of the anatomy and physiology of the vestibular system, vestibular compensation mechanisms and vestibular rehabilitation practice. It also provides a brief information about mHealth apps and rehabilitation apps. Following topic is a retrieval of the literature scoping mobile apps in vestibular rehabilitation and a search of mobile applications available on the Google Play Store, that are or may be intended for use in vestibular rehabilitation. Experimental section consists of two case reports of patients with vestibular hypofunction using mobile app for 4-week therapy. The Effect of Vestibular Rehabilitation in Patients with Peripheral Vestibular Disease

Dizziness is one of the most common reasons for visiting a doctor. Their common cause is a peripheral vestibular lesion. Recently, more and more evidence is emerging regarding the effect of vestibular rehabilitation in these patients. The aim of the theoretical part of this thesis was to provide a comprehensive review of information on peripheral vestibular lesions with an emphasis on the possibilities of physiotherapeutic intervention. Individual peripheral vestibular lesions, vestibular rehabilitation, principles of its functioning and mechanisms of vestibular function repair were described in detail. The main aim of the experimental part of the study was to evaluate the effect of vestibular rehabilitation in patients with peripheral vestibular lesions. Stance and gait stability and the patient's subjective perception of difficulties were evaluated. A total of 31 patients (22 women and 9 men) were included in the study. The age range of the patients was 60 years (SD ± 13 years). The mean duration of the difficulties was 30 months (SD ± 52 months). The aim of the following thesis is to present the case study of a patient during the rehabilitation process with a diagnosis of polytrauma; recurrent central vestibular syndrome, with special mention to diffused axonal injury from polytrauma. The document is divided into two (2) parts the general/theoretical part and the practical or case study part. Factors affecting vestibular compensation in patients after vestibular schwannoma surgery.

Vestibular schwannoma surgery causes in majority of patients the unilateral peripheral or combined vestibular lesion due to surgical interruption of both branches of vestibular nerve. It manifests postoperatively with postural instability, vertigo, oscilopsia and even with vegetative symptoms. Central compensation, in which cerebellum plays dominant role, influences duration of the symptoms. Factors affecting compensation may be divided into several groups (general health status of a patient, the tumor itself, actual state of function of vestibular system, external factors induced by a therapeutist). Therapeutic aim is to induce compensatory mechanisms as soon as possible and to reduce overall duration of the compensation. The aim of this study is to consider predictive factors influencing central vestibular compensation and to influence the vestibular compensation itself. Early operated vestibular rehabilitation is a basic procedure. It leads to faster recovery in majority of patients. Even with maximal effort the full compensation is not always achieved. It results in permanent deficit of vestibular function manifested with postural instability. This statement is based on the fact that vestibular rehabilitation serves as a prevention against falls and increases the overall fitness of the elderly. The practical part of the bachelor's thesis deals with case studies of three patients and the application of vestibular training to these patients. The main components of this training are elements of visual motor skills, eye-hand coordination, movement planning and balance training. Adéla Slámová Title: Use of vestibular rehabilitation in patients after stroke Abstract: The thesis focuses on the application of Vestibular Rehabilitation Therapy (VRT) in the treatment of Cerebrovascular accident (CVA). Annually, on average 50 thousand patients are hospitalised when diagnosed with CVA, of those only 25 % do not suffer the consequences and fully recover to their former health. Vestibular disorders account for one of the consequences of CVA. Patients suffer from dizziness, imbalance and the fear of falling down. To compensate for the deficiencies, VRT could be availed to improve stability, dizziness intensity, lower the risk of falling, ultimately leading to patient's social and occupational activity return. The thesis constitutes theoretical and practical parts. The former part is divided into 4 sections. The first section discusses the issue of CVA. The second one describes the vestibular system. The assessment of the compensation of stabilometric parameters in patients after vestibular schwannoma surgery

In this thesis, we have been dealing with the evaluation of progressing of the postural stability in the patients after the operation of the vestibular schwannoma (VS). The stability was evaluated by means of stabilometric parameters (COP trajectory length, COP confidence ellipse area). We measured the development of the parameters before operation, immediately after resection and before the discharge. The set contained 17 patients (n = 17). The level of static importance we determined p ≤ 0,05. We found a statistically significant difference between the measurement before the operation and the measurement immediately after the operation (COP confidence ellipse area: p = 0,0183, COP trajectory length: p = 0,0355). On the other hand, the change in both parameters in the measurement before the discharge and the testing after the resection wasn't statistically significant (COP confidence ellipse area: p = 0,4081, COP trajectory length: p = 0,9991). Our next goal was to find out if the stability fault correlates with the disorder of function VOR. We testing the function of t-VOR by the subjective visual vertical (SVV) (n = 24), the function of the r-VOR by the dynamic visual acuity (DVA) (n = 28) and the caloric test (n = 21).
